All these targets have recently been registered or are currently under development for the treatment of rheumatoid arthritis. Each chapter explores the biological rationale of a distinct therapeutic target in great detail. Readers will discover the latest in vitro work, animal models, and results from clinical trials.

Available in PDF, EPUB and Kindle. Book excerpt: Many advances have occurred in the last 5 years especially in understanding genetic susceptibility factors, the role of immune response in chronic arthritis and the relationship between cellular processes that underlie inflammation and tissue damage. All these are dealt with in depth in the individual contributions. Genetics studies included illustrate how studies of HLA genes and genes controlling the autoantibody repertoire may be involved. Environmental factors are believed to be important and studies both in humans and in experimental models on the role of viral and bacterial agents are covered. The fascinating relationship between microbes, dysregulated immune responses and oncogenes are also examined. Since autoimmunity is the key aspect of the immunological response in rheumatoid arthritis, the role of antigens is re-examined as is the part played by immune T and B cells. New areas of clinical application of basic science is well presented in this book, forexample, in examining how markers of cartilage destruction may become important in monitoring disease activity. A chapter reviewing possible future novel therapies is included.

Book excerpt: Rheumatoid arthritis (RA) is the most common and most severe form of inflammatory arthritis. The pathogenesis of RA has been the subject of intense research for several decades. The prevailing hypotheses have changed over the years, and have attempted to incorporate the most recent data. Although T cells represent an important component of the cells which infiltrate the joint synovium, their contribution at a late stage of the disease remains a matter of debate. The goal of this book is to outline the major arguments and data suggesting that T cells may, or may not, be central players in the pathogenesis of chronic RA. While each of the editors and authors has his/her own bias (as will be clear by reading the respective chapters), our hope is that the readers will enjoy a complete and balanced view of the critical questions and experiments. This is not just an intellectual exercise since the direction of future therapeutic interventions depends heavily on how one interprets the pathogenesis of RA and the contribution of T cells.

Available in PDF, EPUB and Kindle. Book excerpt: Our goal for this book is to examine the contemporary therapy of rheumatoid arthritis (RA) from the increasingly important perspective of impact upon quality of life, costs and long-term health outcomes. For too long the focus has been on short term, symptomatic, and surrogate indicator outcomes. Yet RA is a life-long disor der with the majority of impact on an individual patient many years following onset. Further, even in the short-term, researchers and rheumatologists have tended to emphasize measurements of disease activity such as joint counts, ESR and physi cian's opinion as to the amount of disease activity present. It is only relatively recently that measures of structural damage, quality of life and impact on broad domains of health have been given increasing emphasis. Also, the significance of early treatment of RA in order to optimise long-term outcomes has a relatively short history [1]. We have been focussed on the disease processes as surrogates for long term outcomes. Until the short-term process measures are validated as surrogates of long-term effects we should also turn our attention to outcomes of disease and the impact of our management on those outcomes [2). Inour view, this book is especially timely. We are at the dawn of a revolution in the management of RA and other complex immunological inflammatory disorders because their molecular, genetic and environmental mechanisms are being unrav elled. Inthe process, we are revealing a substantial number of novel and significant targets for pharmacotherapy.

Available in PDF, EPUB and Kindle. Book excerpt: This report on Future Trends in Inflammation III is the record of what is now firmly established as a series of multidisciplinary meetings organized by the European Biological Research Association. The aim of these meetings is to provide a forum for free exchange of information between basic scien tists of many disciplines and clinicians to provide better understanding of problems of common interest. The next major meeting wi1l be held in 1980.* The European Biological Research Association promotes scientific and clinical collaboration among the member countries ofthe EEe. It encourages exchange of information between scientists and clinicians from centres all over the world. In addition to the major international meetings small work shops are organized on specific problems of common interest. Once again the Editors have attempted to capture the spirit of the meeting by publishing 'verbatim' the discussion. It can be seen that the discussions were exciting and formed an important part of the meeting. It is possible that inaccuracies have crept into the discussions; if so we apologize. It was decided that the proceedings of such a meeting had maximum value with rapid publication. We would like to thank the participants for the enthusiasm and goodwill which persisted throughout the meeting both scientifically and socially. Above all we wish to thank H.R.H. The Duchess of Kent for acting as Patron of this meeting.

Available in PDF, EPUB and Kindle. Book excerpt: The book deals with the role of both oxygen- and nitrogen-centred free radicals in inflammatory diseases such as rheumatoid arthritis. The well-known involvement of the superoxide anion radical in the bactericidal action of inflammatory cells suggests that radicals and the inflammatory response are inextricably linked. The widespread involvement of radicals in human disease seems inevitable, because inflammation is such a conspicuous component of human disease. For the first time, the present text integrates contributions from leading research groups who have been investigating the role of radicals within the context of all stages of inflammtion, such as the recruitment of inflammatory cells, their bactericidal action, inflammatory tissue destruction and inflammatory cell death by apoptosis. The chapters are broadly organised so that they trace the clinical course of the acute and chronic inflammtory response, emphasising the therapeutic implications of recent data on the contribution of nitric oxide and related nitrogen-centred species. The book will be of interest to academic and industrial researchers and clinicians with interests in the fields of inflammation or free radical biology.
