I now publish together all the papers I have written on Fever, because many of my medical friends have from time to time urged me to do it, and also because all the facts detailed and analysed were observed and recorded at the bedside and in the dead-house by myself. While collecting some of these facts in 1847 I caught typhus fever, and three or four years later typhoid fever. I mention this because it was said at the time, 'Before typhus and typhoid fevers can be said to be absolutely different diseases some one must be found who has suffered from both, ' and I was the first, so far as I know, who at that time could be proved to have suffered from both. Book excerpt: Excerpt from International Clinics, Vol. 2: A Quarterly of Illustrated Clinical Lectures and Especially Prepared Original Articles; Twenty-Fifth Series, 1915 The trend of modern medicine is to utilize every available means for clearing up the Obscure conditions which baffle the diagnostic skill of the physician. Our attention is now being directed to the examination of the cerebrospinal fluid. The value of this examina tion as a differential diagnostic agent in many Obscure neurologic and psychologic disturbances is steadily growing in importance. N 0 direct analysis can be made in nervous and mental diseases, so our efforts must be expended in searching the secretions of the body for evidences of the products which attend pathologic alterations of the nervous tissue, particularly the brain. Our first efforts re sulted in failure, because the search was made of the blood, urine, and faeces, which represent the sum total of all of the metabolic products of the body. Nervous tissue comprises only about two per cent. Of the total body, hence one can readily understand why such a small amount of products escapes detection. Examination of the cerebrospinal fluid has been more promising, and at the present time this examination is considered necessary as an aid in diagnosis in all Obscure nervous conditions. The cerebrospinal fluid comes into more intimate relation with nervous tissue than the other secretions, so should contain the products of metabolism of nervous tissue in more concentrated and in least altered form. Its onset has an average age of 30, and occurs in about 1 in 500 individuals of European ancestry living primarily in temperate climates. There appears to be a complex interaction between a genetic predisposition and an environmental trigger that initiates the disease. But these facts do not convey the impact of the disease on the people whose lives it affects. In this elegantly written and comprehensive history, we meet individuals who suffered with MS in the centuries before the disease had a name, including blessed Lidwina of Holland, who took joy from her misery, believing that she was sent to accept suffering for the sins of others; Augustus d'Est, grandson of George III and cousin of Queen Victoria, whose case shows how someone with access to the best of medical care of the age was understood and managed; and Heinrich Heine, the great German poet, who also had access to all medical services that were available, but who progressed into his mattress grave in two decades, aware of the loss of physical ability while still able to compose great poetry to the end. From these early cases the author demonstrates how progress in diagnosing and managing multiple sclerosis has paralleled the development of medical science, from the early developments in modern studies of anatomy and pathology, to the framing of the disease in the nineteenth century, and eventually to modern diagnosis and treatment. From beginning to end, Dr. Murray takes us on a fascinating journey of discovery, in the process showing how the evolution of our understanding of multiple sclerosis has been part of the greater history of medical knowledge.
