Book excerpt: Despite the magnitude of the HIV pandemic, the events involved in the initial HIV-1 entry into the body are not yet fully understood. Although the principle mode of HIV-1 transmission is through mucosal surfaces, the oral epithelium appears to be less susceptible to HIV-1 infection than vaginal epithelium. In addition, infections with co-pathogens that elicit immune activation, such as Candida albicans, may also promote HIV-1 infection. The project objectives are to determine whether (i) HIV-1 is able to bind and integrate into oral and vaginal epithelial cell lines, (ii) epithelial cell lines are able to transfer viable virus from their surface to permissive cells, (iii) epithelial cells are responding to HIV-1 with changes in intracellular signalling or gene expression profiling, and (iv) Candida albicans can affect epithelial susceptibility to HIV-1 or whether it can bind and/or transfer HIV-1 to permissive cells. We demonstrate that oral, oro-pharyngeal and vaginal epithelial cell lines do not express canonical receptors for HIV-1 but they do express other receptors known to promote HIV-1 binding, including GalCer and syndecan-1. Oral and vaginal epithelial cell models can capture HIV-1, which subsequently does not appear to integrate into the epithelial genome. Therefore, viral replication is not supported. Notably, HIV-1 captured on the epithelial surface remains infectious and can be transferred to permissive cells. Furthermore, like epithelial cell lines, C. albicans can also directly bind and transfer HIV-1 to permissive cells. The carbohydrate moieties chitin and (3-glucan appear to play a role in mediating viral binding. Notably, transfer of HIV-1 to permissive cells occurs from chitin but minimally from p-glucan. This indicates that fungal-viral interactions may occur at mucosal surfaces that potentially promote HIV-1 infection.

Understanding the impact of semen on mucosal transmission of HIV could open up novel means of preventing the sexual transmission of HIV. To understand how human semen modulates female reproductive tract susceptibility to HIV infection, we collected human semen from healthy donors after informed consent and separated seminal plasma from spermatozoa via centrifugation. Using post-hysterectomy human female reproductive tract epithelial cells and peripheral blood mononuclear cells, we investigated the impact of seminal plasma on the phenotype, activity and HIV infectivity of the female reproductive tract epithelial cells, CD4+ regulatory T cells and the primary cellular targets of HIV infection, the CD4+ T cells. In cells from the female reproductive tract, seminal plasma enhanced the epithelial cell barrier function and reduced the epithelial cell secretion of RANTES and IL-6. In CD4+ T cells, seminal plasma promoted the non-proliferative expansion of CD4+ regulatory T cells with enhanced suppressive function via increased secretion of the immunosuppressive cytokine TGF-[beta]. Seminal palsma reduced CD4+ T cell surface expression of the HIV receptor CD4, activation and proliferation and protected CD4+ T cells from infection with both, X4 and R5 tropic HIV. Conversely, the seminal plasma-mediated increase in the female reproductive tract epithelial cell and CD4+ T cell surface expression of the HIV co-receptor CCR5 mitigated its protective effects, contributing to the preferential infection by the R5 tropic HIV. The observed seminal plasma protection of CD4+ T cells from HIV infection was mediated by heat stable seminal plasma components of intermediate molecular weight. These data suggest that human semen impacts the barrier integrity and immune responses by both the epithelial cells and CD4+ T cells in the female reproductive tract and accounts for both the relative inefficiency and preferential transmission of R5 tropic HIV. The rich intermediate molecular weight components of seminal plasma mediate CD4+ T cell protection from HIV infection. These findings have the potential to lead to the development of novel approaches to prevent the ongoing global sexual transmission of HIV.

Book excerpt: HIV and pathogenic SIV infection are characterized by chronic gastrointestinal (GI) mucosal dysfunction, which contributes to morbidities and mortality. This includes damage to the epithelial barrier, loss of Th17 cells, microbial translocation and local and systemic inflammation and immune activation. The mechanisms underlying mucosal damage have not been fully elucidated. The broad purpose of this thesis was to address several unanswered questions relating to mechanisms and potential therapies for GI mucosal dysfunction in HIV infection. We aimed to address three main objectives: 1) longitudinally assess the kinetics of GI neutrophil accumulation and reduced neutrophil antimicrobial function in relation to mucosal dysfunction, immune activation, and Th17 cell depletion in acute SIV infection; 2) investigate increased neutrophil lifespan as a mechanism for GI neutrophil accumulation in chronic, treated HIV infection and a potential role for HIV altered mucosal bacteria; and 3) assess the safety and efficacy of fecal microbial transplantation (FMT) as a potential therapeutic for mucosal dysfunction in SIV infection. The role of neutrophils in HIV gastrointestinal mucosal dysfunction is one major theme of the studies described herein. Neutrophils are important for containment of pathogens, but can also contribute to tissue damage due to their release of reactive oxygen species and other potentially harmful effector molecules. Here, we demonstrate that immune alterations such as Th17 loss, prolonged intestinal neutrophil accumulation, and decreased neutrophil functionality occur after the onset of peripheral and mucosal T cell activation and evidence of microbial translocation, and are thus unlikely to be drivers of initial damage. We additionally demonstrate that neutrophils are recruited to the site of lentivirus infection, maintain antimicrobial function in acute infection, and may therefore participate in the antiviral immune response. In chronic treated HIV infection, we report that reduced homeostatic neutrophil apoptosis contributes to neutrophil accumulation in the gastrointestinal tissues, thus implicating neutrophil lifespan as a new therapeutic target for mucosal inflammation in HIV infection. Microbial dysbiosis in HIV infection is a second major underlying theme of this thesis. During HIV infection, the delicate balance of healthy bacterial communities is perturbed, and this dysbiosis associates with T cell activation and disease progression. Here, we report differential effects of HIV-altered mucosal bacteria on neutrophil survival, suggesting that dysbiosis may contribute to GI neutrophil accumulation in treated HIV. We also provide evidence that Lactobacillus species increase neutrophil apoptosis and decrease neutrophil frequency in vitro, which could have important therapeutic implications for reducing neutrophil-driven inflammation in HIV and other chronic inflammatory conditions. Finally, we demonstrate the safety of FMT in primates infected with lentivirus and examine the relationships between alterations to the microbiome and immunological parameters. We observed increased Th17 and Th22 cells as well as decreased activation of CD4+ T cells post-FMT, and these changes correlated most strongly across all sampling time points with lower abundance genera in the colon. Taken together, the data presented in this thesis improves our understanding of the mechanisms contributing to mucosal dysfunction and potential therapies that can be further investigated to improve overall health in HIV-infected individuals.

A comprehensive approach that includes nutritional interventions is likely to maximize the benefit of antiretroviral therapy in preventing HIV disease progression and other adverse outcomes in HIV-infected men and women. Modification of nutritional status has been shown to enhance the quality of life of those suffering HIV/AIDS, both physically in terms of improved body mass index and immunological markers, and psychologically, by improving symptoms of depression.
