Available in PDF, EPUB and Kindle. Book excerpt: Data sharing can accelerate new discoveries by avoiding duplicative trials, stimulating new ideas for research, and enabling the maximal scientific knowledge and benefits to be gained from the efforts of clinical trial participants and investigators. At the same time, sharing clinical trial data presents risks, burdens, and challenges. These include the need to protect the privacy and honor the consent of clinical trial participants; safeguard the legitimate economic interests of sponsors; and guard against invalid secondary analyses, which could undermine trust in clinical trials or otherwise harm public health. Sharing Clinical Trial Data presents activities and strategies for the responsible sharing of clinical trial data. With the goal of increasing scientific knowledge to lead to better therapies for patients, this book identifies guiding principles and makes recommendations to maximize the benefits and minimize risks. This report offers guidance on the types of clinical trial data available at different points in the process, the points in the process at which each type of data should be shared, methods for sharing data, what groups should have access to data, and future knowledge and infrastructure needs. Responsible sharing of clinical trial data will allow other investigators to replicate published findings and carry out additional analyses, strengthen the evidence base for regulatory and clinical decisions, and increase the scientific knowledge gained from investments by the funders of clinical trials. The recommendations of Sharing Clinical Trial Data will be useful both now and well into the future as improved sharing of data leads to a stronger evidence base for treatment. This book will be of interest to stakeholders across the spectrum of research-from funders, to researchers, to journals, to physicians, and ultimately, to patients.

Available in PDF, EPUB and Kindle. Book excerpt: Healthcare decision makers in search of reliable information that compares health interventions increasingly turn to systematic reviews for the best summary of the evidence. Systematic reviews identify, select, assess, and synthesize the findings of similar but separate studies, and can help clarify what is known and not known about the potential benefits and harms of drugs, devices, and other healthcare services. Systematic reviews can be helpful for clinicians who want to integrate research findings into their daily practices, for patients to make well-informed choices about their own care, for professional medical societies and other organizations that develop clinical practice guidelines. Too often systematic reviews are of uncertain or poor quality. There are no universally accepted standards for developing systematic reviews leading to variability in how conflicts of interest and biases are handled, how evidence is appraised, and the overall scientific rigor of the process. In Finding What Works in Health Care the Institute of Medicine (IOM) recommends 21 standards for developing high-quality systematic reviews of comparative effectiveness research. The standards address the entire systematic review process from the initial steps of formulating the topic and building the review team to producing a detailed final report that synthesizes what the evidence shows and where knowledge gaps remain. Finding What Works in Health Care also proposes a framework for improving the quality of the science underpinning systematic reviews. This book will serve as a vital resource for both sponsors and producers of systematic reviews of comparative effectiveness research.

Available in PDF, EPUB and Kindle. Book excerpt: Over the course of the last decade, the treatment of colorectal cancer has evolved quite rapidly. New scientific and clinical advances have modified the standard of care and led to improved patient outcomes. At the same time, the treatment of colorectal cancer has become increasingly complex, requiring the comprehensive review and assessment of multiple issues including genetics, radiology, surgery, molecular diagnostics, chemotherapy, and more. As a result the harmony and open communication between these specialties facilitated by a multidisciplinary team approach are crucial in providing the best care to patients and ensuring successful treatment. Cancers of the Colon and Rectum: A Multidisciplinary Approach to Diagnosis and Management, written by a multidisciplinary team of authors representing a range of disciplines, is a valuable resource for physicians, fellows, nurses, physician assistants, physical therapists, and all health care providers involved in the treatment of colorectal cancer. Cancers of the Colon and Rectum: A Multidisciplinary Approach to Diagnosis and Management summarizes the state-of-the-art issues related to the treatment of colorectal cancer and describes an approach for optimal multidisciplinary care for individuals who have been diagnosed with or who are at higher risk to develop colorectal cancer. About the Series: The Current Multidisciplinary Oncology series, edited by Charles R. Thomas, consolidates and integrates the varied aspects of multidisciplinary care for major topics in oncology, including breast, lung, prostrate, head and neck and more. Book excerpt: In this book dealing with the lung health, the authors focus on various fields, spreading from pulmonary oncogenesis, to inflammatory and parasitic lung diseases. The first section deals with the fundamental research on lung cancer that is mandatory for the development of novel and early biomarkers for diagnosis of the lung cancer. This development could be enhanced using experimental models despite the species barrier. Mouse models can help us understand the sequence of events involved in human lung neoplasia and their underlying molecular mechanisms. The results of the research could be used to identify novel targets for the development of new biological therapies. In the second section of this book, the role of inflammation in various respiratory diseases is outlined. The authors recall cellular mechanisms including neutrophils to improve the understanding of the phenomenon and help develop targeted therapies. The third section on parasitic tropical lung disease highlights the growing importance of neglected tropical diseases due to increased traffic across the continents and migration of the population. Physicians need to be aware of the symptoms and imaging findings of these diseases mainly in travelers and immigrants from tropical endemic areas.

Multiple Myeloma is the second most common hematological malignancy in the United States. This book highlights the interplay between the malignant plasma cell and the microenvironment, and the role of genetic alterations, adhesion molecules and critical cytokines in this complex milieu. The first chapter of this book discusses epidemiology, etiology and pathogenesis of multiple myeloma. The clinical presentation of the common plasma cell dyscrasias and discuss the diagnostic and prognostic work-up with a highlighting on recent advances in chapters 3 and 4. Chapters from 5 to 9 focus on the therapeutic evolution that has occurred, and accumulate the landmark clinical trials that have changed clinical practice for up-front treatment of both transplant eligible and ineligible patients and for those with relapsed and refractory disease. These chapters also highlights on the important role of clinical research with stem cell transplantation, proteasome-inhibition and immunomodulatory drugs that form the backbone of our present day therapeutic armamentarium. Recommendations for response assessment and current understanding of the value of maximizing the depth of response in personalized therapy and patient segmentation are briefly explained in chapters 10 and 11. Chapter 12 and 13 discussed about adjunctive treatment modalities and novel therapeutic compounds in clinical trial and targets for future drug development. This book provides complete information about current landscape for multiple myeloma which is helpful in future aspects.
