METHODE : Enquête qualitative par entretiens semi-dirigés individuels de mères allaitantes, reprenant leur activité professionnelle avant les 6 mois de leur enfant et travaillant dans les Alpes-Maritimes, durant la période d'avril à juin 2014. RESULTATS : Des facteurs intrinsèques motivaient ou non les mères : leurs connaissances, croyances et représentations sur l'allaitement et sur les moyens de le poursuivre en travaillant ; leur attitude et leurs intentions vis-à-vis de l'allaitement, de la relation mère-enfant et de l'expression de lait maternel ; leurs représentations issues de leur culture d'origine ; leurs expériences actuelles et antérieures d'allaitement. Des facteurs extrinsèques pouvaient soutenir ou menacer la poursuite de l'allaitement : l'attitude du père, de l'entourage et de l'environnement socioculturel ; le partage avec d'autres mères ; les conditions d'emploi facilitant ou non l'expression du lait maternel, le temps total et la flexibilité du travail, l'attitude de l'employeur et des collègues ; le discours médical et l'attitude des soignants. La décision des mères pouvait évoluer, selon un continuum allaitement exclusif - allaitement partiel - sevrage, à la lumière de nouvelles connaissances, des difficultés expérimentées et du soutien rencontré. Les mères attendaient des soignants écoute empathique, informations adaptées, respect et accompagnement compétent de leur choix. CONCLUSION : Les facteurs influençant le choix des mères actives sur le devenir de l'allaitement sont multiples, offrant plusieurs axes d'approche pour la promotion de la poursuite de l'allaitement lors de la reprise du travail : respect du Code du travail, modification du regard culturel sur l'allaitement, participation du père aux informations, orientation vers des groupes de soutien à l'allaitement, accompagnement compétent et ajusté par les soignants.

Pour les mamans qui souhaitent suivre ces recommandations, cela peut signifier poursuivre l'allaitement audelà de la reprise d'une activité professionnelle. Ce qui demande de l'organisation et surtout du soutien. Nous avons donc souhaité réaliser un guide pratique destiné à ces mamans. Nous avons utilisé la méthode décrite par l'HAS en 2008 pour l'élaboration d'un document écrit d'information destiné aux patients et aux usagers du système de santé. Nous avons effectué une recherche bibliographique afin de sélectionner les informations validées utilisées pour le contenu initial de la fiche. Nous avons ensuite travaillé en collaboration avec des mamans, des sages-femmes, des médecins généralistes et des puéricultrices : le document initial a été soumis pour discussion à des groupes de travail puis validé lors d'entretiens individuels. Deux groupes de travail et cinq entretiens individuels ont été réalisés. Les items abordés dans le document ont été : composition des repas en l'absence de la maman, conservation et utilisation du lait maternel, matériel nécessaire, droit du travail, difficultés possibles, contacts en cas de questions supplémentaires. La partie "difficultés" a été ajoutée lors du premier groupe de travail. Les autres modifications issues de ce groupe ont porté sur la mise en évidence de certaines informations jugées importantes et l'ajout des recommandations de l'OMS concernant la durée de l'allaitement. Les modifications apportées par le second groupe ont concerné les tournures des phrases, l'ajout de la nécessité de conserver au moins trois tétées par jour et de la notion de confiance en soi. Les entretiens individuels ont témoigné d'une globale satisfaction. Il a été décidé de diffuser le document sous forme papier et sur Internet. La prochaine étape sera de diffuser le document. Il faudra ensuite évaluer l'impact du document et actualiser les données

La reprise de l’activité professionnelle est la cause de quasiment 37% des motifs de sevrage, même si le code du travail prévoit une heure d’allaitement dans la journée. Cependant, allaiter et travailler restent possibles à condition d’organisation. L’utilisation d’un tire-lait avant même la reprise du travail permet de réaliser des stocks de lait que la maman congèlera. Après la reprise, le principe de base est que, lorsque l’enfant est avec sa mère, il a une alimentation au sein, lorsqu’il est chez l’assistante maternelle ou à la crèche, il est nourri avec des biberons préparés à partir des stocks de lait maternel réalisés par la maman. Ce système d’utilisation de tire-lait conjugué à la congélation du lait maternel permet ainsi à la maman active d’allaiter malgré son activité professionnelle.

Ces femmes sont soumises à des contraintes de par leur métier atypique. L’objectif de notre étude était d’étudier les facteurs influençant la conciliation de l’allaitement maternel avec l’activité professionnelle chez la femme militaire.Matériel et méthode : Étude qualitative par entretiens semi-dirigés auprès de 21 femmes du bassin brestois, recrutées dans la Marine nationale, la Gendarmerie et le Service de Santé des Armées. Les entretiens ont été enregistrés et retranscrits. La saturation des données a été obtenue après 19 entretiens. Résultats : les femmes avaient entre 25 et 40 ans. Les différentes catégories de grade étaient représentées. Les femmes militaires allaitent de la même manière que les femmes civiles, mais la poursuite de l’allaitement maternel lors de la reprise du travail et son maintien dans le temps apparaissent difficiles et contraignants pour toutes ces femmes, y compris celles qui parviennent à concilier les deux. Parmi les 21 mères interrogées, 11 femmes allaitaient à la reprise du travail, mais seulement 4 d’entre elles avaient réussi à concilier allaitement et travail durablement. Les facteurs favorisants principaux sont les motivations personnelles et un environnement personnel et professionnel soutenant. A contrario, le manque de connaissance, de soutien et d’aménagementsur le lieu de travail des mères ainsi qu’une reprise du travail précoce (avant 4 mois) et à temps plein sont les principaux facteurs freinant. Conclusion : La prise en charge des femmes militaires allaitantes pourrait être améliorée en favorisant la communication et en apportant une information adaptée. Nous proposons une brochure rappelant les recommandations de santé, faisant le point sur les principales idées reçues et difficultés, tout en abordant les différents moyens de conciliation de l’allaitement avec le travail et précisant les numéros utiles en Finistère.

L'objectif principal était d'identifier les attentes des femmes allaitantes vis-à-vis de leur médecin traitant dans la poursuite de l'allaitement maternel et définir la place de celui-ci. Matériels et méthode : la population de l'étude était constituée de 18 femmes ayant allaité ou allaitant un nourrisson né à terme en bonne santé vivant à Aubagne, Marseille ou ses environs sur une période de 4 mois. Il s'agissait d'une étude qualitative par entretiens téléphoniques semi-dirigés jusqu'à saturation des donnés. Etaient exclus les femmes présentant des troubles cognitifs. Résultats : les mères interrogées rapportaient s'orienter spontanément vers leur sage-femme, gynécologue ou leur conseillère en lactation en cas de difficultés. Elles s'adressaient à leur médecin traitant pour des motifs plutôt en lien avec des pathologies plus générales, pour le suivi de leur nouveau-né et avaient peu d'attentes envers ces derniers. Elles se sentaient soutenues par leur conjoint, mais rapportaient une aide matérielle plus qu'humaine pour mieux organiser leur quotidien, avec notamment l'usage du tire-lait. La reprise de l'activité professionnelle était rapportée comme un frein à l'allaitement maternel. Elles semblaient être toutes satisfaites de leur allaitement, motivées à réitérer pour un futur enfant et abondaient de conseils aux futures mères sur le sujet. Conclusion : le médecin traitant est apparu peu présent concernant la prise en charge l'allaitement maternel. Il a pourtant un rôle central dans la prise en charge des femmes. L'abord de ce sujet par ce dernier permettrait peut-être un recours facilité et un soutien supplémentaire dans la poursuite de l'allaitement.

The emotional system is organized to snooker us humans: encouraging us to take sides, run away from others, to pressure others, to get sick, to blame others, and to have great difficulty in seeing our part in problems. It is hard to see that we become anxious, stressed out and even that we are difficult to deal with. But "thinking systems" can open the doors of perception, allowing us to experience the world in a different way. This book offers both coaching ideas and stories from leaders as to strategies to break out from social control by de-triangling, using paradoxes, reversals and other types of interruptions of highly linked emotional processes. Time is needed to think clearly about the automatic nature of the two against one triangle. Time and experience is required as we learn strategies to put two people together and get self outside the control of the system. In addition, it takes time to clarify and define one's principles, to know what "I" will or will not do and to be able to take a stand with others with whom we are very involved. The good news is that systems' thinking is possible for anyone. It is always possible for an individual to understand feelings and to integrate them with their more rational brains. In so doing, an individual increases his or her ability to communicate despite misunderstandings or even rejection from important others. The effort involved in creating your Mindful Compass enables us to perceive the relationship system without experiencing it's threats. The four points on the Mindful Compass are: 1) Action for Self, 2) Resistance to Forward Progress, 3) Knowledge of Social Systems and the 4) The Ability to Stand Alone. Each gives us a view of the process one enters when making an effort to define a self and build an emotional backbone. It is not easy to find our way through the social jungle. The ability to know emotional systems well enough to take a position for self and to become more differentiated is part of the natural way humans cope with pressure. Now people can use available knowledge to build an emotional backbone, by thoughtfully altering their part in the relationship system. No one knows how far one can go by making an effort to be more of a self-defined individual in relationships to others. Through increasing emotional maturity, we can find greater individual freedom at the same time that we increase our ability to cooperate and to be close to others.
