Book excerpt: Posttraumatic stress disorder (PTSD) is one of the signature injuries of the U.S. conflicts in Afghanistan and Iraq, but it affects veterans of all eras. It is estimated that 7-20% of service members and veterans who served in Operation Enduring Freedom and Operation Iraqi Freedom may have the disorder. PTSD is characterized by a combination of mental health symptoms - re-experiencing of a traumatic event, avoidance of trauma-associated stimuli, adverse alterations in thoughts and mood, and hyperarousal - that last at least 1 month and impair functioning. PTSD can be lifelong and pervade all aspects of a service member's or veteran's life, including mental and physical health, family and social relationships, and employment. It is often concurrent with other health problems, such as depression, traumatic brain injury, chronic pain, substance abuse disorder, and intimate partner violence. The Department of Defense (DoD) and the Department of Veterans Affairs (VA) provide a spectrum of programs and services to screen for, diagnose, treat for, and rehabilitate service members and veterans who have or are at risk for PTSD. The 2010 National Defense Authorization Act asked the Institute of Medicine to assess those PTSD programs and services in two phases. The Phase 1 study, Treatment for Posttraumatic Stress Disorder in Military and Veteran Populations: Initial Assessment, focused on data gathering. Treatment for Posttraumatic Stress Disorder in Military and Veteran Populations Final Assessment is the report of the second phase of the study. This report analyzes the data received in Phase 1 specifically to determine the rates of success for each program or method. Treatment for Posttraumatic Stress Disorder in Military and Veteran Populations Final Assessment considers what a successful PTSD management system is and whether and how such a system is being implemented by DoD and VA. This includes an assessment of what care is given and to whom, how effectiveness is measured, what types of mental health care providers are available, what influences whether a service member or veteran seeks care, and what are the costs associated with that care. This report focuses on the opportunities and challenges that DoD and VA face in developing, implementing, and evaluating services and programs in the context of achieving a high-performing system to care for service members and veterans who have PTSD. The report also identifies where gaps or new emphases might be addressed to improve prevention of, screening for, diagnosis of, and treatment and rehabilitation for the disorder. The findings and recommendations of Treatment for Posttraumatic Stress Disorder in Military and Veteran Populations: Final Assessment will encourage DoD and VA to increase their efforts in moving toward a high-performing, comprehensive, integrated PTSD management strategy that addresses the needs of current and future service members, veterans, and their families.

Book excerpt: Prolonged exposure therapy (PE; Foa & Rothbaum, 1998) is an evidenced-based psychotherapy that operates under the emotional processing theory for anxiety disorder, which integrates components from conditioning theory, cognitive therapy, and information processing theories to explain the mechanisms underlying posttraumatic stress disorder (PTSD) and the process by which exposure treats this disorder. PE has been demonstrated to be an efficacious treatment for veterans and has led to large effect sizes in the reduction of PTSD symptoms (Crowley et al., 2013). Despite its effectiveness, treatment dropout rates for veterans engaged in this therapy are alarming. Given the dire need for efficacious PTSD treatments available to members of the veteran population, the purpose of this study was to examine clinician perspectives on veteran dropout variables that were identified through a literature review. This study also considered possible interventions to decrease treatment dropout and assesses clinician perspectives on the usefulness of such interventions. Participants included clinicians who have administered prolonged exposure therapy to veterans within a mental health setting. Clinicians were recruited through the use of the Pennsylvania Psychological Association's (PPA) listserv, as well as the American Psychological Association's (APA) Division 19 listserv. Data was collected through an original online survey which included: participant demographic information, clinician perspectives regarding PE dropout variables that have been identified in the literature, and clinician perspectives regarding interventions that have been identified in the literature as being effective in decreasing veteran dropout rates. Results of this study identified that participants have a good baseline knowledge of PE dropout variables and interventions to reduce dropout. Furthermore, due to the limited number of participants, further research is needed to assess the generalizability of this data. However, treatment modification recommendations were provided based on the results of the literature and research.

Book excerpt: Combat and war zone veterans are particularly vulnerable to developing Posttraumatic Stress Disorder (PTSD) due to the increased risk of experiencing trauma inherent in military service. Insomnia and nightmares are core symptoms of PTSD and can be factors in the development, maintenance, and exacerbation of PTSD. However, sleep disturbance has received relatively little attention from a treatment point of view until recently. Recent research has demonstrated that cognitive behavioral therapy for insomnia (CBT-I) and imagery rehearsal therapy (IRT) for nightmares are effective stand alone and combination treatments for sleep disturbance in civilian and veteran populations. Although group interventions are lower in cost and more efficient to deliver in a VA setting, studies have yet to test the feasibility and efficacy of group CBT-I/IRT combination treatment for veterans with PTSD. The current pilot study investigated the feasibility and efficacy of a combined CBT-I and IRT protocol intervention for insomnia and nightmares delivered in a group format to a sample of mixed theater veterans with PTSD. Participants (n = 34) were randomly assigned to either a treatment (n = 17) or waitlist control condition (n = 17). After baseline assessment, participants assigned to the treatment condition participated in six weekly group sessions of combined CBT-I and IRT and completed posttreatment and two month follow-up assessments. Those in the waitlist condition completed a six week waiting period. After completing post-waitlist assessment, they were given the opportunity to receive the treatment. Authors hypothesized that those in the treatment condition would experience significant improvement on self-report measures of sleep, nightmare frequency, PTSD-related sleep disturbance, beliefs about sleep, and overall PTSD and depression severity compared to the waitlist condition at posttreatment and two-month follow-up. These hypotheses were largely supported by the study results with some notable exceptions. Significant condition by time interactions were found on measures of sleep and nightmares, beliefs about sleep, and depression at posttreatment. Treatment gains were maintained at two month follow-up for sleep variables but not nightmare frequency or depression. There were no condition by time interactions for measures of PTSD or PTSD-related sleep disturbance at posttreatment. In sum, the findings of this study indicate that a combined CBT-I and IRT delivered in a group format was effective in reducing insomnia symptoms, nightmare frequency, and depression in a sample of mixed theater veterans with PTSD. The study also demonstrated the feasibility of this treatment approach with this population suggesting that a larger randomized controlled study is warranted.

Book excerpt: Emerging research suggests that childhood adversities may increase both the risk and symptomology of posttraumatic stress disorder (PTSD) in our veteran population. Over 40% of our reintegrating military veterans return with significant mental health issues led by combat-related PTSD. PTSD impacts veterans in numerous areas including unemployment, increased criminal justice involvement, increased treatment costs, divorce, co-morbid mental illness, greater levels of domestic violence, homelessness, high college dropout rates, suicide, and long term health problems. The purpose of this study is to investigate the impact of childhood adversities (abuse, neglect, and poverty) upon the severity of combat-related PTSD in veteran populations. Specifically, the researcher examines the direct effects of: (1) childhood trauma; (2) childhood neglect; and (3) childhood poverty (as assessed based on socioeconomic status (SES)) upon the severity of combat-related PTSD. This study of student veterans (n=102) receiving services from a veteran service center at a major metropolitan university in Central Florida is a non-experimental, explanatory, retrospective survey design using structural equation modeling (SEM) to test the relationships among study variables. Findings strongly supported a relationship between childhood trauma and neglect and the severity of combat-related PTSD. Similarly, findings also supported that no relationship existed between childhood SES and the severity of combat-related PTSD. Both childhood trauma and neglect were significantly associated with combat-related PTSD at an even greater effect than that of combat exposure. SES was not found to be significant in the severity of combat-related PTSD. The findings suggest that preventive screening policies to reduce costs and severity of combat-related PTSD might be needed.
