Book excerpt: Plutonium research and development activities have resided at the Los Alamos National Laboratory (LANL) since 1943. The function of the Plutonium Facility (PF-4) has been to perform basic special nuclear materials research and development and to support national defense and energy programs. The original Final Safety Analysis Report (FSAR) for PF-4 was approved by DOE in 1978. This FSAR analyzed design-basis and bounding accidents. In 1986, DOE/AL published DOE/AL Order 5481.1B, ''Safety Analysis and Review System'', as a requirement for preparation and review of safety analyses. To meet the new DOE requirements, the Facilities Management Group of the Nuclear Material Technology Division submitted a draft FSAR to DOE for approval in April 1991. This draft FSAR analyzed the new configurations and used a limited-scope probabilistic risk analysis for accident analysis. During the DOE review of the draft FSAR, DOE Order 5480.23 ''Nuclear Safety Analysis Reports'', was promulgated and was later officially released in April 1992. The new order significantly expands the scope, preparation, and maintenance efforts beyond those required in DOE/AL Order 5481.1B by requiring: description of institutional and human-factor safety programs; clear definitions of all facility-specific safety commitments; more comprehensive and detailed hazard assessment; use of new safety analysis methods; and annual updates of FSARs. This paper describes the safety analysis report (SAR) upgrade program at the Plutonium Facility in LANL. The SAR upgrade program is established to meet the requirements in DOE Order 5480.23. Described in this paper are the SAR background, authorization basis for operations, hazard classification, and technical program elements.

Book excerpt: At the request of the Assistant Manager for Safety and Engineering, the U.S. Department of Energy Richland Operations Office (RL) Engineering Support Division, performed an oversight review of the Plutonium Finishing Plant (PFP) nuclear Criticality Alarm System (CAS). The review was conducted to satisfy requirements and agreements associated with Defense Nuclear Facility Safety Board (DNFSB) Recommendation 2000-2, ''Vital Safety Systems.'' The PFP is managed by Fluor Hanford, Inc. for RL. The field assessment and staff interviews were conducted August 12 through August 19,2002. This was a limited scope assessment that consisted of a review of the nuclear CAS operations, maintenance, and compliance with National Consensus Standards Requirements. The main purpose of the assessment was to determine the adequacy of the existing alarm system and its associated infrastructure to support the PFP facility mission through the remaining facility lifetime. The Review Plan was modeled upon Criteria and Review Approach Documents (CRAD) developed for DNFSB Recommendation 2000-2 reviews conducted across the Hanford Site. Concerns regarding component degradation and failure, increasing numbers of occurrence reports associated with the alarm system, and reliability issues were addressed. Additionally, RL performed a review of the engineering aspects of the CAS including the functions of design authorities and aspects of systems engineering. However, the focus of the assessment was on operations, maintenance, and reliability of the CAS, associated procurement practices, adequacy of safety and engineering policies and procedures, safety documentation, and fundamental engineering practices including training, qualification, and systems engineering. This assessment revealed that the PFP CAS and its associated infrastructure, administrative procedures, and conduct of operations are generally effective. There are no imminent criticality safety issues associated with the operation of the existing CAS. The Assessment Team believes that the CAS, as it presently exists at the PFP facility, is adequate to support the remaining mission lifetime of the facility while continuing to ensure personnel safety. This conclusion is dependent upon a continued level of funding adequate to support the required maintenance and occasional system upgrade. Two findings were identified during this assessment. Additionally, the report identified eight observations and two recommendations. The assessment revealed that recent changes to OSR compliance procedures and other documents do not contain the signature of the CSR as required by procedure. Lack of appropriate approval signatures is a noncompliance with site-level procedures.

The bulk of this activity occurs at Technical Area -55 (TA-55), the nations only operable plutonium facility. TA-55 has developed and demonstrated a number of technologies that significantly minimize waste generation in plutonium processing (supercritical CO2, Mg(OH)2 precipitation, supercritical H2O oxidation, WAND), disposition of excess fissile materials (hydride-dehydride, electrolytic decontamination), disposition of historical waste inventories (salt distillation), and Decontamination & Decommissioning (D & D) of closed nuclear facilities (electrolytic decontamination). Furthermore, TA-55 is in the process of developing additional waste minimization technologies (molten salt oxidation, nitric acid recycle, americium extraction) that will significantly reduce ongoing waste generation rates and allow volume reduction of existing waste streams. Cost savings from reduction in waste volumes to be managed and disposed far exceed development and deployment costs in every case. Waste minimization is also important because it reduces occupational exposure to ionizing radiation, risks of transportation accidents, and transfer of burdens from current nuclear operations to future generations.

The basis for this assessment is the research done at Los Alamos National Laboratory (LANL) in identifying and resolving the technical issues surrounding the production of PuO2 feed, removal of gallium from the PuO2 feed, the fabrication of test fuel, and the work done at the LANL plutonium processing facility. The use of plutonium in MOX fuel has been successfully demonstrated in Europe, where the experience has been almost exclusively with plutonium separated from commercial spent nuclear fuel. This experience in safely operating MOX fuel fabrication facilities directly applies to the fabrication and irradiation of MOX fuel made from surplus weapons plutonium. Consequently, this paper focuses on the technical difference between plutonium from surplus weapons, and light-water reactor recycled plutonium. Preliminary assessments and research lead to the conclusion that no new process or product safety concerns will arise from using surplus weapons plutonium in MOX fuel.

Book excerpt: Recently the Final Safety Analysis Report (FSAR) for the Plutonium Facility at Los Alamos National Laboratory, Technical Area 55 (TA-55) was revised and submitted to the US. Department of Energy (DOE). As a part of this effort, over seventy Process Hazards Analyses (PrHAs) were written and/or revised over the six years prior to the FSAR revision. TA-55 is a research, development, and production nuclear facility that primarily supports US. defense and space programs. Nuclear fuels and material research; material recovery, refining and analyses; and the casting, machining and fabrication of plutonium components are some of the activities conducted at TA-35. These operations involve a wide variety of industrial, chemical and nuclear hazards. Operational personnel along with safety analysts work as a team to prepare the PrHA. PrHAs describe the process; identi fy the hazards; and analyze hazards including determining hazard scenarios, their likelihood, and consequences. In addition, the interaction of the process to facility systems, structures and operational specific protective features are part of the PrHA. This information is rolled-up to determine bounding accidents and mitigating systems and structures. Further detailed accident analysis is performed for the bounding accidents and included in the FSAR. The FSAR is part of the Documented Safety Analysis (DSA) that defines the safety envelope for all facility operations in order to protect the worker, the public, and the environment. The DSA is in compliance with the US. Code of Federal Regulations, 10 CFR 830, Nuclear Safety Management and is approved by DOE. The DSA sets forth the bounding conditions necessary for the safe operation for the facility and is essentially a 'license to operate.' Safely of day-to-day operations is based on Hazard Control Plans (HCPs). Hazards are initially identified in the PrI-IA for the specific operation and act as input to the HCP. Specific protective features important to worker safety are incorporated so the worker can readily identify the safety parameters of the their work. System safety tools such as Preliminary Hazard Analysis, What-If Analysis, Hazard and Operability Analysis as well as other techniques as necessary provide the groundwork for both determining bounding conditions for facility safety, operational safety, and day-to-clay worker safety.
