Cette étude propose de présenter l'état actuel des connaissances et des recommandations pour la prise en charge médicale et de rééducation fonctionnelle de l'AVCia en France, avec pour illustration le recueille des informations médicales des enfants hospitalisés au Centre Hospitalo-universitaire de Caen lors des 10 dernières années, ainsi que le détail et la discussion de la prise en charge de cinq cas cliniques. Ce travail vise à développer les pistes de réflexions sur le développement de la prise en charge de l'AVCia de l'enfant, dans une société de plus en plus attentive au niveau d'efficacité des soins et à la qualité de vie de ces jeunes citoyens en devenir.

Book excerpt: L’accident vasculaire cérébral (AVC) fait l’objet d’un plan national d’actions qui repose sur 4 priorités : prévention, prise en charge, formation des professionnels et promotion de la recherche. Cette pathologie est rare dans la population pédiatrique, néanmoins approfondir les connaissances optimiserait la prise en charge précoce et secondaire. La présence de facteurs prédictifs d’évolution à la phase initiale pourrait orienter la thérapeutique mais également la rééducation motrice et cognitive. Objectifs: Analyser le devenir à moyen et à long terme des enfants ayant présenté un AVC ischémique, et d’établir si des éléments cliniques et/ou para-cliniques précoces pourraient représenter des facteurs prédictifs du devenir. Matériels et méthodes : Nous avons conduit une étude rétrospective retraçant la prise en charge initiale diagnostic et thérapeutique des enfants ayant présenté un AVC ischémique à un âge pédiatrique (1 mois-18 ans) aux CHU de Rennes et Angers. Puis une évaluation du devenir prospectif a été réalisée à l’aide de 2 questionnaires envoyés aux parents. Les enfants ont été classés en 2 groupes selon leur devenir (favorable ou défavorable) afin d’évaluer l’impact des données cliniques et para-cliniques initiales sur leur devenir. Résultats : Nous avons inclus 43 patients, le devenir est connu pour 35 patients (70%). L’âge médian au moment de l’AVC était de 4,3 ans (0,3-17,5) dont 49% des enfants avaient entre >1 ans et 5 ans. Onze enfants n’avaient aucune séquelle (31%), 10 enfants avaient des séquelles mineures à modérées (29%), 12 enfants avaient des séquelles sévères (34%) et 2 sont décédés (6%). La qualité de vie était altérée dans tous les domaines, en particulier l’école et les émotions. Les facteurs prédictifs d’évolution défavorable étaient les troubles de conscience initiaux avec un OR=73 (IC 95% 9,64-1633), les convulsions avec un OR= 7,12 (IC 95% : 1,32-56), le score PedNIHSS rétrospectif>11 avec une OR= 1,24 (1,08-1,51) et la localisation corticale (p“0,001). Il n’y avait pas de corrélation retrouvée entre l’étiologie, l’âge et le devenir (p=0,07). Conclusions : Des éléments pronostiques sont donc présents dès la phase initiale et leur reconnaissance représente un outil d’évaluation pronostique fiable pour déterminer le risque de séquelles majeures. Une étude complémentaire sera nécessaire pour analyser spécifiquement les facteurs potentiellement associés à des séquelles modérées à minimes, fréquentes dans cette population. Ces facteurs prédictifs représenteraient des arguments supplémentaires pour la mise en place de nouveaux protocoles de prise en charge à la phase initiale et secondairement pour optimiser la rééducation motrice et cognitive.
