Book excerpt: La permanence de soins reste un pilier indispensable au système de santé actuel, mais présente encore des faiblesses et des imperfections qui sont optimisables pour améliorer la prise en charge des patients en médecine ambulatoire. Son système reste encore imparfaitement compris par la population, et est régulièrement soumis à des modifications. Cette thèse permet de faire le point en 2020 des différents acteurs de la permanence de soins dans le 54 et le 57, et de décrire un outil numérique dédié à la coordination de soins. Les départements de la Moselle et de la Meurthe et Moselle possède des organisations et des acteurs de la permanence de soins différents, en qualité, quantité, ou même intervenants. En revanche, la régulation reste un socle de l'organisation de la permanence des soins. Elle permet d'orchestrer tout le processus de régulation et d'offre de soins de la PDS. Bien qu'indispensable, nous avons vu qu'il existe des acteurs indépendants du système actuel, et dont le rôle et l'impact reste à définir. De plus, la permanence de soins répond à l'offre de soins, mais une part non négligeable des soins ambulatoire non programmés restent en période hors PDS. C'est dans ce contexte que certains outils numériques ont vu le jour afin d'améliorer la coordination de soins, de faciliter le dialogue entre les différents professionnels de santé, et de répondre partiellement à cette demande de soins. Ainsi l'application Entr'actes s'implante progressivement sur le territoire Mosellan, et semble prometteur et bien accepté par les différents utilisateurs dans son ensemble, mais reste imparfaite et des améliorations sont à effectuer pour optimiser son fonctionnement. Il serait intéressant d'en étudier l'impact sur le territoire après une implantation durable et profonde sur le département de la Moselle.

Book excerpt: Contexte La chirurgie ambulatoire est une activité en augmentation qui nécessite de gérer des flux multiples au sein de l'hôpital en lien avec les soins primaires. La coordination entre ces deux mondes apparaît être un élément de premier ordre ciblé comme axe d'amélioration par la Haute Autorité de Santé. La perception des soignants vis-à-vis de cette coordination n'a fait l'objet que de très peu d'études. L'objectif principal de l'étude était d'analyser les besoins et les représentations des paramédicaux de la périphérie Lilloise vis-à-vis de la coordination des soins pour la prise en charge des patients opérés en chirurgie ambulatoire. L'objectif secondaire était de mettre en évidence des propositions de modification au fonctionnement actuel. Méthode : Une étude qualitative par théorisation ancrée sous forme d'entretiens semidirigés a été réalisée de Janvier à Juillet 2014 jusqu'à saturation théorique des données. Les entretiens avaient été retranscrits, anonymisés puis codés grâce au logiciel Nvivo®. Résultats : 17 entretiens de paramédicaux hospitaliers et libéraux ont été réalisés. Les hospitaliers décrivent l'absence de contact avec les soins primaires et peu de besoins en termes d'informations. Les libéraux se sentent exclus de la coordination actuelle et décrivent un manque d'informations qu'ils arrivent cependant à récupérer en utilisant des solutions de remplacement. La simplicité des actes demandés en post-opératoire limite selon eux le retentissement de cette coordination non optimale. Les propositions de modification sont nombreuses comme par exemple la mise en place de protocoles ou de fiches de liaison ville/hôpital. Conclusion : L'exploration des représentations des paramédicaux a permis de mettre en évidence un manque du point de vue des libéraux. Le développement futur de la chirurgie ambulatoire passe par une modification de la coordination actuelle afin d'assurer une prise en charge optimale du patient.
