Book excerpt: Successful management of diabetes depends on psychological adjustment to lifestyle change (Glasgow, 1994). Diabetes is the most common chronic illness among children in the United States (LaPorte, Matsushima, & Chang, 1995). Serious health complications can be avoided only by following complex treatment protocols involving a difficult balance of diet, medication, glucose testing, and exercise (Diabetes Control and Complications Trial Research Group, 1993). Prevalence of diabetes and poor outcomes are disproportionately higher among Latinos than other populations (Walsh, Katz, & Sechrest, 2002).Each aspect of diabetes treatment adherence is affected by psychosocial variables (Schwartz, 1982). Previous research on adherence has focused on the knowledge, attitudes, and beliefs of patients, rather than on exploring patients' direct experience (Vermeire, Hearnshaw, Van Royen, & Denekens, 2001). Cultural barriers to treatment adherence have been identified for pregnant and postpartum Latinas (Oomen, Owen, & Suggs, 1999), but studies of ecocultural variables relevant to adherence in adolescent males is lacking. The proposed is a phenomenological inquiry (Creswell, 1998) into the psychosocial barriers to treatment adherence through Latino adolescents' own voices and perceptions of their treatment for diabetes.The experience of 12 adolescent male Mexican-Americans diagnosed with diabetes will be analyzed based on semi-structured interviews. Participants will review the findings to provide a validity check.Greater knowledge of the barriers to adherence for Latino youth will provide a guide for tailoring interventions to increase their utilization. A deeper understanding of age and culturally specific variables that enhance or block medical treatment could also be used to improve the healthcare delivery system and to provide improved, culturally competent health care for these young people and their families.In order to administer and complete the study, funding is being sought from the W. T. Grant Foundation. The foundation requires applicants to complete and submit an application for the proposed study, including a narrative outlining the questions guiding the proposal, the study rationale and its relevance to the Foundation's mission, a literature review, the specific questions to be addressed, research methods and design, a data analysis plan, anticipated products, and a communication plan.
