Book excerpt: Introduction : Les accidents de décompression (ADD) neurologique d'origine médullaire sont les plus fréquents des ADD en plongée sous-marine, avec un risque de séquelle de 30%. Les objectifs de ce travail ont été d'affiner la description clinique de l'ADD médullaire et les déterminants de l'évolution initiale, de confirmer la pertinence du score MEDSHUBHYP développé pour cette affection afin de proposer un guide pratique de l'ADD médullaire pour les médecins généralistes. Méthode : Il s'agit d'une étude descriptive analytique rétrospective monocentrique d'une cohorte de 183 patients, construite prospectivement à partir des patients pris en charge à l'HIA Sainte-Anne de Toulon pour un accident de plongée, entre 2010 et 2018. Après étude descriptive des antécédents, des paramètres de plongée et des présentations cliniques, la pertinence du score MEDSHUBHYP a été évaluée, puis des groupes de patients de sévérités initiales et d'évolutivités différentes ont été comparés par analyse statistique univariée. Résultats : Les patients étaient majoritairement des hommes, entre 40 et 60 ans, plongeurs expérimentés et en bon état général. Les plongées étaient assez profondes, successives dans la moitié des cas, et sans erreur de procédure. L'intérêt du score MEDSUBHYP pour l'évaluation de la sévérité clinique initiale et la prédiction de l'évolutivité précoce a été démontré. Le terrain cardiovasculaire semblait associé aux formes cliniques les plus sévères d'ADD alors qu'un antécédent d'ADD, une douleur vertébrale ou une dysfonction sphinctérienne semblaient plus fréquemment observés en cas d'évolutivité péjorative. Conclusion : Cette étude a permis d'apporter un éclairage nouveau sur la clinique de l'ADD médullaire, de préciser les déterminants de la sévérité et de l'évolutivité clinique initiale et de confirmer la pertinence clinique du score MEDSUBHYP. Elle souligne aussi l'intérêt de poursuivre les travaux sur la physiopathologie de l'ADD médullaire et sur les déterminants du pronostic à long terme.

Matériels et méthode : 195 plongeurs (moyenne 48± 12 ans, 42 femmes) présentant des symptômes d'ADD médullaire ont été inclus dans une étude rétrospective et monocentrique. Les renseignements concernant la plongée, la latence d'apparition des symptômes, le délai de recompression et la prise en charge pré-hospitalière ont été étudiés. La sévérité clinique initiale a été estimée par le score MEDSUBHYP et le niveau des séquelles a été évalué à la sortie du centre hyperbare par le score modifié de l'association orthopédique japonaise (MJOAS). Résultats : 34.3% présentent des séquelles neurologiques à la sortie du centre hyperbare. Après analyse multivariée, seules 2 variables indépendantes sont significativement associées à une récupération neurologique incomplète : le score de MEDSUBHYP et le délai de recompression. Parmi ces derniers, l'accident de décompression, lié à la formation de bulles d'azote au cours de la remontée, est le plus fréquent de tous et ses séquelles peuvent être gravissimes. La prise en charge rapide avec oxygénothérapie et orientation vers un caisson hyperbare restent les composantes essentielles du processus de soin. Lors de la plongée classique, à l'air, le plongeur respire environ 79 % d'azote et 21 % d'oxygène. Depuis quelques années, le Nitrox, un air enrichi en oxygène a fait son apparition au sein de la plongée loisir. De part sa plus faible teneur en azote, celui-ci limite la formation de bulles pathogènes et donc les accidents de décompression. Mais la profondeur de la plongée est limitée, sous peine de se confronter à la toxicité pulmonaire et neurologique de l'oxygène, présent en plus grande quantité. On estime aujourd'hui que les procédures de décompression ont prouvé leur efficacité. La diminution du nombre d'accidents doit alors se concentrer essentiellement sur une modification du comportement du plongeur qui trop souvent se permet certains écarts de sécurité, une bonne condition physique et une meilleure détection des sujets à risques. Dans cette optique de sécurité, le Nitrox présente un avantage indéniable quand on sait que l'accident de décompression représente près de 75 % des accidents en plongée sous-marine.
