Book excerpt: The sound pressure pT at the human eardrum has essential advantages as reference signal in audiological and psychoacoustical experiments. Unfortunately, precise pressure measurements very close to the tympanic membrane are difficult. In practice, the microphone has to be positioned at a certain distance from the eardrum. The measured pressure then has to be transformed to the eardrum. As a "classical" approach for the estimation of the necessary transfer function, an acoustical network model of the ear canal is developed from geometrical data of the canal (cross-sectional area function) which is in turn determined from measurements of its acoustical input impedance. Such methods, however, do not provide robust results. In this thesis, the concept of one-dimensional models of the ear canal is examined to find the origin of these errors. For this purpose, the sound field at the human external ear was analyzed using finite element models. Inside the canal, irregular three-dimensional structures occur that cannot be modelled accurately using classical one-dimensional network concepts. Upon these findings, an accurate, efficient and highly feasible method for the estimation of pT was developed. Equal-loudness level contours with reference to the eardrum pressure that were measured as pilot application of the new method are presented.

This initial venture was planned and undertaken when the present editors and their colleagues realised the need for a closer interaction and dialogue between the clinician and. those basic scientists working in the area of implant surgery. Thi s interface, together with the real interface at the material/tissue borders, thus forms the basis of the present conference. These two ideas, we felt, were nicely and effectively captured in the drawing by Edgar Rubins (1915), a perception psycho 1 ogi st, used on the book cover and elsewhere in our 1 iterature. The Proceedings were planned with some difficulty, due to the wi de scope of the conference. However, we felt the best format was to follow the logical progression of implant development. The introductory papers and talks therefore demonstrate the scope of surgical implants in current use. The development of an implant starts with modelling of the proposed implant and its potential environment and the proceedings follow the same format. Following this, materials in current use are discussed.

It was with these questions in mind that we organized the 1985 Mechanics of Hearing Workshop, to bring together auditory researchers to compare models with experimental observations. Tbe workshop forum was inspired by the very successful 1983 Mechanics of Hearing Workshop in Delft [1]. Boston University was chosen as the site of our meeting because of the Boston area's role as a center for hearing research in this country. We made a special effort at this meeting to attract students from around the world, because without students this field will not progress. Financial support for the workshop was provided in part by grant BNS- 8412878 from the National Science Foundation. Modeling is a traditional strategy in science and plays an important role in the scientific method. Models are the bridge between theory and experiment. Tbey test the assumptions made in experimental designs. They are built on experimental results, and they may be used to test hypotheses and predict experimental results. Tbe latter is the scientific method at its best. Cochlear function is very complicated. For this reason, models play animportant role. One goal of modeling is to gain understanding, but the necessary mathematical tools are often formidably complex. An ex am pie of this is found in cochlear macromechanics.

Book excerpt: The history of surgery for chronic suppurative otitis media (CSOM) is littered with stories of techniques and materials that were tried with enthusiasm but without a reasonable expectation of long-term success. The early papers on tympanoplasty, and sadly even some still today, contain numerous short-term follow-up reports of new procedures that looked as if they might revolutionise ear surgery. By the nature of the problem, the longer-term results often proved to be poor and by the nature of humans, the original author didn't report this. As a result, perhaps hundreds of patients elsewhere were subjected to surgery that was already doomed to failure even before the operations started. The journals of the sixties and seventies contain many reports which most would rather forget. I began my otologic career in the early sixties, at the height of the otologic renaissance, and so often we felt that with just one more push conductive deafness would become a thing of the past. Now we know that the problems of middle ear reconstruction are multifactorial and that we are not simply looking for one single step to restore and maintain hearing and stability. What we need, is a greater understanding of why some procedures succeed and others fail. Happily in this book, Professor Ars and his team of leading clinicians and scientists, begin by looking at the ranges of causes of the problems. They then move on to examine the essentials needed for getting good results, and they do this in the light of those things which we know at the outset cannot possibly succeed. Professor Ars has gathered together an excellent group of otologists who start with the initial causes of the problems and move on to look for the essentials of success. Having identified these, they go on to explore how we can lay the foundations to achieve them. Finally, they look at the operative procedures they consider most likely to achieve success.
