Although life expectancy and survival rates in the United States have improved dramatically over the past century, Americans live shorter lives and experience more injuries and illnesses than people in other high-income countries. The U.S. health disadvantage cannot be attributed solely to the adverse health status of racial or ethnic minorities or poor people: even highly advantaged Americans are in worse health than their counterparts in other, "peer" countries. In light of the new and growing evidence about the U.S. health disadvantage, the National Institutes of Health asked the National Research Council (NRC) and the Institute of Medicine (IOM) to convene a panel of experts to study the issue. The Panel on Understanding Cross-National Health Differences Among High-Income Countries examined whether the U.S. health disadvantage exists across the life span, considered potential explanations, and assessed the larger implications of the findings. U.S. Health in International Perspective presents detailed evidence on the issue, explores the possible explanations for the shorter and less healthy lives of Americans than those of people in comparable countries, and recommends actions by both government and nongovernment agencies and organizations to address the U.S. health disadvantage.

Using new survey data from Bangladesh, they analyze exposure at two levels--differences within households attributable to family roles, and differences across households attributable to income and education. Within households, they relate individuals' exposure to pollution in different locations during their daily round of activity. The authors find high levels of exposure for children and adolescents of both sexes, with particularly serious exposure for children under 5. Among prime-age adults, they find that men have half the exposure of women (whose exposure is similar to that of children and adolescents). They also find that elderly men have significantly lower exposure than elderly women. Across households, they draw on results from their previous paper (Dasgupta and others, 2004), which relate pollution variation across households to choices of cooking fuel, cooking locations, construction materials, and ventilation practices. They find that these choices are significantly affected by family income and adult education levels (particularly for women). Overall, the authors find that the poorest, least-educated households have twice the pollution levels of relatively high-income households with highly-educated adults. For children in a typical household, pollution exposure can be halved by adopting two simple measures--increasing their outdoor time from 3 to 5 or 6 hours a day, and concentrating outdoor time during peak cooking periods. The authors recognize that weather and other factors may intervene occasionally, and that child supervision outdoors may be difficult for some households. However, the potential benefits are so great that neighbors might well agree to pool outdoor supervision once they became aware of the implications for their children's health. The challenges described include air pollution, climate change, extreme weather, and related health impacts that range from heat stress, vector-borne diseases, food and water insecurity and chronic diseases to malnutrition and mental well-being. The influence of humans on climate change has been established through extensive published evidence and reports. However, the connections between climate change, the health of the planet and the impact on human health have not received the same level of attention. Therefore, the global focus on the public health impacts of climate change is a relatively recent area of interest. This focus is timely since scientists have concluded that changes in climate have led to new weather extremes such as floods, storms, heat waves, droughts and fires, in turn leading to more than 600,000 deaths and the displacement of nearly 4 billion people in the last 20 years. Previous work on the health impacts of climate change was limited mostly to epidemiologic approaches and outcomes and focused less on multidisciplinary, multi-faceted collaborations between physical scientists, public health researchers and policy makers. Further, there was little attention paid to faith-based and ethical approaches to the problem. The solutions and actions we explore in this book engage diverse sectors of civil society, faith leadership, and political leadership, all oriented by ethics, advocacy, and policy with a special focus on poor and vulnerable populations. The book highlights areas we think will resonate broadly with the public, faith leaders, researchers and students across disciplines including the humanities, and policy makers.

The structural, or compensatory, value is estimated at $3.6 billion. Information on the structure and functions of the urban forest can be used to improve and augment support for urban forest management programs and to integrate urban forests within plans to improve environmental quality in the Washington, D.C. area.

Over the past decade, the toll of ambient air pollution has continued to rise. Air pollution’s significant health, social, and economic effects compel the World Bank to support client countries in addressing air pollution as a core development challenge. This publication estimates that the global cost of health damages associated with exposure to air pollution is $8.1 trillion, equivalent to 6.1 percent of global GDP. People in low- and middle-income countries are most affected by mortality and morbidity from air pollution. The death rate associated with air pollution is significantly higher in low-and lower-middle income countries than in high-income countries. This publication further develops the evidence base for air-quality management through up-to-date estimates of air pollution’s global economic costs. The analyses presented here build on previous cost estimates by the Bank and its partners, as well as on more comprehensive air-quality data from monitoring stations in many cities across the world. By providing monetary estimates of air pollution’s health damages, this publication aims to support policy makers and decision-makers in client countries in prioritizing air pollution amid competing development challenges. Its findings build a robust economic case to invest scarce budgetary resources in the design and implementation of policies and interventions for improving air quality. Such investments will deliver benefits for societies at large, and particularly for vulnerable groups. This publication builds a strong case for scaling up investments for air pollution control in low-and middle-income countries.

Book excerpt: Many veterans returning from the conflicts in Iraq and Afghanistan have health problems they believe are related to their exposure to the smoke from the burning of waste in open-air "burn pits" on military bases. Particular controversy surrounds the burn pit used to dispose of solid waste at Joint Base Balad in Iraq, which burned up to 200 tons of waste per day in 2007. The Department of Veterans Affairs asked the IOM to form a committee to determine the long-term health effects from exposure to these burn pits. Insufficient evidence prevented the IOM committee from developing firm conclusions. This report, therefore, recommends that, along with more efficient data-gathering methods, a study be conducted that would evaluate the health status of service members from their time of deployment over many years to determine their incidence of chronic diseases.
