Book excerpt: This grant addresses the molecular mechanisms of resistance to the anti-estrogen, Tamoxifen, in breast cancers. Both estrogens and antiestrogens influence the cell cycle during the GI phase. In breast tumors, loss of the cdk inhibitor, p27, is associated with a poor prognosis and with steroid resistance. We postulate that altered p27 degradation in breast cancer may contribute to resistance to antiestrogen therapy. In the last year, we have assayed cell cycle effects of estradiol in the ER+ breast cancer cell line, MCF-7. MCF-7 arrests in G1 upon removal of estradiol or on addition of Tamoxifen. Upon re-addition of estradiol, cells re-enter the cell cycle with onset of S phase by 12 hours and loss of both p21 and p27 proteins. Cyclin D1 associated kinase activity rose transiently in early G1. Activation of cyclin E/cdk2 kinase in mid-late G1 was accompanied by loss of cyclin E-associated p21 and p27. The requirement for p27 in the quiescence induced by estradiol depletion was investigated using antisense p27 oligonucleotides. Introduction of the antisense oligonucleotides into quiescent estradiol depleted MCF-7 cells reduced p27 levels five fold. Despite the continued absence of estradiol, the p27 antisense treated group entered into S phase, with an S phase fraction of 28 % at 34 hours post-transfection, while control cells remained arrested. Thus, the loss of p27 was sufficient to mimic the effect of estradiol on G1-to-S phase progression in MCF-7 cells. These data suggest that loss of p27 is critical for estradiol dependant stimulation of breast cancer proliferation and an increase in p27 is essential for cell cycle arrest following an inhibition of estradiol signaling. Ongoing studies address how these effects are altered in steroid resistant breast cancer.

Unfortunately, ER positive tumors that are initially responsive, invariably acquire resistance to hormonal therapies (reviewed in (1)).: TAM-resistant tumors usually show continued expression of the ER (2, 3). Estradiol regulates cell proliferation and development in the mammary gland. The elucidation of mechanisms whereby estradiol:ER influences cell cycle regulators and how these are blocked by Tamoxifen is highly relevant to the development of new treatments for steroid resistant breast cancer. Both estrogens and antiestrogens influence the cell cycle during the early Ol phase (4). The cell cycle is governed by a family of cyclin dependent kinases (cdks), whose activity is regulated by positive effectors, the cyclins, by phosphorylation and by negative regulators, the cdk inhibitors (reviewed in (5-7)). p27 or kinase inhibitor protein 1 (KIP 1) is strongly expressed in normal mammary epfthelial cells (8). That p27 protein levels are frequently reduced in primary breast cancers and this correlates with poor prognosis, suggests that p27 is an important negative regulator of the normal breast cell cycle (8-10) . The cellular abundance of p27 is importantly regulated by ubiquitin-mediated proteolysis (11). p27 protein decreases when quiescent MCF-7 breast cancer cells are stimulated to reenter the cell cycle with estradiol treatment and p27 increases when antiestrogens induce Gi arrest ((12, 13) and PNAS manuscript appended).

In the two years since its publication, much new and exciting in formation has been published and the full interplay of regulatory mechanisms is now beginning to emerge. We have divided this book into four sections that we hope will unify important concepts and help to crystallize areas of consensus and/or disagreement among a diverse group of basic and clinical scientists working on the disease. The first section is devoted to studies on oncogenes, antioncogenes, proliferation, and tumor prognosis. The first chapter, by Sunderland and McGuire, introduces the characteristics of breast cancer as studied by patho logists to establish prognostic outcome. Of particular interest is a new proto oncogene called HER-2 (or neu), which is rapidly becoming accepted as a valuable new tumor marker of poor prognosis. The second chapter, by Lee Bookstein and Lee, introduces the best known antioncogene, the retinoblas toma antioncogene, whose expression is sometimes lost in breast cancer. Malignant progression appears to be influenced by the balance of proto oncogene and antioncogene expression.

Book excerpt: Previous evidence in our laboratory demonstrated the ability of estrogen to regulate specifically the protein kinase C (PKC) $\delta$ isoform in the rat and rabbit corpus luteum. The purpose of this study was to evaluate estrogen's ability to regulate PKC, specifically the PKC $\delta$ isoform, in the estrogen responsive, MCF-7 human breast cancer cell line. With the increasingly characterized role of PKC $\delta$ in growth regulation, we were interested to also determine the ability of PKC $\delta$ to regulate growth in the MCF-7 cell line. Treatment of MCF-7 cells with 10$\sp{-10}$ through 10$\sp{-8}$ M estrogen for 7 days down-regulated specifically PKC $\delta$ mRNA and protein; expression of other PKC isoforms was unchanged. These concentrations of estrogen were maximally proliferative for the MCF-7 cells and an inverse correlation between PKC $\delta$ expression and proliferation in MCF-7 cells was observed. PKC $\delta$ protein levels were found to be 10 fold higher in MCF-7 compared to the estrogen receptor-negative, MDA-MB 231 cells which grow aggressively in the absence of estrogen. In view of the inverse correlation between PKC $\delta$ expression and proliferation in MCF-7 cells we wished to determine whether activation of PKC $\delta$ could signal growth inhibition. Phorbol ester treatments that lead to G1 growth arrest and induction of the cyclin-dependent kinase inhibitor p21$\rm\sp{Waf1/Cip1}$ in MCF-7 cells promoted activation of PKC $\delta$, as evidenced in immunecomplex phosphorylation assays. To show that phorbol ester stimulated G1 growth arrest could be mediated by the induction of p21$\rm\sp{Waf1/Cip1}$ by activated PKC $\delta,$ we demonstrated that constitutively active PKC $\delta$ in the absence but not in the presence of dominant negative PKC $\delta$ activated the WAF1/CIP1 promoter-luciferase construct in transient transfection assays. Activated PKC $\delta$ can therefore signal the transcriptional induction of p21$\rm\sp{WAF1/CIP1}.$ Our results are consistent with the hypothesis that modulation of PKC $\delta$ expression and activation state provides a pathway for growth regulation in breast cancer cells.

Currently, subtyping breast cancers into hormone receptor (HR) positive, human epidermal growth factor receptor-2 overexpressing (HER2+), and triple negative breast cancer (TNBC) is the basis of diagnosing and treating this disease. The main treatment strategies for breast cancer include surgery, endocrine therapy, molecular targeted therapy, chemotherapy, radiotherapy, immunotherapy and gene therapy. However, resistance of breast cancer cells to chemotherapeutic agents, molecular targeted therapies and immunotherapy may occur either intrinsically or de nova, and is often ultimately responsible for treatment failure. Therefore, drug resistance poses a major challenge to breast cancer treatment. Current developments: Drug resistance in breast cancer is a complex clinical condition originating from a wide range of molecular alterations. The development of endocrine therapy resistance is believed to be associated with many cellular changes, such as ESR1 gene mutations, bypassing estrogen signaling pathway and altered tamoxifen metabolism. Meanwhile, changes in immune response, alternation of drug-binding property and downstream pathways are involved in the mechanisms of drug resistance in HER2+ breast cancer. In addition, resistance to chemotherapeutic agents predominantly arises from increased drug efflux and cross resistance. Current studies suggest that treatment strategies and therapeutics have to be designed specifically to each patient in different clinical situations. The use of modern genomic, proteomic and functional analytical techniques has contributed to identify novel genes and signaling networks involved in breast cancer drug resistance. Moreover, the use of high-throughput techniques in combination with bioinformatics and systems biology approaches has aided the interrogation of clinical samples and allowed the identification of molecular signatures and genotypes that predict responses to certain drugs. Despite much progress has been made in the field of breast cancer drug resistance, such as combination therapy and drug-loaded nanoparticles, the complexity and variability of drug resistance mechanism still inevitably lead to the continuous occurrence of drug resistance. Therefore, with the increasing amounts of anti-breast cancer agents, there are now unprecedented opportunities to understand and overcome drug resistance through further research into mechanisms and corresponding strategies, which will help achieve lasting disease control and bring survival benefits to patients with advanced cancer.
