Book excerpt: Adolescent risky sexual behaviors such as early sexual debut, low rates of condom use, and high rates of partner change are associated with a number of negative health and social outcomes including contracting sexual transmitted infections and unintended pregnancy. Utilizing a sample of 529 African American adolescents, this study examines the contributions of parenting behaviors and family structure as factors associated with engagement in risky sexual behavior. Participants reported few differences in mother's parenting across family structures. While fathers and relatives provided greater responsiveness on average than stepfathers. Variation in parenting fully explained differences in risky sexual behavior for males. Yet family structure differences persisted for females, with females in mother step-father homes continually engaging in greater mean risky sexual behavior than those in mother-father households even after accounting for parental responsiveness and demandingness. Implications for prevention and intervention are discussed.

Book excerpt: This book explores the various psychosocial, sociocultural, and contextual factors that affect the sexual health of Black students who attend Historically Black Colleges and Universities (HBCUs), and how this environment can help develop strategies to improve sexual health outcomes for its students. The college environment provides young people with a new sense of independence, self-determination, and peer pressure to engage in risky sexual behaviors, and research has shown that Black students at HBCUs bear a disproportionate burden of poorer sexual health outcomes than students at predominately white institutions (PWIs). Uniquely focusing on the sexual milieu of Black students, Hall-Byers explains why a better understanding of these settings is needed to guide successful interventions that benefit and support the sexual health of Black students. Chapters compare data and research on sexual health outcomes of young Black men and women in comparison to those at predominately white institutions, as well as looking at the role of HBCU campus contexts and cultures, the potential psychosocial and sociocultural influences, what culturally responsive approaches may look like, and recommendations on how HBCU campuses can increase positive sexual health, such as through access, collaborative efforts among administrative offices, and reallocating resources. Book excerpt: One in five people in the United States had a sexually transmitted infection (STI) on any given day in 2018, totaling nearly 68 million estimated infections. STIs are often asymptomatic (especially in women) and are therefore often undiagnosed and unreported. Untreated STIs can have severe health consequences, including chronic pelvic pain, infertility, miscarriage or newborn death, and increased risk of HIV infection, genital and oral cancers, neurological and rheumatological effects. In light of this, the Centers for Disease Control and Prevention, through the National Association of County and City Health Officials, commissioned the National Academies of Sciences, Engineering, and Medicine to convene a committee to examine the prevention and control of sexually transmitted infections in the United States and provide recommendations for action. In 1997, the Institute of Medicine released a report, The Hidden Epidemic: Confronting Sexually Transmitted Diseases. Although significant scientific advances have been made since that time, many of the problems and barriers described in that report persist today; STIs remain an underfunded and comparatively neglected field of public health practice and research. The committee reviewed the current state of STIs in the United States, and the resulting report, Sexually Transmitted Infections: Advancing a Sexual Health Paradigm, provides advice on future public health programs, policy, and research.

Book excerpt: Adolescent females of color, particularly Black and Hispanic adolescents, are often viewed as a homogenous group with adolescent boys of color, thus ignoring unique gender-racial specific risk and protective factors to drug use and HIV/AIDS that may be present. Such an absence can lead to flawed outcomes in HIV, STIs (sexually transmitted infections), and substance abuse prevention work that may continue to marginalize girls of color. Using empowerment theory and intersectionality as a framework, this study examines the extent to which ethnic identity, social support, and psychological empowerment is on drug use and sexual risk behavior. The study uses a sample of (N = 830) female adolescents who identify as being Black only or Non-White Hispanic only. All participants resided in a northeastern urban community in New Jersey. A majority of the participants (90%) were between the ages of 15–17 years of age. Confirmatory factor analysis was employed to test the factor structure of the scale used to measure psychological empowerment and structural equation modeling was used to test the hypothesized model of the mediating role of drug use on sexual risk behavior. Findings revealed that 30-day drug use significantly mediated the relationship between social support, ethnic identity, and psychological empowerment on sexual risk behavior. By highlighting the strengths of Black and Hispanic girls, researchers can attempt to learn from those who are not engaging in risky behaviors as a way to incorporate a strengths-based view in prevention.

Book excerpt: The purpose of this study is to explore ethnic identity as a protective factor against sexual activity among a sample of African American adolescents. The study sample consisted of 1084 African American adolescents who were administered a survey comprised of questions about their health, risk behaviors, and their backgrounds. The variables examined were sexual intercourse, ethnic identity, parent-child communication regarding specific topics, mother-adolescent conflict and negative communication. The study also examined family structure, educational aspirations, attitudes about sex, self-esteem, refusal skills, lifetime substance use, perceived peer lifetime sexual behavior, age and gender. Analyses conducted examined the relationship between sexual activity and the independent variables as well as associations between two nominal variables. Relationships between interval/ratio variables were investigated. Multiple logistic regression analyses were used to assess whether the relationship between ethnic identity and lifetime sexual intercourse persisted, controlling for individual, family, and social factors. The findings indicate ethnic identity, and individual, family, and peer factors were related to lifetime sexual intercourse. The effects were stronger for substance use and the influence of peers on sexual behavior. When taking into account the other individual, family, and peer factors, the relationship between ethnic identity and lifetime sexual intercourse was non-significant. Additionally, ethnic identity did not moderate the risk factors explored in this study. Gender differences are clearly delineated in the study with more males having sexual intercourse than females. The results of this study recommend multifaceted interventions that begin as early as elementary school and involve families, schools, communities, and organizations working to promote responsible sexual behavior of adolescents. In addition, the findings of other studies provide support for intervention programs that facilitate the development of ethnic identity, such as youth mentoring programs with a focus on Rites of Passage programs. Interventions should also be developed to include peer groups that are comprised of friends, because of the influence peers have on sexual activity and the engagement in other risk behaviors such as substance use.-- Abstract.

Book excerpt: "Health disparities research has indicated that urban, low-income African-American adolescents experience ecological and contextual factors like community violence, socioeconomic status, and limited sexual health knowledge which contribute to high rates of sexual risk among urban African American adolescents. However, protective factors like parental monitoring and parent-adolescent communication about sex may decrease sexual risk among this urban teen population. A sample of 1,102 African American adolescents aged 13-17 from urban Midwestern high schools were included in this study. The current study hypothesized that: (1) parent-adolescent communication about sex would be positively associated with adolescent sexual health knowledge, (2) there would be a significant negative association between parental monitoring and adolescent sexual risk, (3) both parental monitoring and parent-adolescent communication about sex would be negatively associated with adolescent sexual risk and (4) there would be gender differences in parental monitoring, (5) and parent-adolescent communication about sex. Results indicated no significant association between sexual health knowledge and parent-adolescent communication about sex or parental monitoring. Findings indicated a significant negative association between parental monitoring and adolescent sexual risk, with the association being stronger for boys than girls. There was a significant negative association between sexual health knowledge and adolescent sexual risk. Implications suggest that parental monitoring has greater influence on sexual risk in African American adolescents and thus, is more of a protective factor than parent-adolescent communication about sex."--Abstract from author supplied metadata.

Book excerpt: Hypertension is a preventable and yet major risk factor for early death and morbidity among African Americans. Compared to other women in the US, African American women continue to die earlier and more frequently from preventable and controllable chronic health conditions that are notably due to hypertension and hypertension-related illnesses. While there are multiple factors contributing to the high death rates of African American women, hypertension is one of the most common and modifiable risk factors associated with fatal health outcomes among African American women. The rate of death resulting from hypertension is more than double for African American females compared to white females. Even armed with increased knowledge and awareness, African American women are encountering barriers to controlling their hypertension, which places them at higher risk of becoming sicker and dying earlier than their white counterparts. The purpose of this mixed-methods study was to examine the effects of psychosocial stress on the hypertension self-management behaviors. The rationale for the study is supported by findings from a systematic literature review identifying gaps and contributions in the health literature on African American women and hypertension management. Findings underscore a need to continue to examine psychosocial factors as barriers to African American women's hypertension self-management. Specifically, the study found statistically significant associations between psychosocial stress and depression as it relates to the hypertension self-management of African American women. Further investigation is warranted to better understand the significance of the relationships between psychosocial stress, depression and African American women's hypertension self-management.
