Book excerpt: Excerpt from Appendicitis, Its Pathology and Surgery Asked for an example of correct nomenclature, I should adduce the term tuberculous appendicitis. It implies the cause of the inflammation, the presence of the tubercle bacillus, and a sequence of morbid changes, including ulceration and, perchance, perforation of the appendix. It permits us to speak correctly of tuberculous ulceration and of tuberculous perforation, and, in either instance, the mind imagines a definite morbid process with a definite cause. The temptation is great to apply to certain cases of appendicitis. The term' which Niemeyer did so much to popularise. Doubtless, cases may occur which come within his definition, that 'catarrh consists in engorgement of the blood-vessels of any mucous membrane, accompanied by abnormal secretion, swelling, succulence of its tissues, and copious generation of young cells. 1 But I have found that modern methods of histological research almost invariably Show that'the appendicular catarrh is only a small part of the disease, and not the essential part. The belief in the existence. Of such a disease as catarrhal appendicitis might be an obstacle to the thorough investigation of appendicitis. Many of the cases which, on superficial examination, I used to think were simply catarrhal, have since proved to be cases of appendicitis with ulceration and bacterial invasion of the appendicular walls.2 When this work was begun, I thought catarrh of the mucous lining one of the commonest of the causes of appendicitis, and therefore, paid much attention to the condition of the epithelium; I suppose we may assume that the histological evidences of catarrh are (a) an excessive number of goblet. Cells, (6) shedding of epithelium, (c) hyperaemia, and, (d) perhaps, cell infiltration. But, hitherto, I have not met with a case in which these. Have been the sole histological changes. Book excerpt: Excerpt from Lectures on Appendicitis and Notes on Other Subjects There has been little to add to a contribution on the subject of the pathology of appendicitis which I offered to the profession at the meeting of the Pan-American Medical Congress in 1893 and published in the first edition of these lectures in 1895. Students of the subject of appendicitis in this country and in Europe have corroborated the testimony then offered without making addition to the principles or opposing the theories advanced. We may assume that the pathology of appendicitis is thoroughly understood, and that we need not look for many further additions to our knowledge upon that subject in the future. In operative methods I have adopted the McBurney blunt dissection operation for entering the abdominal cavity, but have made no other changes from the methods described in the second edition of the lectures. My statistics continue to show the advantages of leaving out of the technique of operation, features which have a special death-rate of their own. It is apparent to many observers of methods that gauze packing has a special death-rate of its own, that iodoform gauze packing has a special death-rate of its own, that the plans of ligating the appendix like an artery, of making long and multiple incisions, of leaving infected or concretion-bearing appendices among adhesions after opening one abscess, all have special death-rates of their own. At the time of publication of this edition I am firm in the conviction, based upon practical results, that the principles of treatment that were laid down in the first edition taught the avoidance of methods which make appendicitis operations unpopular with physicians in some parts of this country and in other countries. In this edition I have added several brief features which seemed to be to the point, namely: A note on the frequency of the occurrence of appendicitis. A series of summaries to facilitate an easy review of matters of leading importance. The testimony of ten consecutive appendicitis patients on their views of the comparison between medical and surgical treatment. A note on the attitude of ten physicians on the subject of appendicitis; and, A description of the formalin preparation of catgut. Available in PDF, EPUB and Kindle. Book excerpt: "The purpose of this book is to provide a 'ready reference' for pathologists and surgeons (and residents in training) on the pathology of the vermiform appendix and the surgical treatment of its disorders." "At present there is no single English-language monograph devoted entirely to this topic despite the fact that the appendix is the organ most commonly removed in intra-abdominal 'emergency' operations." "It is now over l00 years since the first surgical removal of an appendix for acute appendicitis. In this time there have been major advances in our understanding of pathological processes and a great increase in the availability of diagnostic tests and therapeutic agents, yet many purportedly normal appendices are still removed. This is partly due to the difficulty in localizing the source of intra-abdominal pain and the potential complications of acute appendicitis, but is also due to the fact that many other disease processes mimic the appendicitis syndrome and there is no specific diagnostic laboratory test for the disease." "This text covers the full range of disease processes in the appendix, both neoplastic and non-neoplastic. The development, structure and possible functions of the organ are discussed, as is the current state of knowledge regarding the pathogenesis of disease in the appendix. Other possible causes for the pain experienced in the appendicitis syndrome are considered and explanations commonly given for the causes of this pain are reviewed. The potential for non-surgical therapy is explored." "The book concludes with a review of the surgical treatment of pathological processes in the appendix, thus bringing together in one text both the pathology of the appendix and its treatment."--BOOK JACKET.Title Summary field provided by Blackwell North America, Inc. Available in PDF, EPUB and Kindle. Book excerpt: Although pediatric surgery is a distinct and evolving specialty, it still remains an integral part of most general surgical and paediatric medical practice. Nevertheless, surgery in children does differ from adult practice in various fundamental ways, and there are key physiological and anatomical differences that constantly need underlining. Progress and improvement in outcome has also been rapid but it is sometimes difficult for practitioners to keep themselves up-to-date with the usual surgical or paediatric text books. This book will give a concise overview of all important topics and is designed to provide information in order to recognise the common surgical conditions; namely typical symptoms and signs, investigation and then treatment management. It will also provide an anatomical and physiological background to aid understanding, in addition to emphasising logical, and where possible, evidence-based practice by the use of flow charts, tables and algorithms. Authored by an international range of leading contributors, this is the first book of its kind to offer comprehensive coverage to this topic in a quick reference, pocket-book format.

Available in PDF, EPUB and Kindle. Book excerpt: The second edition of Paediatric Surgery has been designed to be highly relevant to the practice of clinical paediatric surgery. With this in mind, topics are presented in a clear and easily digestible format, with the most important learning points in each area clearly identified. The principles of the basic sciences, and how these relate to competent clinical practice, are covered in each clinical chapter, and are an important part of the Intercollegiate Board examination and other higher examinations in paediatric surgery in Europe and Australasia. While many of the topics from the previous edition have been retained and updated as appropriate, there are a number of important additions. The important and developing field of minimally invasive surgery has been given more prominence. Oncology has been expanded to include more detail on solid tumours as well as more on central venous access and the surgical assessment of the acutely unwell child undergoing chemotherapy. Thoracic surgery has been expanded to include the management of empyema and the open and endoscopic approach to resection and biopsy. Finally, important 'generic' issues such as communication skills, the critical analysis of journal articles and evidence-based medicine have all been introduced. The second edition of Paediatric Surgery will be an invaluable resource for all trainee paediatric surgeons, providing a summary of practical, evidence-based advice for the candidate for the Intercollegiate Board Speciality Examination in Paediatric Surgery (FRCS Paeds), or other equivalent examination, without overwhelming and unnecessary detail.
