Book excerpt: Mothers of children with Autism Spectrum Disorder (ASD) report more stress, anxiety and depression than fathers and parents of both typically-developing children and children with other disabilities such as Down syndrome. Because women commonly hold the majority of the responsibility for childcare activities and healthcare decisions, maternal depression in a family with a child with ASD involves all members of the family, and research and care should be approached with a family focus. Improving our understanding of the prevalence and relationship of depressive symptoms to family functioning and child outcomes is needed to support professional practices as well as national and state policy decisions. This dissertation is composed of three papers and uses a sequential explanatory mixed methods design to answer three questions: (a) what is the prevalence of depression in this population, (b) what modifiable factors are associated with depression, including self-efficacy and family functioning, and (c) how do mothers with varying levels of depressive symptoms describe their experiences raising a child with ASD? This study also proposes a conceptual model to illustrate factors associated with maternal depression. This is the first study with mothers of children with ASD to explore the role of maternal self-efficacy and family functioning together, and to explore the mother's perspective using rigorous qualitative methods. A more thorough understanding of depression and the role of associated factors in this population can have implications for the development of more effective screening and targeted interventions.

Book excerpt: This dissertation reports on the results of a qualitative investigation of the experiences of mothers whose children with autism spectrum disorder were experiencing transitions into and out of adolescence. A Constructivist Grounded Theory methodology was implemented in this study. Semi-structured interviews were conducted with 78 mothers who had children between the ages of 12 and 22. Mothers' experiences were influenced by several factors, arranged in four core categories: parent stressors (67%), services (4%), professionals (24%), and parent solutions (5%). The constructivist grounded theory process culminated with the Parents' Perceptions Theory (PPT) emerging from the data. The PPT theory explains that maternal stress is elevated when services or clinical practices are perceived to be unhelpful. Specifically, when services or practices were perceived as not addressing the needs of the parents or child from the mothers' perspectives, mothers reported higher stress. The PPT provides a new account grounded in mothers' own experiences for the observation that mothers' who actively receive services still report significant stress.In Study 2, the author sought to explore whether the PPT would also be evident in the daily accounts of parenting stress in parents of younger children with ASD (ages 3-11). Semi-structured interviews about daily parenting stress conducted with 20 parents (18 mothers; 2 fathers) were analyzed for the presence of the coding scheme and the PPT that emerged from Study 1. Three of the core categories were present in the data: Parent stressors (88%), services (2%), and professionals (10%). The Parents' Perceptions Theory was also reflected. Parents' perceptions of the usefulness of services or clinical practices were found to influence their reported stress. The application of Constructivist Grounded Theory across two samples of parents of children with autism spectrum disorder provided important insight into how the disconnect between parents' perceptions of whether services are meeting their caregiving needs and the receipt of services contributes to parenting stress. The PPT suggests that delivery of services should be informed by regular assessment of parental perceptions of whether their needs are being met in order to provide better support to these families throughout the child's lifespan.

Book excerpt: This mixed method study examined mood disorders in mothers who were rearing a child with ASD. The objective of the quantitative section of the study was to examine the occurrence of maternal depression and bipolar disorders in mothers of children with ASD compared to mothers of typically developing (TD) children. The Depression History Questionnaire that was adapted from Interactive Autism Network (IAN) at Kennedy Krieger Institute (KKI) database was used for this comparison. The qualitative section of the study explored the meaning of the lived experience of mothers who concurrently were depressed and rearing a children with ASD using a phenomenological approach. Results of the quantitative inquiry indicated that mothers of children with ASD report a history of: (a) depression and bipolar disorder; (b) treatment for depression or bipolar disorder; (c) suicidal ideation or suicidal attempts; and (d) depression, bipolar disorder, and suicide among the participants' first and second-degree relatives more frequently than mothers of TD children. The following five emerged themes emerged from the qualitative exploration: a) Regardless of when I was first diagnosed, having a child with ASD contributed to and intensified my depression; b) I feel a lack of support and understanding either from my husband, the extended family, or the community; c) Challenges and stressors of rearing a child with ASD are ongoing; what I do is never enough and is physically and mentally exhausting; d) It is difficult to balance all aspects of my life; it may be less demanding if I sacrifice one of my roles; and finally, e) There is a history of mental health challenges in my family. These findings provide valuable insight about concurrently coping with depression and rearing a child with ASD and propose interventions to address the challenge.
