Its development and progress, for various reasons, have been slow, principally because the anesthetist must have an accurate knowledge of anatomy and a high degree of technical skill in order that the anesthesia may be safe and satisfactory, and the operation not delayed. Heretofore, few surgeons have given the time necessary to master the method of administration, and the details have been carried out by assistants, equally unqualified. In order to overcome certain inherent defects in regional anesthesia it is necessary in certain cases and desirable in many to use some form of general anesthesia with the regional. In chronic abdominal disease general intraperitoneal exploration is desirable for the purpose of locating and, if possible, removing all lesions at the primary operation. The entire peritoneum is sensitive and in such operations it is impossible to secure, with regional anesthesia, the relaxation and freedom from pain that are essential; A combination of regional anesthesia and a little ether given at the proper time is satisfactory, although in abdominal surgery general ether anesthesia is so satisfactory that most surgeons are content not to use the regional.
