Book excerpt: Excerpt from An Oration on Certainty in Medicine: Delivered Before the Philadelphia Medical Society, February 10, 1830 Allow me to grace the following paper by prefixing to it the name of one, honoured by my country as her first and favourite surgeon, who was my father's friend, and I feel authorized to add, my own. In the chec quered course of a professional life, there is no better or more honourable gratification than the approbation of those who have risen to high eminence by the means, acknowledged by every one, of scientific merit and sterling integrity. That Dr physick may long con tinne to enjoy the honours and repose of age, earned before the time, unassailed by envy or reproach, is the earnest wish of thousands of his fellow citizens, and of none more so than of the author of these sheets. Book excerpt: This volume is a collection of 18 papers on the communication of certainty and uncertainty. The first part introduces recent theoretical developments and general models on the topic and its relations with modality, subjectivity, inter-subjectivity, epistemicity, evidentiality, hedging, mitigation and speech acts. In the second part, results from empirical studies in medical and supportive contexts are presented, all of which are based on a conversational analysis approach. These papers report on professional dialogues including advice giving in gynecological consultations, breaking diagnostic bad news to patients, emergency calls, addiction therapeutic community meetings and bureaucratic-institutional interactions. The final part concerns the qualitative and quantitative analysis of corpora, addressing scientific writing (both research and popular articles) and academic communication in English, German, Spanish and Romanian. The collection is addressed to scholars concerned with the topical issues from a theoretical and analytical perspective and to health professionals interested in the practical implications of communicating certainty or uncertainty.

Book excerpt: Excerpt from An Inquiry Into the Degree of Certainty in Medicine: And Into the Nature and Extent of Its Power Over Disease The main title of my inquiry is the same as that of the very elegant and classical essay of Cabanis, published half a century ago. I adopt it for the simple reason that there is no other at all appro priate. Since writing my own essay I have read, for the first time for nearly twenty years, the work of Cabanis. Besides the differences that must almost unavoidably exist, in the conception and treatment of a complex and difficult question by different minds, I may be permitted to say, that the leading and principal design and purpose of the two works are not exactly identical. The essay of Cabanis is more elementary than my Inquiry is. About the Publisher Forgotten Books publishes hundreds of thousands of rare and classic books. Available in PDF, EPUB and Kindle. Book excerpt: A “finely gifted writer” shares “fifteen brilliantly written episodes covering the years from studenthood to the end of medical residency” (Oliver Sacks, MD, author of The Man Who Mistook His Wife for a Hat) Singular Intimacies is the story of becoming a doctor by immersion at Bellevue Hospital, the oldest public hospital in the country—and perhaps the most legendary. It is both the classic inner-city hospital and a unique amalgam of history, insanity, beauty, and intellect. When Danielle Ofri enters these 250-year-old doors as a tentative medical student, she is immediately plunged into the teeming world of urban medicine: mysterious illnesses, life-and-death decisions, patients speaking any one of a dozen languages, and overworked interns devising creative strategies to cope with the feverish intensity of a big-city hospital. Yet the emphasis of Singular Intimacies is not so much on the arduous hours in medical training (which certainly exist here), but on the evolution of an instinct for healing. In a hospital without the luxury of private physicians, where patients lack resources both financial and societal, where poverty and social strife are as much a part of the pathology as any microbe, it is the medical students and interns who are thrust into the searing intimacy that is the doctor-patient relationship. In each memorable chapter, Ofri’s progress toward becoming an experienced healer introduces not just a patient in medical crisis, but a human being with an intricate and compelling history. Ofri learns to navigate the tangled vulnerabilities of doctor and patient—not to simply battle the disease.

Book excerpt: Set against the unexplained stroke of the author’s newborn daughter, this stunning, unflinchingly honest memoir is a thought-provoking reflection on uncertainty in medicine and in life. Growing up as the daughter of a dedicated surgeon, Elizabeth L. Silver felt an unquestioned faith in medicine. When her six-week-old daughter, Abby, was rushed to the Neonatal Intensive Care Unit with sudden seizures, and scans revealed a serious brain bleed, her relationship to medicine began to change. The Tincture of Time is Silver’s gorgeous and haunting chronicle of Abby’s first year. It’s a year of unending tests, doctors’ opinions, sleepless nights, promising signs and steps backward, and above all, uncertainty: The mysterious circumstances of Abby’s hospitalization attract dozens of specialists, none of whom can offer a conclusive answer about what went wrong or what the future holds. As Silver explores what it means to cope with uncertainty as a patient and parent and seeks peace in the reality that Abby’s injury may never be fully understood, she looks beyond her own story for comfort, probing literature and religion, examining the practice of medicine throughout history, and reporting the experiences of doctors, patients, and fellow caretakers. The result is a brilliant blend of personal narrative and cultural analysis, at once a poignant snapshot of a parent’s struggle and a wise meditation on the reality of uncertainty, in and out of medicine, and the hard-won truth that time is often its only cure. Heart-wrenching, unflinchingly honest, and beautifully written, The Tincture of Time is a powerful story of parenthood, an astute examination of the boundaries of medicine, and an inspiring reminder of life’s precariousness.

Book excerpt: Excerpt from Oration on the Guidance of a Sound Philosophical Spirit in the Investigations of Medical Science: Read Before the Cincinnati Medical Society, January 4, 1837 Fundamental to the whole host of follies and errors which infested medical science for so long a succession of years, was this one great, pervading, ruinous misconception - that truth could be elicited by the subtle workings of the mind, independent of the teachings Of experience, - that the man of genius could elaborate in the workshop of his own acute and vigorous faculties a just and satisfactory ex planation of all the phenomena of nature. Deserting the only path of all correct views of the anatomy and physiology of man, and of the diseases to which his body is liable - the path of observation they indulged in the most extravagant conceptions of the organiza tion and actions of the living animal economy. To be sure the plan of attaining their fancied elevation Of knowledge was a very com penduous one, - in which no task was imposed on their industry of reseach'! Immured in the closet, each one could fabricate at pleas ure, the gossamer webs of his own Speculation, and spangle them over with the gaudiest tinsel of a false Iogic. We glory in the present more certain state of medical science; we rejoice in the presence and agency of a sound philosopical Spirit, which is every where in busy, ceaseless force and activity in the discovery of new truths, the detection of old errors, and the addi tional illumination of the admitted doctrines and facts of our science. Our minds, in confiding trust of the onward march of medical knowledge to still higher and higher approximations to certainty, dart forward to the disclosures of coming years. We feel a rejoicing satisfaction, dilating our bosoms, as we contemplate the triumphant progress of medicine in future years. Guided and inspirited by the genius of a sound, authentic, elevating philosophy, medicine will achieve still greater victories over disease, and diffuse in a still wider and wider circle the benefits of its curative influence. Book excerpt: The idea of preparing a new critical edition of Elisha Bartlett’s Essay on the Philosophy of Medical Science was suggested to me several years ago by Dr. H. Tristram Engelhardt, Jr. Since that time it has been a pleasure to get to know the life and work of Elisha Bartlett. I am pleased to be completing this book in the bicentennial year of Bartlett’s birth. Bartlett was born in 1804 in Smithfield, Rhode Island, less than twenty-five miles from Worcester, Massachusetts, my present home—a short journey even in Bartlett’s day. I have been able to walk at some of the sites to which Bartlett continually returned during his life. Visiting Bartlett’s grave in the Slatersville cemetery has been an inspiration for the preparation of this book. Proximity to several institutions with rich holdings in Bartlett’s works and in nineteenth-century American history of medicine greatly facilitated my research. First, though, I want to acknowledge the College of the Holy Cross for supporting my sabbatical leave for the academic year 2003-2004. The American Antiquarian Society, in Worcester, Massachusetts, was generous in giving me access to its remarkable resources. I was able to find many of Bartlett’s published works and other nineteenth-century medical literature there, and the entire library staff provided quick and able research assistance.

Available in PDF, EPUB and Kindle. Book excerpt: Students of the history of medicine and of American history in general will welcome this collection of thirty papers originally published in nineteenth-century medical journals and lay publications. Each highlights a specific problem or medical attitude of the period, and together they present an illuminating panorama of the medical profession and of public health in nineteenth-century America. Many of the problems faced by students, practitioners, and patients of the last century are surprisingly similar to those still being encountered today. Dr. Brieger has selected papers that illustrate the issues and developments in medical education, medical practice, surgery, hospitals, hygiene, and psychiatry. They range from Benjamin Rush's "On the Cause of Death in Diseases That Are Not Incurable," to a paper by Robert F. Weir "On the Antiseptic Treatment of Wounds, and Its Results" and an article by Stephen Smith, "New York the Unclean." The final selection, the Announcement of The Johns Hopkins Medical School, stands as a landmark that foretells the beginning of a new era.
