Objectif : évaluer l’impact d’une politique de promotion de l’allaitement maternel mise en place dans un service de médecine néonatale pour accompagner les mères de nouveau-nés prématurés. Patients et méthode : l’étude était hospitalière et prospective. Elle incluait tous les couples mères-enfants dont les mères exprimaient à la naissance le désir d’allaiter et dont les enfants étaient nés à un âge gestationnel compris entre 24 et 34 semaines d’aménorrhée. Les différentes actions entreprises pour soutenir les mères dans leur projet d’allaitement et de formation de l’équipe soignante étaient menées par une puéricultrice consultante en lactation. L’étude s’étendait de l’année 2005 à l’année 2007 et comparait ces 2 années. Les critères de jugement principaux étaient le taux d’allaitement à la sortie et la durée d’allaitement. Les critères de jugement secondaires, appréciant la mise en route de la lactation, étaient l’utilisation du kit double pompage sur le tire-lait électrique, le délai de première expression du lait maternel, le délai du premier portage en peau à peau et le délai de première mise au sein. Résultats : Durant la période étudiée, 830 enfants sont nés entre 24 et 34 SA et ont été hospitalisés en médecine néonatale : 268 en 2005, 287 en 2006 et 275 en 2007. Les populations étaient comparables pour l’âge gestationnel, le poids de naissance et la catégorie socioprofessionnelle des mères. Le désir exprimé d’allaitement est resté stable (70% vs 75%, p=0,27) de même que le taux d’allaitement maternel à la sortie (83.1 % vs 80.0%, p= 0,62). La durée d’allaitement maternel a augmentée de façon significative, de 13,0±9,8 à 21,6±19,1 semaines (p=0,01). Cette augmentation a été observée aussi bien pour l’allaitement maternel exclusif (11,4±8,7 vs 15,3±11,0 semaines, p0,01) que pour l’allaitement partiel (3,6±2,2 vs 11,9±14,0 semaines, p0,01). Celles-ci s’étaient appuyées sur des référentiels théoriques validés et étaient pilotées par un pédiatre néonatologiste, un cadre de santé et une consultante en lactation. Les données étaient recueillies de façon prospective du 1er Janvier 2005 au 31 Décembre 2008 et de façon rétrospective du 1er Janvier 2011 au 31 Août 2012. Les critères de jugement principaux étaient le taux d’allaitement à la sortie et la durée d’allaitement. Les critères de jugement secondaires, témoins de la mise en route de la lactation, étaient le délai de première expression, le délai du premier peau à peau et le délai de première mise au sein. Résultats: 549 enfants étaient inclus durant la période d’étude. Les populations étaient comparables pour l’âge gestationnel, le sexe, le poids de naissance, l’âge des mamans et la parité. Le désir d’allaitement augmentait de manière significative entre 2005 et 2012 (67.8 % vs .8 %, p=0.001) contrairement au taux d’allaitement à la sortie d’hospitalisation (84 % vs 64 %, p=0.06).
