Book excerpt: On 28 December 2014 an Airbus A320-216 aircraft registered as PK-AXC was cruising at 32,000 feet on a flight from Juanda Airport, Surabaya, Indonesia to Changi Airport, Singapore with total occupants of 162 persons. The Pilot in Command (PIC) acted as Pilot Monitoring (PM) and the Second in Command (SIC) acted as Pilot Flying (PF). The Flight Data Recorder (FDR) recorded that many master cautions activated following the failure of the Rudder Travel Limiter which triggered Electronic Centralized Aircraft Monitoring (ECAM) message of AUTO FLT RUD TRV LIM SYS. The crew tried repeatedly to reset the computers but the autopilot and auto-thrust disengaged and the flight control reverted to Alternate Law. The investigation showed that the loss of electricity and the RTLU failure were caused by a cracked solder joint. All occupants of the plane were killed in the accident.

The accident was caused by the loss of an engine during a critical phase of flight, the non-retraction of the landing gear after the engine failure, and the Captain, the PNF, taking over control of the airplane before having clearly identified the problem.

The cargo door was recovered in two pieces from the ocean floor at a depth of 14,200 feet on September 26 and October 1, 1990. The probable cause of this accident was a faulty switch or wiring in the door control system. Contributing to the cause of the accident was a deficiency in the design of the cargo door locking mechanisms. Also contributing to the accident was a lack of timely corrective actions by Boeing and the FAA following a 1987 cargo door opening incident on a Pan Am B-747.

Flight 801 was operating in U.S. airspace as a regularly scheduled international passenger service flight under the Convention on International Civil Aviation and the provisions of 14 Code of Federal Regulations Part 129 and was on an instrument flight rules flight plan. The National Transportation Safety Board determines that the probable cause of the Korean Air flight 801 accident was the captain's failure to adequately brief and execute the nonprecision approach and the first officer's and flight engineer's failure to effectively monitor and cross-check the captain's execution of the approach. Contributing to these failures were the captain's fatigue and Korean Air's inadequate flight crew training. Contributing to the accident was the Federal Aviation Administration's (FAA) intentional inhibition of the minimum safe altitude warning system (MSAW) at Guam and the agency's failure to adequately manage the system. The safety issues in this report focus on flight crew performance, approach procedures, and pilot training; air traffic control, including controller performance and the intentional inhibition of the MSAW system at Guam; emergency response; the adequacy of Korean Civil Aviation Bureau (KCAB) and FAA over.

Kennedy International Airport, New York, United States, on a scheduled flight for Seoul, Republic of Korea. The flight had 269 persons on board. Soon after departure from Anchorage, Alaska, KE 007 deviated to the right (north) of its direct track, this deviation resulted in penetration of Sovjet Russian air space. Military aircraft operated by the USSR attempted to intercept KE 007 over Kamchatka Peninsula. The interception attempts were unsuccessful. Upon approaching Sakhalin Island, USSR, the flight was intercepted by USSR military aircraft and shot down on the assumption that is was a United States RC-135 (spy) aircraft. There were no survivors.

The Chinese investigation team pointed out that the Korean ATC was not fully licensed and mistakenly directed the airliner to descend to a wrong altitude and that the airport did not inform the crew of the weather conditions at the time. A contributing factor was that the airline made all announcements in Chinese and English, while most passengers were Korean.

Excerpt: Korean Air Lines Flight 007 (KAL 007, KE 007) was a Korean Air Lines civilian airliner that was shot down by Soviet interceptors on 1 September 1983, over the Sea of Japan, near Moneron Island just west of Sakhalin island. All 269 passengers and crew aboard were killed, including Lawrence McDonald, a sitting member of the United States Congress. The aircraft was en route from New York City to Seoul via Anchorage when it strayed into prohibited Soviet airspace around the time of a planned missile test. The Soviet Union initially denied knowledge of the incident, but later admitted shooting the aircraft down, claiming that it was on a spy mission. The Politburo said it was a deliberate provocation by the United States, to test the Soviet Union's military preparedness, or even to provoke a war. The United States accused the Soviet Union of obstructing search and rescue operations. The Soviet military suppressed evidence sought by the International Civil Aviation Organization (ICAO) investigation, notably the flight data recorders, which were eventually released eight years later after the collapse of the Soviet Union. The incident was one of the tensest moments of the Cold War, and resulted in an escalation of anti-Soviet sentiment, particularly in the United States. Contributing to the accident were the ground delay between de-icing and takeoff clearance.
