Book excerpt: Concernant les patients diabétiques de type 2 (DT2) ayant une résistance majeure à l'insuline et étant mal équilibrés malgré l'optimisation d'un traitement par multi-injections, il convient d'établir le bénéfice d'un traitement par pompe sous-cutanée à insuline (PSCI) et ainsi de mieux situer la place de cette dernière dans la stratégie thérapeutique du patient DT2. Etude rétrospective sur 25 patients DT2 sous PSCI, suivis au Centre Hospitalier Alpes-Léman (CHAL). Recueil à la mise sous pompe (M0) puis à 6,12 et 24 mois (M6, M12, M24) des critères de jugement : HbA1c, poids (et IMC), besoins insuliniques, consultations chez le spécialiste, hospitalisations directement liées au DT2. Pour ces 2 derniers critères, un recueil sur les 24 mois antérieurs à la pompe a été réalisé. Sous PSCI, l HbA1c (%) a nettement diminué de 10,2±1,5 à M0, puis 9,2±1,2 à M6, 8,3±1,1 à M12 et 8,2±1,0 à M24. Les besoins insuliniques (ui/kg/jour) ont diminué de 1,14±0,25 à M0, à 1,05±0,24 à M6 puis 0,95±0,21 à M12 et 0,97±0,22 à M24. Chacun des 25 patients a eu une diminution de l'HbA1c à M12 et M24. Concernant les besoins insuliniques, 24 patients ont eu une diminution des besoins insuliniques à M12 et M24. Les hospitalisations ont diminué de 2,9±1,2, dans les 24 mois ayant précédé la mise sous pompe, à : 1±1 dans les 24 mois qui ont suivi. Ces diminutions sont indépendantes du sexe, de l âge de mise sous PSC) et de l'ancienneté du DT2. Les moyennes de poids (kg) ont diminué de 101,6±16,4 à M0, à 101,4±16,7 à M6, puis 99,3±15,8 à M12, et 98,8±15,9 à M24 sans que l'on ai retrouvé cette tendance chez tous les patients. La réduction, forte et durable, de l'HbA1c, ainsi que la diminution des besoins insuliniques sur les 2 ans de suivi après instauration de la PSCI corrobore les données de la littérature (IRESIST, OPT2MISE). Sur le plan économique, la diminution du nombre moyen d'hospitalisations, malgré un surcoût thérapeutique, permet de définir la PSCI comme un moyen thérapeutique supplémentaire des DT2 déséquilibrés, en complément d'une prise en charge centrée sur le patient et l'observance des mesures hygiéno-diététiques
