Book excerpt: This study reports on the developmental functioning levels of children from birth through age 2 in early intervention services and children ages 3-5 in early childhood special education services at the time of entry into services, using data from the Oregon Early Childhood Assessment System. The assessment system contains data on the assessed developmental functioning levels of children based on the 16 Oregon early childhood foundation areas and the three U.S. Office of Special Education Programs federal reporting child outcome areas. The study analyzes developmental functioning data and key demographic variables (primary disability, gender, race/ethnicity, and primary home language) for the entire population of children in early intervention and early childhood special education services who received services over a 13-month period during 2006/07. The study results show the percentage of children in early intervention and early childhood special education services assessed as functioning below age-expected skill levels on skills assessments using age-related criteria embedded in the Oregon Early Childhood Assessment System database. With few exceptions, the developmental skills that were most often assessed as functioning below age-expected skill levels among children across all demographic subgroups were those important to school readiness in literacy and mathematics. Appended are: (1) The Oregon Early Childhood Assessment System; (2) Research on early intervention and early childhood special education services; and (3) Study methods and limitations. Book excerpt: Background: Current policy in Oregon under the Individuals with Disabilities Education Act (IDEA) indicates that children diagnosed with developmental delay (DD) are eligible for services before school, but not eligible for the receipt of services in school-based programs. Due to this definition of eligibility, children with DD face additional barriers transitioning from early intervention/early childhood special education into school-based education services. Purpose: The present study investigated the relationship between enrollment in school-based special education programs given a change in primary disability diagnosis before or after the age of five in children originally diagnosed with DD. Methods: Children met eligibility criteria for the present study if they were enrolled in early intervention/early childhood special education services in the State of Oregon with a primary disability diagnosis of DD and had a change in primary disability diagnosis before third grade (n= 5,076). Results: Odds of enrollment in special education during school were greater in children with a change in primary disability diagnosis after the age of five in comparison to children that had a change in primary disability diagnosis before the age of five, while adjusting for demographic characteristics (diagnosis after age five adjusted odds ratio: 2.37, 95% CI 1.92, 2.92). Conclusion: The results of this study suggest that children who exit IDEA because they have met the maximum age of eligibility are likely to reenter the special education system after a gap in the receipt of service access. A gap in service access during this period of development can have great implications on later life success.

Book excerpt: There is increasing evidence that the first few years after birth are particularly important in child development and present opportunities for enrichment but also vulnerabilities do to poverty and other social stressors. Elected officials have begun proposing potentially costly programs to intervene early in the lives of disadvantaged children. Have such interventions been demonstrated to yield substantial benefits? To what extent might they pay for themselves through lower welfare and criminal justice costs incurred by participating children as they grow into adults? This study synthesizes the results of a number of previous evaluations in an effort to answer those questions. Conclusions are that under carefully controlled conditions, early childhood interventions can yield substantial advantages to recipients in terms of emotional and cognitive development, education, economic well-being, and health. (The latter two benefits apply to the children's families as well.) If these interventions can be duplicated on a large scale, the costs of the programs could be exceeded by subsequent savings to the government. However, the more carefully the interventions are targeted to children most likely to benefit, the more likely it is that savings will exceed costs. Unfortunately, these conclusions rest on only a few methodologically sound studies. The authors argue for broader demonstrations accompanied by rigorous evaluations to resolve several important unknowns. These include the most efficient ways to design and target programs, the extent to which effectiveness is lost on scale-up, and the implications of welfare reform and other safety net changes.

Ten years later, the Board on Children, Youth, and Families of the Institute of Medicine (IOM) and the National Research Council (NRC) held a 2-day workshop in Washington, D.C., to review and commemorate a decade of advances related to the mission of the report. The workshop began with a series of highly interactive breakout sessions in which experts in early childhood development examined the four organizing themes of the original report and identified both measurable progress and remaining challenges. The second day of the workshop, speakers chosen for their diverse perspectives on early childhood research and policy issues discussed how to build on the accomplishments of the past decade and to launch the next era in early childhood science, policy, and practice. From Neurons to Neighborhoods: An Update: Workshop Summary emphasizes that there is a single, integrated science of early childhood development despite the extent to which it is carved up and divided among a diversity of professional disciplines, policy sectors, and service delivery systems. While much work still remains to be done to reach this goal, the 2010 workshop demonstrated both the promise of this integrated science and the rich diversity of contributions to that science.
