L'une, vitale ou simplement ressentie, aboutit à un diagnostic et un traitement immédiat, l'autre s'apparente souvent à une situation de crise ci nécessite une attention particulière avec temps, disponibilité et compétence. Pourtant elles se présentent toutes deux à l'hôpital général et il faut y répondre. Une enquête réalisée pendant deux mois au C.H.G. du Havre a permis d'étudier la fréquentation psychiatrique du service des urgences ( en l'absence de psychiatre sur place ) : il existe entre 14 et 19 % d'admissions pour motif psychiatrique, le plus souvent chez des gens jeunes pour tentative de suicide: 80 % d'entre eux sont hospitalisés en médecine, avec de nombreuses récidives â la sortie. La crise constitue un phénomène en extension, que les patients préfèrent exprimer dans un lieu neutre. Cependant, l'accueil médicochirurgical n'est pas adapté pour eux et les médecins somaticiens non formés à cette pathologie, d'où une mauvaise prise en charge. Par ailleurs, lorsqu'il est isolé, l'accueil des urgences psychiatriques ne permet pas une sécurité organique. Ce sont deux services complémentaires. A certains endroits, des solutions ont été trouvées pour assurer une permanence d'accueil de qualité dans un site banalisé, avec possibilité de soins immédiats, tant psychiatriques qu'organiques. Elles vont de la garde de psychiatrie dans le service des urgences médicochirurgicales à l'unité d'accueil psychiatrique au sein de l'hôpital général, en passant par le centre d'accueil et de crise. Compte tenu des diverses structures hospitalières existant au Havre, sous la dépendance d'une administration unique, une unité de psychiatrie de liaison permettrait de résoudre les problèmes locaux.

Il existe donc au HAVRE un service d'urgence médico-chirurgicale, l'Accueil MédicoChirurgical, l'AMC, de l'Hôpital Jacques Monod, et un Service d'Accueil et d'Urgence psychiatrique, le SAU, situé lui au Centre Pierre Janet. Pourtant, l'AMC reste un des lieux privilégiés des urgences psychiatriques. En 1993, on constate que les pathologies psychiatriques représentent près de 25 % des motifs d'hospitalisation ou encore 11 % de l'activité des services médicaux-chirurgicaux. C'est pour répondre à cet état de fait qu'une unité de consultations psychiatriques a été ouverte en Mai 92 au sein des urgences médico-chirurgicales du centre Hospitalier du HAVRE. Elle a pour vocation, à la fois de permettre un accueil des urgences psychiatriques à l'hôpital général et également un suivi psychiatrique des patients hospitalisés dans les services médicochirurgicaux. Nous pensons que ces deux problèmes ne sont que deux versants d'une question identique qui est à traiter dans une dynamique globale menée par une équipe unique dans le cadre de la psychiatrie publique.

Ce phénomène est avant tout visible dans les services d’urgence des hôpitaux généraux. Devant ce constat et dans un contexte global de fermeture de lits d’hospitalisation, des alternatives ont été développées, parmi lesquelles, les centres d’accueil et de crise (CAC). Méthode : Au moyen d’une revue de la littérature nous nous proposons de détailler l’évolution de l’approche française face aux urgences en psychiatrie, puis nous développerons les notions de crise et de travail de crise en psychiatrie. Les apports d’un CAC seront détaillés à l’aide des résultats d’une étude lilloise. Résultats : L’intérêt porté aux situations d’urgence en psychiatrie est un phénomène assez récent. La meilleure connaissance de ces situations a amené à les classer en deux catégories : la psychiatrie en urgence (30%) et l’urgence psychiatrique (70%). Cette deuxième catégorie représente les urgences médico-psycho-sociales évoluant souvent dans un contexte relationnel et qui ne trouvent pas leur place à l’hôpital psychiatrique. Le développement du travail de crise et l’ouverture des centres d’accueil et de crise ont contribué à améliorer l’accueil, l’accès aux soins, la prévention et de préciser la demande de soins psychothérapeutiques dans le domaine de l’urgence psychiatrique. L’étude lilloise portant sur l’accueil des urgences psychiatriques au CHRU de Lille de 2000 à 2004, démontre que l’ouverture d’un CAC contribue à une diminution significative du nombre total d’hospitalisations et plus spécialement des hospitalisations sous contrainte.

Political thinkers reasoned that if government was to rest in the hands of individuals, then measures should be taken to understand the deepest reaches of the self, including the state of madness. Marcel Gauchet and Gladys Swain maintain that the asylum originally embodied the revolutionary hope of curing all the insane by saving the glimmer of sanity left in them. Their analysis of why this utopian vision failed ultimately constitutes both a powerful argument for liberalism and a direct challenge to Michel Foucault's indictment of liberal institutions. The creation of an artificial environment was meant to encourage the mentally ill to live as social beings, in conditions that resembled as much as possible those prevailing in real life. The asylum was therefore the first instance of a modern utopian community in which a scientifically designed environment was supposed to achieve complete control over the minds of a whole category of human beings. Gauchet and Swain argue that the social domination of the inner self, far from being the hidden truth of emancipation, represented the failure of its overly optimistic beginnings. Madness and Democracy combines rich details of nineteenth-century asylum life with reflections on the crucial role of subjectivity and difference within modernism. Its final achievement is to show that the lessons learned from the failure of the asylum led to the rise of psychoanalysis, an endeavor focused on individual care and on the cooperation between psychiatrist and patient. By linking the rise of liberalism to a chapter in the history of psychiatry, Gauchet and Swain offer a fascinating reassessment of political modernity.
