Book excerpt: Amyotrophic lateral sclerosis (ALS) our Lou Gehrig's disease is a fatal, mostly non-familial disease that affects the nervous system of humans by causing the degeneration of nerve cells in the brain and spinal cord. The degeneration halts communication between the nervous system and voluntary muscles in the body. This leads to muscle paralysis and eventually the muscles that aid in breathing are affect; causing respiration to fail. The disease, which affects 20,000-30,000 men and women in the United States at any given time, has no effective treatment; most people with ALS die from respiratory failure within 5 years of the onset of symptoms. Recent epidemiologic studies report an association between the development of ALS and prior service in the U.S. military. The studies evaluated either veterans of the 1991 Persian Gulf War or veterans who served in the military in the period 1910-1982. Due to these findings, the Department of Veterans Affairs (VA) asked the National Academies to conduct an assessment of the potential relationship between military service and the later development of ALS. The project was assigned to the Institute of Medicine (IOM), which appointed a committee and gave it the task of evaluating the scientific literature on ALS in veterans. The committee began its work by identifying medical and scientific literature on ALS. PubMed, a database created and managed by the National Library of Medicine. Amyotrophic Lateral Sclerosis in Veterans; Review of the Scientific Literature presents the findings of this committee. The committee reviewed, evaluated, and summarized the scientific literature on ALS in veterans, composed primarily of peer-reviewed, published literature. This report includes the recommendations of the committee.

We aim to collect knowledge from the global network of researchers working on topics related to “Neurological and Neuropsychiatric disorders affecting military personnel and Veterans.” There are currently many gaps in the diagnosis, prevention, and treatment of conditions that affect this population disproportionately. This collection of work in the Frontiers in Neurology will give authors the opportunity to share with the global scientific community important research findings that address these gaps.For this collection, we are looking for submissions with topics that specifically address the unique challenges affecting military personnel and Veterans with Neurologic and Neuropsychiatric disorders.

Book excerpt: The authors prospectively assessed the association between Amyotrophic Lateral Sclerosis (ALS) mortality and self-reported military service in the American Cancer Society's Cancer Prevention Study II (CPS II) cohort, a cohort that includes over 500,000 men who were surveyed by questionnaire in 1982. ALS mortality was assessed via linkage with the National Death Index. Their original analyses included ALS deaths (n = 280) up to 1998. They have now extended the follow-up to 2002 (ALS deaths = 513). Mantel-Haenszel relative risks (RR) adjusted for age and smoking were calculated and Cox proportional hazards models stratified on single year of age were used when adjusting for additional variables. Men who served in the military (69% of their population) had a significantly increased ALS mortality (RR = 1.5) compared to those who did not serve. The increase in ALS mortality was similar among men who served in the Army or National Guard (RR = 1.5), Navy (RR = 1.7), or Air Force (RR = 1.5). This increased risk of ALS appeared largely independent of branch of service, years of service, number of major conflicts during service, and war periods during service (i.e., World War II, the Korean War, and the Vietnam War). The project will continue as planned. The authors will extend the follow-up and examine the possible role of vitamin E supplements in prevention.

Mixtures of 2,4-dichlorophenoxyacetic acid (2,4-D), 2,4,5-trichlorophenoxyacetic acid (2,4,5-T), picloram, and cacodylic acid made up the bulk of the herbicides sprayed. The main chemical mixture sprayed was Agent Orange, a 50:50 mixture of 2,4-D and 2,4,5-T. At the time of the spraying, 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD), the most toxic form of dioxin, was an unintended contaminant generated during the production of 2,4,5-T and so was present in Agent Orange and some other formulations sprayed in Vietnam. Because of complaints from returning Vietnam veterans about their own health and that of their children combined with emerging toxicologic evidence of adverse effects of phenoxy herbicides and TCDD, the National Academies of Sciences, Engineering, and Medicine was asked to perform a comprehensive evaluation of scientific and medical information regarding the health effects of exposure to Agent Orange, other herbicides used in Vietnam, and the various components of those herbicides, including TCDD. Updated evaluations were conducted every two years to review newly available literature and draw conclusions from the overall evidence. Veterans and Agent Orange: Update 11 (2018) examines peer-reviewed scientific reports concerning associations between various health outcomes and exposure to TCDD and other chemicals in the herbicides used in Vietnam that were published between September 30, 2014, and December 31, 2017, and integrates this information with the previously established evidence database.

Book excerpt: To compare serum samples from individuals diagnosed with amyotrophic lateral sclerosis (ALS) to serum samples from matched individuals who did not develop ALS. In this study we aim to identify candidate serum biomarkers that are unique for ALS and identify a subset of diagnostic serum biomarkers for early detection of ALS prior to the appearance of overt symptoms. Scope: The significance of a positive identification of protein biomarkers for ALS is indisputably great. However, to date no validated clinically relevant biomarkers have been found to allow a more specific diagnosis of ALS at an earlier stage. Previous efforts to identify ALS associated biomarkers have often focused on the identification of genes and proteins characteristic for familial ALS, yet validated biomarkers for sporadic ALS, which accounts for as much as 90-95% of all ALS cases, have yet to be identified. Major Findings: None at this time. Progress: This study received USAMRMC HSSRB approval on 15 Jan 2007. The Durham VA and Univ. of Cincinnati IRB protocols have also been approved. Durham VA IRB began consenting the VA ALS registry members for this study. Model system 2-D gel and mass spectrometry studies have been conducted to develop improved techniques for biomarker identification.

Particular controversy surrounds the burn pit used to dispose of solid waste at Joint Base Balad in Iraq, which burned up to 200 tons of waste per day in 2007. The Department of Veterans Affairs asked the IOM to form a committee to determine the long-term health effects from exposure to these burn pits. Insufficient evidence prevented the IOM committee from developing firm conclusions. This report, therefore, recommends that, along with more efficient data-gathering methods, a study be conducted that would evaluate the health status of service members from their time of deployment over many years to determine their incidence of chronic diseases.
