Thus, those who exhibited suicidal tendencies were subjected to an amalgamation of pharmacological, social, and psychological interventions, which practioners labeled the "moral treatment." By the 1890s, however, the consensus about the causes of suicide became unglued as a bacteriological medicine and the rise of the social sciences jointly served to call into question eclectic diagnoses. The goal of American Suicide is to demonstrate how the apparent contradictions among sociological, psychoanalytic, and neurobiological explanations of the etiology of suicide may be resolved. Only througha reintegration of culture, psychology, and biology can we begin to construct a satisfactory answer to the questions first raised by Durkheim, Freud, and Kraepelin.

Haller Jr., explores the epistemological foundations of EBM and the challenges these conceptual tools present for both conventional and alternative therapies. As he explores a possible reconciliation between their conflicting approaches, Haller maintains a healthy, scientific skepticism yet finds promise in select complementary and alternative (CAM) therapies. Haller elucidates recent research on the placebo effect and shows how a new engagement between EBM and CAM might lead to a more productive medical practice that includes both the objectivity of evidence-based medicine and the subjective truth of the physician-patient relationship. Haller's book tours key topics in the standoff between EBM and CAM: how and why the double blinded, randomized clinical trial (RCT) came to be considered the gold standard in modern medicine; the challenge of postmodern medicine as it counters the positivism of evidence-based medicine; and the politics of modern CAM and the rise of the National Center for Complementary and Alternative Medicine. He conducts an in-depth case study of homeopathy, explaining why it has emerged as a poster-child for CAM, and assesses CAM's popularity despite its poor performance in clinical trials. Haller concludes with hope, showing how new experimental protocols might tease out the evidentiary basis for the placebo effect and establish a foundation for some reconciliation between EBM and CAM.

Book excerpt: An argument against the "lifeboat ethic" of contemporary bioethics that views medicine as a commodity rather than a tradition of care and caring. Bioethics emerged in the 1960s from a conviction that physicians and researchers needed the guidance of philosophers in handling the issues raised by technological advances in medicine. It blossomed as a response to the perceived doctor-knows-best paternalism of the traditional medical ethic and today plays a critical role in health policies and treatment decisions. Bioethics claimed to offer a set of generally applicable, universally accepted guidelines that would simplify complex situations. In Thieves of Virtue, Tom Koch contends that bioethics has failed to deliver on its promises. Instead, he argues, bioethics has promoted a view of medicine as a commodity whose delivery is predicated not on care but on economic efficiency. At the heart of bioethics, Koch writes, is a "lifeboat ethic" that assumes "scarcity" of medical resources is a natural condition rather than the result of prior economic, political, and social choices. The idea of natural scarcity requiring ethical triage signaled a shift in ethical emphasis from patient care and the physician's responsibility for it to neoliberal accountancies and the promotion of research as the preeminent good. The solution to the failure of bioethics is not a new set of simplistic principles. Koch points the way to a transformed medical ethics that is humanist, responsible, and defensible.

Tristram Engelhardt, Jr. Since that time it has been a pleasure to get to know the life and work of Elisha Bartlett. I am pleased to be completing this book in the bicentennial year of Bartlett’s birth. Bartlett was born in 1804 in Smithfield, Rhode Island, less than twenty-five miles from Worcester, Massachusetts, my present home—a short journey even in Bartlett’s day. I have been able to walk at some of the sites to which Bartlett continually returned during his life. Visiting Bartlett’s grave in the Slatersville cemetery has been an inspiration for the preparation of this book. Proximity to several institutions with rich holdings in Bartlett’s works and in nineteenth-century American history of medicine greatly facilitated my research. First, though, I want to acknowledge the College of the Holy Cross for supporting my sabbatical leave for the academic year 2003-2004. The American Antiquarian Society, in Worcester, Massachusetts, was generous in giving me access to its remarkable resources. I was able to find many of Bartlett’s published works and other nineteenth-century medical literature there, and the entire library staff provided quick and able research assistance.
