Book excerpt: Civil Society Organizations (CSOs) can make a vital contribution to public health and health systems but harnessing their potential is complex in a Europe where government-CSO relations vary so profoundly. This study is intended to outline some of the challenges and assist policy-makers in furthering their understanding of the part CSOs can play in tandem and alongside government. To this end it analyses existing evidence and draws on a set of seven thematic chapters and six mini case studies. They examine experiences from Austria Bosnia-Herzegovina Belgium Cyprus Finland Germany Malta the Netherlands Poland the Russian Federation Slovenia Turkey and the European Union and make use of a single assessment framework to understand the diverse contexts in which CSOs operate. The evidence shows that CSOs are ubiquitous varied and beneficial and the topics covered in this study reflect such diversity of aims and means: anti-tobacco advocacy food banks refugee health HIV/AIDS prevention and cure and social partnership. CSOs make a substantial contribution to public health and health systems with regards to policy development service delivery and governance. This includes evidence provision advocacy mobilization consensus building provision of medical services and of services related to the social determinants of health standard setting self-regulation and fostering social partnership. However in order to engage successfully with CSOs governments do need to make use of adequate tools and create contexts conducive to collaboration. To guide policy-makers working with CSOs through such complications and help avoid some potential pitfalls the book outlines a practical framework for such collaboration. This suggests identifying key CSOs in a given area; clarifying why there should be engagement with civil society; being realistic as to what CSOs can or will achieve; and an understanding of how CSOs can be helped to deliver.

At first, the group has everything it needs, a thriving membership, and support from major donors. Soon, the group undergoes an identity crisis over money and power, eventually fading from the scene. As government and development institutions embraced activist demands—decentralizing AIDS care through policies of health systems strengthening—civil society was increasingly rendered obsolete. Charting this transition—from subjects, to citizens, and back again—reveals the inefficacy of protest, and the importance of community resilience. The product of in-depth ethnography and focused anthropological inquiry, this is the first book on AIDS activists in Mozambique. AIDS activism’s strange decline in southern Africa, rather than a reflection of citizen apathy, is the direct result of targeted state and donor intervention.

The point of departure is the study of 'new regionalism', which refers to the wave of regional integration globally since the 1980s. However, whilst the current regionalism studies undoubtedly contributes to a deeper understanding of regional processes, important gaps remain, in particular the relatively scant emphasis given to civil society. This particularly relates to regions in the global South, including Southern Africa. The overarching aim of this book is therefore to analyse the dynamics of civil society regionalization in Southern Africa, both empirically and from a theoretical perspective, through analysing the cases of trade and HIV/AIDS. The study finds that CSOs can be more active in regional governance than has previously been conceptualized and are also highly active in terms of constructing regionalization through framing issues and, to a less extent, making identities 'regional'. Furthermore, the book enhances knowledge of the heterogeneous nature of civil society regionalization. Lastly, it is demonstrated that 'going regional' is only partly an autonomous process and also has to be understood as under the influence of the deeper statist and capitalist social structures marking the regional order in Southern Africa.

In the past decade, international funding for the HIV and AIDS response provided by governments rose from about US$1 billion to US$8.7 billion; donors increasingly shifted their financial support toward funding community responses to this epidemic. Yet little is known about the global magnitude of these resource flows and how funding is allocated among HIV and AIDS activities and services. Although some studies have been carried out to gather information on the community response by civil society organizations (CSOs), most of them provide only partial information limited to a specific intervention (for example, orphan support) or specific local communities. To address this knowledge gap, the report attempts to answer the following questions: How large is donor funding for community-based interventions that are run by either large nongovernmental organizations (NGOs) or smaller community-based organizations (CBOs)? How do the funds reach various types of CSOs? What are CSOs' other sources of funding, and to what extent are the CSOs dependent on donor funding? How are these funds used for by CSOs? Are there differences among different types of CSOs working on HIV and AIDS?

During my tenure as President, in 1996, Brazil adopted a law guaranteeing free and universal access to AIDS treatment for all people living with HIV/AIDS. Brazil became the first developing country to provide publicly-financed AIDS treatment for all people living with HIV/AIDS. We now have one of the world’s most successful AIDS programs that is considered a model for other dev- oping countries. Today, 185,000 people receive life-saving AIDS cocktails in Brazil, and thousands of lives have been saved. But this was not an easy battle. There were many challenges along the way. Twenty years ago, Brazil’s achie- ments today might have seemed impossible. During the 1980s, in Brazil, as elsewhere, there was overwhelming stigma associated with AIDS; people living with HIV often lost their jobs and died quickly before the advent of life-saving antiretroviral drugs. Brazil’s AIDS movement was extraordinarily important in promoting progressive AIDS policies; associations of people living with HIV were the first to denounce pervasive AIDS-related discri- nation and called public attention to the importance of AIDS. Activists protested in the streets for over a decade, engaged the media, and framed AIDS as a human rights issue.

The AIDS pandemic was not only catastrophic to the health of millions worldwide but also fractured international relations, global access to new technologies, and public health policies in nations across the globe. As he struggled to get ahead of the disease, Piot found science does little good when it operates independently of politics and economics, and politics is worthless if it rejects scientific evidence and respect for human rights. Piot describes how the epidemic altered global attitudes toward sexuality, the character of the doctor-patient relationship, the influence of civil society in international relations, and traditional partisan divides. AIDS thrust health into national and international politics where, he argues, it rightly belongs. The global reaction to AIDS over the past decade is the positive result of this partnership, showing what can be achieved when science, politics, and policy converge on the ground. Yet it remains a fragile achievement, and Piot warns against complacency and the consequences of reduced investments. He refuses to accept a world in which high levels of HIV infection are the norm. Instead, he explains how to continue to reduce the incidence of the disease to minute levels through both prevention and treatment, until a vaccine is discovered.
