The Institute of Medicine recommends that the United States and African nations move toward a strategy of shared responsibility such that these nations are empowered to take ownership of their HIV/AIDS problem and work to solve it.

Available in PDF, EPUB and Kindle. Book excerpt: "The President's Emergency Plan for AIDS Relief (PEPFAR) is well positioned to serve as a foundation for other global health programs, building on its health infrastructure, training, and systems. To fulfill that potential in the vital area of women's health will require integrating HIV/AIDS services with family planning and reproductive health services. The results from U.S. health investments in Tanzania indicate that this is a feasible and cost-effective strategy to combat the AIDS epidemic and promote the health of women and girls, and through them their families and communities. The lessons being learned in Tanzania should inform the scale up of strategic integration under PEPFAR for these critical interventions. This report examines the situation in Tanzania, where the United States has supported the national government in making notable progress toward integrating HIV services with family planning and reproductive health, particularly through PMTCT programs. Integration in Tanzania has been driven by a number of factors, including political commitment from the national government, specified funding from the United States, and experience brought by some PEPFAR implementing partners in the area of family planning-HIV integration. Yet despite the improved policy environment, ongoing barriers remain in implementation, financing for integration, and integration of family planning as a core component of PEPFAR's treatment programs"--Publisher's description.

Book excerpt: This book provides a clear and detailed examination of why it is so difficult to secure comprehensive political engagement and actionable, effective policy on sexual and reproductive health rights in sub-Saharan Africa. In an engaging analysis, Nana Poku employs expert knowledge to examine the prospects for large-scale improvements. He explores not only the full range of normative sensitivities, but also conceptual misunderstandings, legal difficulties and complex challenges of securing and maintaining adequate funding while AIDS remains a pandemic in the region. Up-to-date, succinct yet highly detailed, lucid and compelling in its diagnoses of highly complex issues, this book is a valuable, accessible study of a topic that is regional in focus but with clear global implications.

Book excerpt: One in five people in the United States had a sexually transmitted infection (STI) on any given day in 2018, totaling nearly 68 million estimated infections. STIs are often asymptomatic (especially in women) and are therefore often undiagnosed and unreported. Untreated STIs can have severe health consequences, including chronic pelvic pain, infertility, miscarriage or newborn death, and increased risk of HIV infection, genital and oral cancers, neurological and rheumatological effects. In light of this, the Centers for Disease Control and Prevention, through the National Association of County and City Health Officials, commissioned the National Academies of Sciences, Engineering, and Medicine to convene a committee to examine the prevention and control of sexually transmitted infections in the United States and provide recommendations for action. In 1997, the Institute of Medicine released a report, The Hidden Epidemic: Confronting Sexually Transmitted Diseases. Although significant scientific advances have been made since that time, many of the problems and barriers described in that report persist today; STIs remain an underfunded and comparatively neglected field of public health practice and research. The committee reviewed the current state of STIs in the United States, and the resulting report, Sexually Transmitted Infections: Advancing a Sexual Health Paradigm, provides advice on future public health programs, policy, and research.

Book excerpt: he starting point for this guideline is the point at which a woman has learnt that she is living with HIV and it therefore covers key issues for providing comprehensive sexual and reproductive health and rights-related services and support for women living with HIV. As women living with HIV face unique challenges and human rights violations related to their sexuality and reproduction within their families and communities as well as from the health-care institutions where they seek care particular emphasis is placed on the creation of an enabling environment to support more effective health interventions and better health outcomes. This guideline is meant to help countries to more effectively and efficiently plan develop and monitor programmes and services that promote gender equality and human rights and hence are more acceptable and appropriate for women living with HIV taking into account the national and local epidemiological context. It discusses implementation issues that health interventions and service delivery must address to achieve gender equality and support human rights.
