Available in PDF, EPUB and Kindle. Book excerpt: A balanced regulation of bone formation and resorption in the healthy individual is required for a healthy bone. On the other side, there are many factors which can lead to alterations in bone density and microarchitecture. Menopause is a condition which can increase the remodeling process in favor of resorption. Moreover, there are also some diseases, i.e. chronic kidney bone disease, that increase the possibility of fractures and the subsequent disability leading to increased mortality. However, it is clear that drugs are an essential element of the therapy and this issue is analyzed extensively in this book. Some novel pathophysiological mechanisms are also presented, offering advanced knowledge to the reader. The book includes chapters from scientific departments and researchers from all over the world.

Book excerpt: This open access book aims to provide a comprehensive but practical overview of the knowledge required for the assessment and management of the older adult with or at risk of fragility fracture. It considers this from the perspectives of all of the settings in which this group of patients receive nursing care. Globally, a fragility fracture is estimated to occur every 3 seconds. This amounts to 25 000 fractures per day or 9 million per year. The financial costs are reported to be: 32 billion EUR per year in Europe and 20 billon USD in the United States. As the population of China ages, the cost of hip fracture care there is likely to reach 1.25 billion USD by 2020 and 265 billion by 2050 (International Osteoporosis Foundation 2016). Consequently, the need for nursing for patients with fragility fracture across the world is immense. Fragility fracture is one of the foremost challenges for health care providers, and the impact of each one of those expected 9 million hip fractures is significant pain, disability, reduced quality of life, loss of independence and decreased life expectancy. There is a need for coordinated, multi-disciplinary models of care for secondary fracture prevention based on the increasing evidence that such models make a difference. There is also a need to promote and facilitate high quality, evidence-based effective care to those who suffer a fragility fracture with a focus on the best outcomes for recovery, rehabilitation and secondary prevention of further fracture. The care community has to understand better the experience of fragility fracture from the perspective of the patient so that direct improvements in care can be based on the perspectives of the users. This book supports these needs by providing a comprehensive approach to nursing practice in fragility fracture care.

Book excerpt: Since the publication of the first edition, the U.S. Surgeon General released the first-ever report on bone health and osteoporosis in October 2004. This report focuses even more attention on the devastating impact osteoporosis has on millions of lives. According to the National Osteoporosis Foundation, 2 million American men have osteoporosis, and another 12 million are at risk for this disease. Yet despite the large number of men affected, the lack of awareness by doctors and their patients puts men at a higher risk that the condition may go undiagnosed and untreated. It is estimated that one-fifth to one-third of all hip fractures occur in men. This second edition brings on board John Bilezikian and Dirk Vanderschueren as editors with Eric Orwoll. The table of contents is more than doubling with 58 planned chapters. The format is larger – 8.5 x 11. This edition of Osteoporosis in Men brings together even more eminent investigators and clinicians to interpret developments in this growing field, and describe state-of-the-art research as well as practical approaches to diagnosis, prevention and therapy. Available in PDF, EPUB and Kindle. Book excerpt: This new edition includes the most up-to-date information about the pathophysiology and new therapeutic alternatives for osteoporosis of advanced age. It is written by investigators (clinicians and basic scientists) in the field of aging bone research. It was just recently that a new syndrome, osteoporosis of advanced age, was recognized as a new entity with its own pathophysiology and potential treatments. Unfortunately, and despite the fact that hip fractures are mostly a consequence of this process, and apart from the first edition of this book, there are few publications about this subject. The first edition of this book demonstrated that there is a need to understand the particular features of osteoporosis in older persons, which is the main aim of this book. The institutions where the authors are affiliated are highly recognized as the main centers in aging bone research. As an innovation, the second edition includes some aspects of muscle and bone interaction focusing of the impact of age-related muscle loss (sarcopenia) on the pathogenesis of falls and fractures in older persons. As in the previous edition, this book is expected to be the reference guide for geriatricians, gerontologists, and bone biologists as well as medical trainees in order to understand the mechanisms of senile osteoporosis with a practical approach to the treatment of its consequences.

Book excerpt: There are a number of therapies and treatments available for the prevention of fragility fractures in people thought to be at risk, or to prevent further fractures in those who have already had one or more fragility fractures. However, identifying who will benefit from preventative treatment is imprecise. A number of risk assessment tools are available to predict fracture incidence over a period of time, and these may be used to aid decision making. These tools are limited in that they may not include all risk factors, or may lack details of some risk factors. Tools are dependent on the accuracy of the epidemiological data used to derive them and tools validated in other populations may not apply to the UK. Two tools, FRAX and QFracture, are available for use in the UK. It is not clear whether these tools are equally accurate and whether choice of tool should depend on circumstances. This short clinical guideline aims to provide guidance on the selection and use of risk assessment tools in the care of people who may be at risk of fragility fractures in all settings in which NHS care is received.

Available in PDF, EPUB and Kindle. Book excerpt: This first-ever Surgeon General's Report on bone health and osteoporosis illustrates the large burden that bone disease places on our Nation and its citizens. Like other chronic diseases that disproportionately affect the elderly, the prevalence of bone disease and fractures is projected to increase markedly as the population ages. If these predictions come true, bone disease and fractures will have a tremendous negative impact on the future well-being of Americans. But as this report makes clear, they need not come true: by working together we can change the picture of aging in America. Osteoporosis, fractures, and other chronic diseases no longer should be thought of as an inevitable part of growing old. By focusing on prevention and lifestyle changes, including physical activity and nutrition, as well as early diagnosis and appropriate treatment, Americans can avoid much of the damaging impact of bone disease and other chronic diseases. This Surgeon General's Report brings together for the first time the scientific evidence related to the prevention, assessment, diagnosis, and treatment of bone disease. More importantly, it provides a framework for moving forward. The report will be another effective tool in educating Americans about how they can promote bone health throughout their lives. This first-ever Surgeon General's Report on bone health and osteoporosis provides much needed information on bone health, an often overlooked aspect of physical health. This report follows in the tradition of previous Surgeon Generals' reports by identifying the relevant scientific data, rigorously evaluating and summarizing the evidence, and determining conclusions.

Book excerpt: Completely revised and updated, and utilizing the most current evidence and practice guidelines for the treatment of osteoporosis, this comprehensive third edition discusses the basic aspects of bone metabolism, the pathophysiology of osteoporosis, current diagnostic techniques and medical treatment strategies. Osteoporosis is a common disorder that is prevalent in over 20 million Americans over the age of 60. One and a half million osteoporotic fractures occur in the United States every year, including 300,000 hip fractures. Mortality rates after hip fracture approach 25%, with another 50% of patients experiencing a major decrease in their prior level of independence and quality of life. Despite the wide prevalence and severe consequences of osteoporosis, it remains a disorder that is severely under-diagnosed and treated. In this context, specialists and primary care physicians alike are having increasing difficulty keeping up with the rapid changes to the field and incorporating these advances to clinical care. In the years since the last edition of this book was published, the osteoporosis field has changed drastically. In addition to revising and updating existing chapters and removing a few that are no longer as relevant, new chapters discuss an advanced understanding of the cellular and molecular mechanisms underlying the disorder, the introduction of new diagnostic imaging techniques, a more nuanced appreciation of the risks and benefits of osteoporosis therapies, and the introduction of two new classes of osteoporosis medications. Following the format of the second edition, and including helpful key points at the opening of each chapter, this text will present a comprehensive overview of both the basic and clinical concepts relating to each topic, when appropriate. Chapter authors were chosen based on their high level of expertise and leadership in the field. Taken together, this text should thus be of great interest to physicians of multiple specialties, allied health professionals, as well as basic and clinical researchers.

Book excerpt: The second edition of Dr. Sydney Lou Bonnick’s text Bone Densitometry in Clinical Practice is an expansion of her highly regarded first edition, which has provided the bone densitometry community with simply the best, most accurate, and most precisely written resource in our field. Dr. Bonnick has applied her very careful and exact scientific approaches to expand and improve on her widely regarded initial text. In addition to the chapters in the first edition on the science of bone densitometry and its clinical appli- tion, this text has new chapters and a CD-ROM that come at a very critical time in our field. The clinical use of bone densitometry is increasing exponentially as more professional societies have endorsements and guidelines on the application of bone densitometry in the assessment and management of osteoporosis. The recent endorsement of population screening by the US Preventive Services Task Force (USPSTF) has now provided g- ernmental validation to this technology, whose proper use Dr. Bonnick has pioneered. In a new chapter, Dr. Bonnick compares the similarities and differences in the recent gui- lines from the USPSTF and the National Osteoporosis Foundation, American Assoc- tion of Clinical Endocrinologists, American College of Obstetrics and Gynecology, and the North American Menopause Society.
