Available in PDF, EPUB and Kindle. Book excerpt: This book, part of the European Society of Intensive Care Medicine textbook series, provides detailed up-to-date information on the physical, cognitive, and psychological impairments that are frequently present following a stay in an intensive care unit and examines in depth the available preventive and therapeutic strategies, including adapted rehabilitation programs. Beyond acquainting readers with the multiple facets of post-intensive care syndrome (PICS), the book aims to promote the effective follow-up of patients, thereby enhancing their ability to work and their functional autonomy, and to identify risk factors for the development of PICS as a stimulus to beneficial organizational changes in intensive care departments. The background to the book is the realization by healthcare providers that the quality of life of patients who have required a stay in an intensive care unit can be severely impaired or even become unacceptable. All too often, the diverse sequelae are overlooked by specialists of other disciplines. Moreover, families and caregivers are also at high risk of post-traumatic stress disorder and depression. The European Society of Intensive Care Medicine has developed the Lessons from the ICU series with the vision of providing focused and state-of-the-art overviews of central topics in Intensive Care and optimal resources for clinicians working in Intensive Care. This book, written by renowned experts in the field, will facilitate the transmission of key knowledge with significant clinical and financial benefits.

Available in PDF, EPUB and Kindle. Book excerpt: This book is open access under a CC BY 4.0 license. It constitutes a unique source of knowledge and guidance for all healthcare workers who care for patients with sepsis and septic shock in resource-limited settings. More than eighty percent of the worldwide deaths related to sepsis occur in resource-limited settings in low and middle-income countries. Current international sepsis guidelines cannot be implemented without adaptations towards these settings, mainly because of the difference in local resources and a different spectrum of infectious diseases causing sepsis. This prompted members of the Global Intensive Care working group of the European Society of Intensive Care Medicine (ESICM) and the Mahidol-Oxford Tropical Medicine Research Unit (MORU, Bangkok, Thailand) - among which the Editors – to develop with an international group of experts a comprehensive set of recommendations for the management of sepsis in resource-limited settings. Recommendations are based on both current scientific evidence and clinical experience of clinicians working in resource-limited settings. The book includes an overview chapter outlining the current challenges and future directions of sepsis management as well as general recommendations on the structure and organization of intensive care services in resource-limited settings. Specific recommendations on the recognition and management of patients with sepsis and septic shock in these settings are grouped into seven chapters. The book provides evidence-based practical guidance for doctors in low and middle income countries treating patients with sepsis, and highlights areas for further research and discussion.

International clinical practice guidelines for the management of sepsis are presented, and individual chapters focus on aspects such as fluid resuscitation, vasopressor therapy, response to multiorgan failure, antimicrobial therapy, and adjunctive immunotherapy. The closing section looks forward to the coming decade, discussing novel trial designs, sepsis in low- and middle-income countries, and emerging management approaches. The book is internatio nal in scope, with contributions from leading experts worldwide. It will be of value to residents and professionals/practitioners in the fields of infectious diseases and internal medicine, as well as to GPs and medical students.

Particular attention is devoted to source control in the management of the infection, antimicrobial therapy and sepsis support, which represent the cornerstones of treating patients with this problem. The importance of a multidisciplinary approach is highlighted not only by the instructive and informative sections on the acute manifestations of appendicitis, cholecystitis, and cholangitis, perforations and diverticulitis, Clostridium difficile infection and the role of candida in abdominal sepsis, but also by an extensive discussion of issues including antimicrobial resistance, damage control surgery, principles of antimicrobial therapy, hemodynamic support, adjunctive therapies, and thromboprophylaxis. Abdominal sepsis is the host’s systemic inflammatory response to intra-abdominal infections. It is associated with significant morbidity and mortality rates, and represents the second most common cause of sepsis-related mortality at intensive care units. Gathering contributions by authoritative experts from all around the world, this book will allow acute care surgeons, abdominal surgeons, intensive care clinicians, and students to broaden their understanding of intra-abdominal sepsis in daily clinical practice.

Book excerpt: Continuous renal replacement therapies (CRRT) started off as an alternative to hemo- or peritoneal dialysis. Today's machines and techniques are the result of 4 decades of developments, studies, and practices which can be divided into 4 distinct stages: exploration and development; birth of a new specialty called critical care nephrology; design of specific new devices and machines; and interaction among various specialists to adapt extracorporeal therapies for multiple organ support and sepsis. This book features contributions from prominent CRRT experts from around the world. It is an important tool for educating a new generation of nephrologists and intensivists. At the same time, it provides the most advanced CRRT users with the latest technological information, the most updated clinical evidence, and the personal opinion of key leaders who contributed to the last 40 years of history in the field.

Book excerpt: Te practice of intensive care medicine is at the very forefront of titration of treatment andmonitoringresponse. Te substrateofthiscareisthe criticallyill patientwho,by defnition, is at the limits of his or her physiologic reserve. Such patients need immediate, aggressive but balanced life-altering interventions to minimize the detrimental aspects of acute illness and hasten recovery. Treatmentdecisionsandresponsetotherapyareusually assessed by measures of physiologic function, such as assessed by cardio-respiratory monitoring. However, how one uses such information is ofen unclear and rarely supported by prospective clinical trials. In reality, the bedside clinician is forced to rely primarily on physiologic principles in determining the best treatments and response to therapy. However, the physiologic foundation present in practicing physicians is uneven and occasionally supported more by habit or prior training than science. A series of short papers published in Intensive Care Medicine since 2002 under the heading Physiologic Notes attempts to capture the essence of the physiologic perspectives that underpin both our understanding of disease and response to therapy. Tis present volume combines the complete list of these Physiologic Notes up until July 2006 with the ass o cia t ed r e vie w a r tic les o v er t h e s a m e in t er val t ha t a ls o addr ess e d t hes e cen tral issues.

Book excerpt: Organ transplantation as a high-cost surgical and medical procedure poses an extraordinarily rich and complex set of social, ethical, and policy issues. A June 1988 symposium at Vanderbilt University gathered leaders in a wide variety of fields to synthesize the current state of knowledge concerning organ transplantation policy and to access policy options. Collected here are the revised papers presented at that symposium; also included is one influential earlier paper on the same topic. Together, they constitute a major contribution to the debate on organ transplantation policy and its moral, legal, financial, and political implications.

Patient-generated data can contribute precious information to healthcare professionals by enabling monitoring under normal life conditions and also helping patients play a more active role in their own care. This book presents the proceedings of MEDINFO 2019, the 17th World Congress on Medical and Health Informatics, held in Lyon, France, from 25 to 30 August 2019. The theme of this year’s conference was ‘Health and Wellbeing: E-Networks for All’, stressing the increasing importance of networks in healthcare on the one hand, and the patient-centered perspective on the other. Over 1100 manuscripts were submitted to the conference and, after a thorough review process by at least three reviewers and assessment by a scientific program committee member, 285 papers and 296 posters were accepted, together with 47 podium abstracts, 7 demonstrations, 45 panels, 21 workshops and 9 tutorials. All accepted paper and poster contributions are included in these proceedings. The papers are grouped under four thematic tracks: interpreting health and biomedical data, supporting care delivery, enabling precision medicine and public health, and the human element in medical informatics. The posters are divided into the same four groups. The book presents an overview of state-of-the-art informatics projects from multiple regions of the world; it will be of interest to anyone working in the field of medical informatics.
