Book excerpt: With the growing age of the population, the proportion of elderly patients with diffuse coronary artery disease and severe comorbidities is increasing. The characteristics of these patients render them unsuitable for currently recognized revascularization procedures (CABG, PTCA). Therefore, there is an urgent need for new treatment modalities which could alleviate their symptoms and increase blood flow to the ischemic myocardium, while being less invasive for their fragile condition. The aim of this book is to provide an anatomical and physiological background as well as a complete review of the alternative methods of revascularization which are under development, namely transmyocardial laser revascularization, therapeutic angiogenesis and neurostimulation.

Book excerpt: An interdisciplinary panel of pioneers and opinion leaders review the basic, preclinical, clinical, and developmental pathways to new treatment strategies, such as therapeutic angiogenesis and myogenesis. The authors take advantage of new biological understanding, novel therapeutic targets, multiple available and well-studied therapeutic strategies, and the necessary imaging techniques to measure outcomes. Their in-depth discussions cover the identification of new therapeutic targets and pathways, the investigation of transcriptional factors, master switch molecules, cell-based approaches, chemokines, a better understanding of the effects of aging, endothelial dysfunction, and hypercholesterolemia in response to angiogenic stimuli. Highlights include examination of drug delivery problems, outcomes measure, stem therapy, high-risk interventions, development pathways, and future possibilities.

This book was released on 2012-12-06 with total page 248 pages. Available in PDF, EPUB and Kindle. Book excerpt: The last five years have witnessed an increasing interest in the subject of transmyocardial laser revascularization (TMR) as illustrated by the number of abstracts presented at the meetings of the American Heart Association and the American College of Cardiology (Figure). The ideas and concepts associated with this particular method of myocardial revascularization have changed dramatically over even this short period of time. The original premise of "de-evolving" mammalian hearts to recreate a reptilian-like myocardial circulation by making multiple channels through the myocardium has been almost (but perhaps not quite) completely dismissed. Now, the most popular notion is that there is an angiogenic response to myocardial channel making. It is this development of new blood vessels that is thought to be responsible for the apparent improvements in symptoms and blood flow. Along the way, the idea that a channel could stay open and allow blood to flow directly from the ventricular chamber has found little support. Rather than directly explore all of these issues and merely duplicate previously published articles, our aim was to take a novel approach: that is, to step back from these arguments and provide perspective from the vantage point of distance. In the case of trans myocardial revascularization, distance comes both in terms of history and in terms of methodology and knowledge from other fields of research. Historically, innovative methods of myocardial revascularization are by no means uncommon. The first two chapters deal with this historical' perspective.

Book excerpt: The percutaneous method of transluminal myocardial revascularization has roused enthusiasm in the field of cardiology because it is minimally invasive and relatively inexpensive. In his progressive new book, George Abela, a widely recognized pioneer in cardiology and lasers, has brought together the expertise of international leaders on the subject in this premier reference, Myocardial Revascularization: Novel Percutaneous Approaches. This resource provides interventional cardiologists, technical personnel and nurses in cardiac catheterization labs with the most current scholarly and clinical information available, including: * A historical overview of myocardial revascularization (including the Vineberg procedure and the more recent TMR/PMR procedures) * Biological responses and mechanisms of action in myocardial revascularization using mechanical, laser, ultrasound, and cryoenergy * Current technology for the percutaneous method of the procedure * Descriptions of other novel approaches including percutaneous in situ coronary venous arterialization * Discussion of results from preliminary and advanced TMR/PMR in canine and human procedures * Examination of various catheter navigation systems and potential adjunct gene therapies * Myocyte transplantation to replenish the depleted myocardium * Insights into the economics of these novel percutaneous techniques

Book excerpt: The rapid development of myocardial revascularization from angioplasty to stents, and now to new laser technology has meant that there is an abundance of information available which has not been distilled into a user-friendly format. Confusion exists as to where we are with angiogenesis: what can it do? how do we do it? what is acceptable as a satisfactory result? Drs Kornowski, Epstein and Leon answer the need for a concise reference on the subject. They have assembled a team of internationally-recognised experts who are at the forefront of research and clinical application on myocardial revascularization and angiogenesis. They examine direct, transmyocardial and percutaneous approaches to revascularization. In the latter part of the Handbook, there is discussion about growth factors protein and gene therapy which should serve as a guide and provide background facts so as to ensure greater informational access to the interventional cardiologist.

Book excerpt: Miniswine underwent procedures to evaluate treating chronic ischemia with the implantation of autologous satellite cells and laser transmyocardial revascularization (TMR). The objective was to combine two therapies to restore cardiac function. This experiment involved three surgical procedures: (1) placing a constrictor on the coronary artery; (2) producing channels and implanting cells; (3) obtaining samples. The swine were divided into groups: Group 1, Ischemia; Group 2, Ischemia + Laser TMR; Group 3, Ischemia + Laser TMR+ Cells; Group 4, Ischemia + Cells. Sonomicrometry and Millar pressure transducers were used to determine contractility, left ventricular pressure, and pressure-volume loops. There were no significant differences (p

Specifically, the laser’s unique physical properties and precise bio-tissue interactions render this versatile source of biologic energy an attractive tool for multiple therapeutic purposes in cardiovascular medicine. Over the course of the last 2 decades the utilization of laser technology has become an important component for the management of patients with complex cardiovascular diseases. During this time period, cutting edge laser technology including a variety of wave length generators, newly designed catheters, and a selection of advanced optic fibers have been introduced and applied in the cardiovascular circulation. Improved lasing techniques in the cardiac catheterization suites and operation rooms have been implemented for treatment of ischemic coronary syndromes, peripheral arterial occlusive disease and other atherosclerotic thrombotic conditions. Moreover, during this 20 year time frame, several multicenter and single center clinical studies have been published focusing on the role and utilization of lasers in coronary and peripheral revascularization. And within the rapidly expanding field of interventional cardiac electrophysiology, laser technology has recently revolutionized the management of fractured, abandoned and malfunctioning leads of cardiac pacemakers and automatic defibrillators. Consequently, replacing a notoriously cumbersome and high risk open heart surgery with safe and markedly efficient percutaneous laser based extraction. This textbook will provide the most authoritative, comprehensive and contemporary information covering technological progress, clinical experience and pertinent aspects of laser applications in cardiovascular medicine. It will be of interest to cardiologists, vascular surgeons and interventional radiologist as well as medical students, scientists, biomedical engineering students and graduates.

Book excerpt: Nuclear cardiology is no longer a medical discipline residing solely in nuclear medicine. This is the first book to recognize this fact by integrating in-depth information from both the clinical cardiology and nuclear cardiology literature, and acknowledging cardiovascular medicine as the fundamental knowledge base needed for the practice of nuclear cardiology. The book is designed to increase the practitioner's knowledge of cardiovascular medicine, thereby enhancing the quality of interpretations through improved accuracy and clinical relevance. The text is divided into four sections covering all major topics in cardiology and nuclear cardiology: Basic Sciences and Cardiovascular Diseases Conventional Diagnostic Modalities Nuclear Cardiology Management of Cardiovascular Diseases
