Download or read book OSHA 2002 Recordkeeping Simplified written by James Roughton and published by Elsevier. This book was released on 2002-10-22 with total page 497 pages. Available in PDF, EPUB and Kindle. Book excerpt: The Occupational Safety and Health Administration (OSHA) has updated their recordkeeping requirements for the first time since 1971. This results in a significant number of changes for every employer with ten or more employees, which can often cause confusion and failure to comply. OSHA 2002 Recordkeeping Simplified goes beyond the explanation that OSHA supplies to provide an easy understanding of these new requirements.OSHA 2002 Recordkeeping Simplified provides an easy to follow format that allows all those in charge of recordkeeping to comply with the updated standards. The book follows the standards as OSHA provides them and adds commentary in order to explain and simplify. Jim Roughton provides a comparison of the old standards to the new to allow for an easier transition. The text is divided into several major subject sections. First the requirements are addressed to outline new elements compared to the current requirements. Next the supplement information for each subject area is divided into several parts: The first part reviews the relevant sections of the requirement and provides the basic concepts of how recordkeeping works; the second part provides answers to most frequently asked questions about recording and reporting of occupational injuries and illnesses. These questions and answers elaborate on the basic recordkeeping concepts and are further defined in each section. In addition, a series of flow diagrams are used to track the flow of the standard and examples of citations are offered through case histories. - A simplified user's guide to the 2002 OSHA record keeping requirements - Provides answers to frequently asked questions - Offers case histories of citations

Available in PDF, EPUB and Kindle. Book excerpt: To ask the right question, one needs to have some idea of what the answer might be. So it is with remediation. There is no such thing as too much information when it comes to characterizing a site, as information can aid in selecting the best remediation options. Unfortunately, the collection of data for making an informed decision is often costly,
Book Synopsis Corrective Action Investigation Plan for Corrective Action Unit 541 by :
Download or read book Corrective Action Investigation Plan for Corrective Action Unit 541 written by and published by . This book was released on 2014 with total page pages. Available in PDF, EPUB and Kindle. Book excerpt: Corrective Action Unit (CAU) 541 is co-located on the boundary of Area 5 of the Nevada National Security Site and Range 65C of the Nevada Test and Training Range, approximately 65 miles northwest of Las Vegas, Nevada. CAU 541 is a grouping of sites where there has been a suspected release of contamination associated with nuclear testing. This document describes the planned investigation of CAU 541, which comprises the following corrective action sites (CASs): 05-23-04, Atmospheric Tests (6) - BFa Site; 05-45-03, Atmospheric Test Site - Small Boy. These sites are being investigated because existing information on the nature and extent of potential contamination is insufficient to evaluate and recommend corrective action alternatives (CAAs). Additional information will be obtained by conducting a corrective action investigation before evaluating CAAs and selecting the appropriate corrective action for each CAS. The results of the field investigation will support a defensible evaluation of viable CAAs that will be presented in the investigation report. The sites will be investigated based on the data quality objectives (DQOs) developed on April 1, 2014, by representatives of the Nevada Division of Environmental Protection; U.S. Air Force; and the U.S. Department of Energy (DOE), National Nuclear Security Administration Nevada Field Office. The DQO process was used to identify and define the type, amount, and quality of data needed to develop and evaluate appropriate corrective actions for CAU 541. The site investigation process also will be conducted in accordance with the Soils Activity Quality Assurance Plan, which establishes requirements, technical planning, and general quality practices to be applied to this activity. The potential contamination sources associated with CASs 05-23-04 and 05-45-03 are from nuclear testing activities conducted at the Atmospheric Tests (6) - BFa Site and Atmospheric Test Site - Small Boy sites. The presence and nature of contamination at CAU 541 will be evaluated based on information collected from field investigations. Radiological contamination will be evaluated based on a comparison of the total effective dose at sample locations to the dose-based final action level. The total effective dose will be calculated as the total of separate estimates of internal and external dose. Results from the analysis of soil samples will be used to calculate internal radiological dose. Thermoluminescent dosimeters placed at the center of each sample location will be used to measure external radiological dose. Appendix A provides a detailed discussion of the DQO methodology and the DQOs specific to each CAS.
Book Synopsis How to Organize and Run a Failure Investigation by : Daniel P. Dennies
Download or read book How to Organize and Run a Failure Investigation written by Daniel P. Dennies and published by ASM International. This book was released on 2005 with total page 228 pages. Available in PDF, EPUB and Kindle. Book excerpt: Learning the proper steps for organizing a failure investigation ensures success. Failure investigations cross company functional boundaries and are an integral component of any design or manufacturing business operation. Well-organized and professionally conducted investigations are essential for solving manufacturing problems and assisting in redesigns. This book outlines a proven systematic approach to failure investigation. It explains the relationship between various failure sources (corrosion, for example) and the organization and conduct of the investigation. It provides a learning platform for engineers from all disciplines: materials, design, manufacturing, quality, and management. The examples in this book focus on the definition of and requirements for a professionally performed failure analysis of a physical object or structure. However, many of the concepts have much greater utility than for investigating the failure of physical objects. For example, the book provides guidance in areas such as learning how to define objectives, negotiating the scope of investigation, examining the physical evidence, and applying general problem-solving techniques.

Appendix A provides a detailed discussion of the DQO methodology and the DQOs specific to this CAS. The scope of the corrective action investigation for CAU 565 includes the following activities: (1) Remove stored samples, shelves, and debris from the interior of Building 26-2106. (2) Perform field screening on stored samples, shelves, and debris. (3) Dispose of stored samples, shelves, and debris. (4) Collect samples of investigation-derived waste, as needed, for waste management purposes. (5) Conduct radiological surveys of Building 26-2106 in accordance with the requirements in the ''NV/YMP Radiological Control Manual'' to determine if there is residual radiological contamination that would prevent the release of the building for unrestricted use. This Corrective Action Investigation has been developed in accordance with the ''Federal Facility Agreement and Consent Order'' that was agreed to by the State of Nevada, the U.S. Department of Energy, and the U.S. Department of Defense. Under the ''Federal Facility Agreement and Consent Order'', this Corrective Action Investigation Plan will be submitted to the Nevada Division of Environmental Protection for approval. Field work will be conducted following approval of the plan.
Book Synopsis Remediation Manual for Petroleum Contaminated Sites by : David L. Russell
Download or read book Remediation Manual for Petroleum Contaminated Sites written by David L. Russell and published by CRC Press. This book was released on 1992-11-23 with total page 188 pages. Available in PDF, EPUB and Kindle. Book excerpt: Based on proven investigation into cleanup techniques, the material in this manual gives engineers a working knowledge of the field and a basis for making key decisions during the cleanup process. It is easy to get petroleum into the ground, but hard to get it back out again. The problem does not exist in the groundwater or the soil alone, but in a dynamic balance between the soil, water, and air in the soil. Gasoline and any of its volatile components can and do move between the soil, air and water, and they cost billions of dollars every year in contamination cleanup. This new book provides the information needed for cost- and time-effective petroleum-contaminated site cleanup. Originally developed as an oil company's training aid, this book discusses the hows and whys of site cleanup-geology, data gathering, cleanup alternatives, remediation costs, and manage-ment of consultants/contractors Plus the_book contains a criti-cal examination of the benefits and limitations of each type of remediation technology. There is never only one way to clean up a particular site. The choice of the method of cleanup is one of the greatest factors affecting the cost. Another important cost factor is the level of cleanup required to satisfy the regulatory cleanup community. This new book will not only provide a fundamental under-standing of site remediation techniques but also the knowledge to make cost-effective, environmentally-sound choices during the remediation process. Copies of this handy manual are available for immediate delivery. An order form follows the detailed table of contents on the reverse.

Two FAA Aviation Safety Inspectors have provided evidence raising serious questions of conduct violating the Fed. Aviation Reg¿s. (FARs) in the inspection and maint. program of Southwest Airlines (SWA). FAA employees have engaged in conduct, ¿which constitutes a violation of Fed. law, rule or reg¿n., gross mismgt., an abuse of authority and a substantial damage to public safety.¿ The Maint. Inspector for SWA knowingly allowed the airline to operate in March 2007 (and possibly beyond), and well after the inspection deadlines on a mandatory FAA Airworthiness Directive. There may be a pattern of regulatory abuse and that these regulatory lapses may be more widespread. Illustrations.
