Dans ce but, pour compléter la littérature, une étude a été menée sur un échantillon de patients volontaires pour évaluer la sécurité de cette technique en surveillant les paramètres vitaux. Les résultats démontrent que cette pratique est envisageable en toute sécurité si l’on respecte certaines conditions. Cette alternative est à réserver à des patients sains, peu anxieux et doit être réalisée par un praticien expérimenté.

Book excerpt: L’@avulsion des 4 dents de sagesse peut se dérouler selon différentes modalités et le plus souvent, l’anesthésie générale est la solution retenue.Le but de cette thèse est d’étudier si l’avulsion de 4 dents de sagesse, en ambulatoire, sous anesthésie locale stricte, en utilisant le chlorhydrate d’articaïne à 4% adrénalinée à 1/200 000ème, à la dose maximale recommandée (7mg/Kg ou sept cartouches de produit) expose ou non à un risque de toxicité systémique.6 patients ASA 1, non fumeurs sont retenus pour recevoir deux injections de 3,5 cartouches espacées de 30 minutes chacune. 1 patient, répondant aux mêmes critères est retenu pour recevoir 7 cartouches en injection unique. Des prélèvement sanguins sont réalisés dans les deux cas à des intervalles réguliers afin de doser la concentration plasmatique de l’articaïne.Aucuns signes de surdosage, de manifestation de toxicité ou d’allergie ne sont rapportés.A partir des courbes temps-concentrations plasmatiques, les principaux paramètres pharmaccinétiques de l’articaïne sont analysés. Les pricipales caractéristiques pharmacocinétiques de l’articaine après infiltration sous-muqueuse sont une vitesse de résorption et d’élimination très rapide ; un Tmax voisin de 10 min et une demie-vie d’élimination de l’ordre de 30 minutes. Le choix de l’articaïne comme anesthésique local pour réaliser l’avulsion de 4 dents de sagesse est particulièrement adaptée à ce type d’intervention chirurgicale de par ses propriétés pharmacocinétiques permettant de limiter au mieux le risque de toxicité systémique.
