Book excerpt: Perioperative anesthesia related mortality: a retrospective cohort study with 11.562 anesthetics procedures Introduction: Studying the postoperative in-hospital mortality rate is crucial to understanding the perioperative care process failures and implementing strategies that improve patient outcomes. Anesthesia-related perioperative deaths are rare and have been declining over the past 50 years. However there are a small number of patients at high-risk for complications and death that would benefit with a new approach. Objectives: To evaluate the incidence, preoperative risks factors, and classify the cause of perioperative deaths up to 30 days of procedures requiring anesthesia. Methods: A retrospective cohort study was performed at a reference university Hospital in south Brazil evaluating the data related to pre, trans, and postoperative characteristics of surgical patients between January, 2012 and December, 2013. The details of the surgery and anesthesia from those inpatients that died post-surgery were ascertained from the Information Management System. Deaths were reviewed and classified by three Anesthesiologists (according to ANZCA Classification) into three major groups: Attributable to Anesthesia, Not Related to Anesthesia, and Unassessable Death. Results: A total of 11562 surgeries were performed resulting in 321 (2,77%) 30-day perioperative death. Most deaths occurred after 48 hours (76,6%) in patients ASA physical state 3 or higher (86,9%) submitted to major (59,2%) urgent (54,4%) surgeries, in which a small death percentage was attributable to anesthesia (0,6%) or had apparent anesthesia contribution (3,8%). Most of the deaths were classified as inevitable and have occurred irrespectively of anesthesia or surgical procedures (50%). Surgical contribution was pointed in 25,3% of the death cases, and 16,3% were considered incidental death, which was not related to the indication for surgery and was not due to factors under the control of the anesthetist or surgeon. The death attributable to anesthesia was due to pulmonary aspiration and those with possible anesthesia contribution were related to inappropriate adverse event management. Sepsis was the most final death case (45,3%) followed by advanced cancer (20,6%). The procedure most frequent was Exploratory laparotomy (21,8%) followed by arterial by-pass and colorectal resection. Conclusion: The qualitative analysis of perioperative deaths contributes to the recognition of assistance failures and to define plans to prioritize the most problematic areas that could be improved. Our study confirmed that death attributable to anesthesia is a rare event, therefore it signalizes the urgent necessity to advance in the perioperative care of patients scheduled to exploratory laparotomies, specially those associated to sepsis.

Book excerpt: 113 This system of monitoring might be improved further if all cases in this category were investigated jointly by a forensic pathologist and an anaesthesiologist at the very outset of the investigation and during the actual autopsy. Free Papers The Influence of Pancuronium on Primary Conjugated Bile Acids A. Fassoulaki, T. Mihas, A. Mihas and P. Kaniaris Experimental studies have been reported in which a prolonged action of steroid neuro muscular blocking agents followed bile acids administration(1, 2). A prolonged neuro muscular action of pancuronium has also been detected in patients with biliary obstruc tion(3). In the present study serum bile acids and intracellular liver enzymes were estimated after anaesthesia in which pancuronium was used as a muscle relaxant (Fig. 1). An at tempt is made to investigate the effect of pancuronium on bile acid levels. Material and Methods Twelve female patients aged between 40 and 50 years were studied. All of them were visited in the ward the night before the operation by the anaesthesiologist where their clinical condition was assessed and their consent was obtained to participate in the pre sent study. None of them had a history of hepatic disease or was taking drugs. The oper ation performed in all cases was modified radical mastectomy and intracellular liver en zymes are routinely determined in these patients preoperatively. The twelve patients were reassured and stated that they did not want to have any tranquillizer for premedi cation.

Book excerpt: Second in a series of publications from the Institute of Medicine's Quality of Health Care in America project Today's health care providers have more research findings and more technology available to them than ever before. Yet recent reports have raised serious doubts about the quality of health care in America. Crossing the Quality Chasm makes an urgent call for fundamental change to close the quality gap. This book recommends a sweeping redesign of the American health care system and provides overarching principles for specific direction for policymakers, health care leaders, clinicians, regulators, purchasers, and others. In this comprehensive volume the committee offers: A set of performance expectations for the 21st century health care system. A set of 10 new rules to guide patient-clinician relationships. A suggested organizing framework to better align the incentives inherent in payment and accountability with improvements in quality. Key steps to promote evidence-based practice and strengthen clinical information systems. Analyzing health care organizations as complex systems, Crossing the Quality Chasm also documents the causes of the quality gap, identifies current practices that impede quality care, and explores how systems approaches can be used to implement change.

Available in PDF, EPUB and Kindle. Book excerpt: This annual edition presents the most recent statistics since 1990 of over 80 health indicators for WHO's 193 Member States. This fourth edition includes an expanded set of over 76 key indicators and a section with 10 highlights in global health statistics in the past year. This book has been collated from publications and databases of WHO's technical programmes and regional offices. The core set of indicators was selected on the basis of relevance for global health, availability and quality of data, and accuracy and comparability of estimates. The statistics for the indicators are based on an interactive process of data collection, compilation, quality assessment, and estimation between WHO technical programmes and its Member States. In this process, WHO strives to maximize accessibility, accuracy, comparability, and transparency of country health statistics. In addition to national statistics, this publication presents statistics on the inequalities in health outcomes and interventions coverage within countries, disaggregated by urban/rural setting, wealth/assets, and educational level. Such statistics are primarily derived from the analysis of household surveys and are only available for a limited number of countries.
