Kominski and published by RAND Corporation. This book was released on 1993 with total page 88 pages. Available in PDF, EPUB and Kindle. Book excerpt: This study examined why the average cost of Medicare hospital discharges increased more rapidly than inflation after the implementation of Medicare's prospective payment system (PPS). The average cost per Medicare case rose by 28.4 percent between 1984 and 1987. The increase in the hospital market basket was 11.0 percent during this period, thus, the real increase in cost per case was 15.7 percent. The authors decomposed this change in real cost per case into two major components: changes across DRGs (i.e. case mix) and changes within DRGs (i.e. intensity). Average cost per case increased 11.2 percent due to changes in case mix, and 4.5 percent due to higher costs per case within DRGs. We further decomposed the across- and within-DRG increases into the following components: technology, outpatient shift, and a residual. The authors estimate that technology changes accounted for 5.8 percent of the total increase in cost per case, while outpatient shift accounted for 3.4 percent of the total increase.

Book excerpt: The case mix concept has been introduced in the USA more than twenty years ago in order to measure hospital productivity and to promote quality of care. The DRGs were selected by HCFA in 1983 as the case mix classification system for the MEDICARE prospective payment system (PPS). Many other countries have adopted the case mix concept after long periods of testing and accepting, but with large variations in data collection, information standards, grouping tools, financing methods and quality of care developments all over the world. Each country has developed a local clinical and political culture about case mix tools. The present book is intended to update the case mix situation, country by country.

Available in PDF, EPUB and Kindle. Book excerpt: Case management has become one of the most effective means by which a continuum of care functions effectively and efficiently. A complete examination of the application of continuum of care, The Case Management Workbook: Defining the Role of Physicians, Nurses, and Case Managers defines the roles of the case management team and provides an overview of key post acute venues of care. It supplies an overview of the process, its structure, and the responsibility of each member of the team. The book discusses how managing the care of the patient in today’s healthcare environment will take a concerted effort from the entire case management team—not just a handful of case managers. The authors demonstrate that the case management team includes many more individuals than simply those with the title of case manager. The team must consist of key clinical personnel including the nurse, the physician and the case manager, each of whom have specific roles in case management. The book also highlights the role administration plays in assuring each member of the team is on board with the hospital’s or health system’s goal in assuring the patient is at the right place at the right time in their episode of care. In the words of the authors, case managers are ... the unsung heroes of effective healthcare delivery. They push and they prod, they inform and they educate, they provide valuable information to physicians and families with a common goal—to assure the patient is provided care in the most appropriate venue correlative to the medical resources needed and provided. In their book they explore a key byproduct of an effective case management system— the realization of an effective integrated health system whereby physicians and hospitals are aligned as caretakers of future health resources.

Book excerpt: Among the issues confronting America is long-term care for frail, older persons and others with chronic conditions and functional limitations that limit their ability to care for themselves. Improving the Quality of Long-Term Care takes a comprehensive look at the quality of care and quality of life in long-term care, including nursing homes, home health agencies, residential care facilities, family members and a variety of others. This book describes the current state of long-term care, identifying problem areas and offering recommendations for federal and state policymakers. Who uses long-term care? How have the characteristics of this population changed over time? What paths do people follow in long term care? The committee provides the latest information on these and other key questions. This book explores strengths and limitations of available data and research literature especially for settings other than nursing homes, on methods to measure, oversee, and improve the quality of long-term care. The committee makes recommendations on setting and enforcing standards of care, strengthening the caregiving workforce, reimbursement issues, and expanding the knowledge base to guide organizational and individual caregivers in improving the quality of care.

Book excerpt: This book is vital for health service managers and clinicians, in both purchaser and provider organizations, in community and hospital settings. It helps the reader understand the principles and purposes of casemix and provides practical examples of using casemix groupings to manage services better. Its lessons are not just for acute services, but provide a way of understanding the complete spectrum of services required for a wide range of conditions, from individuals at risk to those with irreversible and progressive disease. The book explains why casemix groups are useful and the reasons for grouping and analysing patient records. It focuses on the difference between groupings of patients with conditions, and groupings of intervention episodes. Using both enables better identification of the services required to meet the needs of the population, and better communication between purchasers and providers. It has potential for managing the whole healthcare system from a population based perspective.

Book excerpt: It is widely recognized that studies of hospital cost and production relationships have been handicapped by the multiproduct nature of hospital output. In part the problem arises because statistical data on the mix of output or services produced by hospitals are not generally available. In this study, data on the mix of cases proceduced by a group of missouri hospitals have been combined with american hospital association data to explain variation in average costs using several alternate approaches. The distinctive feature of the study is the use of the casemix variable instead of patient days or cases to explain differences in 'average costs' between hospitals.
