Book excerpt: The quantitative analysis of blood flow within central and peripheral blood vessels has attracted more and more interest, for with the rapid developments in vascular surgery and the introduction of percutaneous transluminal angioplasty, it is becom ing increasingly important to be able to measure regional blood flow in man. In clinical radiology, angiography has been used predominantly from the point of view of its morphological applications. However, theoretically angiography may also be regarded as a specific application of indicator-dilution measurements of blood flow. The indicator is contrast medium (CM), the changing concentration of which is re corded by cinematography or video-electronic systems at sites downstream from the point of injection. The curves of density thus obtained correspond to indicator-di lution curves. The blood flow can be calculated from the concentration-time curves ofCM in much the same way as it is estimated from other indicator-dilution curves. In our early work with clinical application of videodensitometric measurement of blood flow in the iliac artery, we found that this method did not offer as high a degree of accuracy as one would have expected from investigations by other authors. We saw that we needed to examine the following problems: 1. We had to investigate whether our conventional X-ray equipment and video densitometer could be viewed as a linear measuring system. To what extent do unavoidable nonlinear changes in the measuring signals influence the blood flow values? 2.

Book excerpt: A volume on the biliary system appeared in 1965, based on a symposium of the NATO Advanced Institute held in September, 1963, in Newcastle upon Tyne in England. It soon became an authorita tive text on the problems of the biliary tract and, for instance, the discussion on the biliary secretion of organic anions, organic cations and inorganic ions were classic references. The worn pages of the volume in many libraries, including my own, bespeak its usefulness over ten long years. The initiative and energy of the Director of the first Institute have to be admired, even more so since he was able to assemble a Second Institute in Aalborg, Denmark, in 1975. His wisdom is reflected in the selection of the subjects discussed. The comparison between the two volumes tells something about the advancement in the time interval in knowledge about liver and biliary tract diseases, about the turnover of researchers interested in the liver and its diseases, and also about the present philosophy in biologic science. For this com parison, this volume is particularly suited because, in contrast to many other recent conferences, the more leisurely pace of the NATO Institutes permits more comprehensive but still scholarly reviews of the problems. The foreword of the first volume, written by my colleague, the late Harry Sobotka, expresses his, now dated, astonishment about the progress of knowledge of biology of the bile, so much less accessible to study than urine.

It is true that many articles and papers from symposia on this subject are available but they are all scattered through a volumnous literature. Dr Cox and his colleagues have brought together in an orderly fashion the current available material on hepatobiliary scintigraphy in an excellent volume suitable for both the clinician as well as the clinical scientist. This volume begins with a detailed discussion of anatomy and physiological functions of the liver and bi liary tract followed by a section on scintigraphic functional imaging of the liver. A description of the chemistry and pharma 99m ceutical considerations of Tc labeled hepatobiliary agents, especially those of Ida-derivatives is included. Next the text follows the usual pattern of discussion on the pharmacodynamics of radiopharmaceuticals, followed by a description of various clinical disease patterns of the liver and the use of cholescintigraphy in evaluating these diseases. The last sections deal with computer applications in quantitation of liver function followed by a discussion 99m of the clinical role for Tc labeled hepatobiliary agents in comparison to ultrasonography, CT, radiography and in vitro laboratory tests. One notable feature of this book is its discussion on the evaluation of new agents in normal experimental animals and in animals with induced liver disease, correlating this data to define the best radiopharmaceutical and then evaluating the same in patients. This type of methodological treatment of the subject matter is commendable.
