Book excerpt: Background: Incivility among nurses continues to be a widespread problem despite national agencies mandating nurses foster safe and healthy work environments. Incivility affects patient outcomes in a negative manner; incivility decreases effective communication and increases physical and emotional distress among nursing staff who experience such behavior in the workplace. Incivility and bullying have been linked to sentinel events, medication errors, decreased quality of care, and poor patient outcomes. Incivility in the workplace can lead to decreased job satisfaction, more illnesses and absences among staff members, and lower nurse retention rates, all of which have a massive financial impact on the healthcare system. Governing bodies have mandated that incivility must be addressed as the negative impact to patients, nurses, providers, and health care systems is too great to ignore. Health care systems must implement interventions to address incivility. Framework: This quality improvement project incorporated Sister Callista Roy's Adaptation Model with the Stevens Star Model of Knowledge Transformation for evidence-based practice change. Methods: An educational intervention was offered via a 14-minute PowerPoint presentation, created by the project manager, with the goals to increase the nursing staff's ability to recognize workplace incivility, reduce workplace incivility on a nursing unit, and to increase confidence confidence in the staff members' ability to respond to workplace incivility when it occurs. Staff were surveyed before and after the education, using the Workplace Incivility Civility Scale. Group mean scores were compared pre- and post-education using independent t-tests. Findings: The educational intervention helped staff recognize uncivil behaviors. The amount of uncivil behavior did not change significantly, as perceived by participants, although there was not a great deal of uncivil behavior observed initially. Staff members' confidence in dealing with incivility did not increase significantly. Conclusion and Recommendations: There are no simple strategies to decrease incivility. Combating incivility requires a multi-faceted approach, including education of staff, implementing a practice change, and teaching staff cognitive rehearsal strategies while offering opportunities to practice the strategies learned. Educating staff to recognize uncivil behavior can decrease wuch behaviors, simply through increased awareness. Nurses can be receptive to such education and will benefit from education aimed at improving their ability to curtail uncivil behavior in the workplace. Recommend future education to offer more interventions and practice opportunities for the staff to improve their skills in handling incivility.

A qualitative methodology was used to determine if incivility affects the nurse educator-staff nurse relationship. A focus group interview was utilized. The results are the perceptions of the nurse educators who participated in the study. The nurse educators from this research group have experienced incivility in nursing education in the hospital environment.

Workplace incivility in nursing has lasting effects on the profession through worsening the nursing shortage, contributing to low morale, negative working environments, safety issues for staff and patients, stress-related health problems, absenteeism, and decreased quality of patient care. Incivility can be from a variety of sources including: Nurse managers, clinical team leads, unit coordinators, physicians, patients, and other healthcare professionals. Buy-in by nursing executive leadership and evidence-based education for nurses is mandatory to address organizational incivility. The purpose of this DNP project is to equip unit nurse leaders with evidence-based practices to improve unit culture related to incivility, thereby enhancing patient outcomes, safety, and staff morale. The identified target population in this project were nurses who are training to be future charge nurses (CNs). The educational intervention facilitated enhancing future CNs’ knowledge, skills and attitudes to shift unit culture to a more civil culture through an online learning module. Data was collected in qualitative and quantitative methods. Qualitative methods involved guided online forum posts at implementation conclusion, while quantitative data involved completion of the Clark Workplace Civility Index© (CWCI). Each method demonstrated future CNs increased their self-awareness and responsiveness by enhancing their knowledge, skills and attitudes toward workplace incivility. In conclusion, the implementation of the online learning module provided effective, successful education and may help in the eradication of workplace incivility for practicing and future nurses.

The problem of academic incivility from the male nursing student perspective has not been previously studied. The purpose of this study was to explore male students' perspectives of faculty incivility and compare male nursing students' perspectives to that of non-nursing male students. Twenty-five non-nursing male students and twenty-seven male nursing students completed a questionnaire about faculty incivility. A majority (92 %) of non-nursing students perceived that ineffective teaching is the most disruptive behavior and 56% have experienced or seen this behavior occur in the last 12 months. While 100% of nursing students perceived that making condescending remarks or putdowns is the most disruptive behavior, the top disruptive behavior experienced or seen in the last 12 months was ineffective teaching (67%). The most frequent threatening faculty behavior perceived by nursing students was general taunts or disrespect to other students (33%), whereas the most frequent threatening faculty behaviors perceived by the non-nursing students was challenges to other faculty knowledge or credibility (24%). Additionally, many male nursing students (89%) perceived academic incivility a mild to serious problem, while only 64% of their male non-nursing student colleagues did. Recommendations for nursing education based on the study findings include: faculty development, communication, setting goals and expectations, diversity awareness and change, and the ability for students to report incivility. Recommendations for nursing practice include recognizing the link of incivility in the workplace and the impact on new graduate nurses, particularly in the areas of retention and patient safety. Addressing incivility in academia could afford the opportunity to break the cycle of incivility before it reaches the nursing environment, mitigating the loss of qualified male nurses or deleterious impacts on patient care.
