Les programmes d'échanges transfusionnels constituent actuellement le principal traitement de référence, tant en prévention primaire que secondaire. Mais cette thérapie est le plus souvent instituée sans échéance définie, alors que les effets secondaires à long terme sont loin d'être négligeables. Ce travail de thèse s'est intéressé à la validité des échanges transfusionnels chroniques en tant que traitement optimal de la vasculopathie cérébrale de l'enfant drépanocytaire. Couplant les données disponibles dans la littérature avec les informations tirées de l'observation de 29 enfants intégrés en programme d'échanges à l'hôpital Necker, nous avons tenté de dégager les réels bénéfices ainsi que les limites de chacune des options thérapeutiques actuellement disponibles. Il s'est avéré que seule la transplantation de moelle offrait des perspectives de guérison, moyennant des effets indésirables parfois importants. Néanmoins, lorsque celle-ci n'est pas possible, les échanges transfusionnels restent la seule alternative ayant fait ses preuves. Au cours de notre étude, des questions se sont spontanément dégagées, sans que la recherche bibliographique puisse nous éclairer : nous nous sommes aperçus de l'étendue de notre ignorance concernant les mécanismes des accidents vasculaires cérébraux dans la drépanocytose, ainsi que de notre large méconnaissance des véritables effets des divers traitements. Nous avons donc consacré une partie de notre discussion à élaborer un inventaire (non exhaustif) des innombrables énigmes dont les solutions pourraient modifier sensiblement notre stratégie de prise en charge.

Les enfants malades sont exposés, entre autre, au risque d'accident vasculaire cérébral. Cette complication concerne les populations SS et SpO thalassémiques. L'objectif de cette étude rétrospective était de faire un état des lieux du dépistage des complications vasculaires cérébrales chez l'enfant drépanocytaire en Martinique, de 2004 à 2007. Nous avons étudié 124 enfants âgés de 2 à 18 ans, drépanocytaires SS ou SpO thalassémiques sans antécédent d'accident vasculaire cérébral suivis au Centre Intégré de la Drépanocytose. La proportion retrouvée des principales complications aiguës de la maladie correspondait aux données de la littérature. Nous avons ensuite étudié la population des enfants ayant au moins eu une consultation médicale entre 2004 et 2007 : 105 enfants. Parmi ces enfants, 78% avaient bénéficié d'un dépistage de la vasculopathie cérébrale par échographie Doppler transcrânienne (EDTC). 15% présentaient des EDTC pathologiques et 7,5% avaient une vasculopathie confirmée. Un des facteurs associés retrouvé était l'atopie. Une prise en charge thérapeutique adaptée permettait d'éviter la survenue d'accidents vasculaires cérébraux.

In the wake of Hurricane Katrina, public agencies subcontracted disaster relief to private companies that turned the humanitarian work of recovery into lucrative business. These enterprises profited from the very suffering that they failed to ameliorate, producing a second-order disaster that exacerbated inequalities based on race and class and leaving residents to rebuild almost entirely on their own. Filled with the often desperate voices of residents who returned to New Orleans, Markets of Sorrow, Labors of Faith describes the human toll of disaster capitalism and the affect economy it has produced. While for-profit companies delayed delivery of federal resources to returning residents, faith-based and nonprofit groups stepped in to rebuild, compelled by the moral pull of charity and the emotional rewards of volunteer labor. Adams traces the success of charity efforts, even while noting an irony of neoliberalism, which encourages the very same for-profit companies to exploit these charities as another market opportunity. In so doing, the companies profit not once but twice on disaster.
