Despite current protocols to identify risks for diabetes, a gap exists in diagnosing and treating Filipinos with the disease. Knowledge about Filipinos and their health is imprecise because national data used to track disease prevalence does not identify Filipinos separately from the Asian and Asian-Pacific Islander group. The purpose of this study is to identify factors that influence effective diabetes self-management among a sample of Filipinos. Specific aims include: 1. Describe the characteristics of a sample of Filipinos with pre-diabetes and Type 2 diabetes mellitus. 2. Examine the relationships between health literacy, self-efficacy, depression, select demographic variables and characteristics (e.g. socio-economic status, age, gender, marital status, education level, year of migration to the U.S., language spoken at home and in the community, language read and understand, years diagnosed with diabetes, presence of blood glucose machine, oral glucose prescription, smoking, hypertension, exercise) and diabetes self-management practices in a sample of Filipinos with pre-diabetes and Type 2 diabetes. 3. Identify factors that increase the probability (odds) for effective self-management of HgbA1C among a sample of Filipinos with pre-diabetes and Type 2 diabetes mellitus. 4. To gain a broader understanding of the barriers to prescribed self-management practices in a subset of Filipinos with pre-diabetes and Type 2 diabetes mellitus. Methods: Quantitative and Qualitative approaches were used for this study. Quantitative: A descriptive correlational design. A convenience sample of 99 Filipinos was recruited and enrolled over a 6-month period, from 2 private practice clinics and the community in southern California. Inclusion Criteria: 18 to 70 years old, pre-diabetes (HgbA1C 5.7-6.4%) or Type 2 diabetes (HgbA1C ≥ 6.5%). Exclusion Criteria: Type 1 diabetes, and medical diagnosis of dementia or depression. Participants completed demographic questions, Diabetes Management Self-Efficacy Scale (DMSES), Patient Health Questionnaire (PHQ-9) and the 3-Health Literacy (3-HL) scale. Descriptive statistics were used for all analysis variables to describe participant characteristics. Spearman Rho correlation and logistic regression analyses were used to examine the relationships among patient characteristics, diabetes self-efficacy, diabetes related emotional distress, diabetes health literacy, and HgbA1C level. A logistic regression model containing 5 independent variables (takes pills, hypertension, blood glucose machine, times blood glucose checked, and years diagnosed with diabetes) was statistically significant X2(5, N=94) = 40.62, p Implications: This study addressed this gap in the knowledge of the predictors of effective diabetes self-management among a sample of Filipinos. Study outcomes provide valuable information to inform practitioners, administrators, and policymakers in the development and implementation of more efficient, patient-based care to increase type 2 diabetic patient engagement in self-management practices, improve patient satisfaction, health, and lower healthcare costs.

The diagnostic process is a complex, collaborative activity that involves clinical reasoning and information gathering to determine a patient's health problem. According to Improving Diagnosis in Health Care, diagnostic errors-inaccurate or delayed diagnoses-persist throughout all settings of care and continue to harm an unacceptable number of patients. It is likely that most people will experience at least one diagnostic error in their lifetime, sometimes with devastating consequences. Diagnostic errors may cause harm to patients by preventing or delaying appropriate treatment, providing unnecessary or harmful treatment, or resulting in psychological or financial repercussions. The committee concluded that improving the diagnostic process is not only possible, but also represents a moral, professional, and public health imperative. Improving Diagnosis in Health Care, a continuation of the landmark Institute of Medicine reports To Err Is Human (2000) and Crossing the Quality Chasm (2001), finds that diagnosis-and, in particular, the occurrence of diagnostic errorsâ€"has been largely unappreciated in efforts to improve the quality and safety of health care. Without a dedicated focus on improving diagnosis, diagnostic errors will likely worsen as the delivery of health care and the diagnostic process continue to increase in complexity. Just as the diagnostic process is a collaborative activity, improving diagnosis will require collaboration and a widespread commitment to change among health care professionals, health care organizations, patients and their families, researchers, and policy makers. Self-monitoring of blood glucose (SMBG) is advocated as a method to better achieve control. The Key Questions were: Key Question 1. Is regular SMBG effective in achieving target A1c levels for patients with type 2 diabetes? Key Question 2. Is regular SMBG effective in maintaining target A1c levels for patients with type 2 diabetes? Key Question 3. Does regular SMBG reduce the frequency of hypoglycemia in patients with type 2 diabetes? Key Question 4. Is there evidence that different frequencies of testing result in differences in improvements in A1c? METHODS: We searched PubMed from 2004-2006 using standard search terms. We performed an update search in July 2007. Titles, abstracts, and articles were reviewed in duplicate by physicians trained in the critical analysis of literature. Data were extracted by quantitative analysts. Pooled analyses were performed for trials with A1c outcomes at six months and 12 months or greater of follow-up. All other data were narratively summarized. RESULTS: We screened 52 titles, 14 were rejected, and we performed a more detailed review on 38 articles. From this, we identified 14 randomized controlled trials (RCTs) that measured the effect of SMBG compared to a group not receiving SMBG and monitored A1c levels with at least three months of follow-up. Four trials were excluded; one because it presented duplicate data and three because they evaluated SMBG in both the control and intervention groups, leaving 10 trials contributing to the efficacy analysis. We identified five observational studies assessing effectiveness in diabetic Veterans.

Book excerpt: Background: It China, more than 20 million people were diagnosed with diabetes in 2000 and it was estimated there would be 42.3 million people with diabetes by 2030 (Wild, Roglic, Green, Sicree, & King, 2004). Diabetes mellitus self-management (DMSM) is considered the cornerstone for controlling diabetes and preventing diabetic complications. To develop effective interventions to promote DMSM in China, it is critical to identify the factors influencing DMSM relevant to this population. The purposes of this study were to test a hypothesized model describing the effects of individual and environmental factors on DMSM in patients with type 2 diabetes in China, and to test if the estimated effects are the same between two subgroups, patients treated with insulin vs. patients treated without insulin. The individual factors included diabetes knowledge, beliefs of treatment effectiveness, and diabetes self-efficacy. Environmental factors included social support from family members and provider-patient communication. Methods: A cross-sectional survey research design was used in this study. The study was conducted at a major hospital in Beijing China. A convenience sample of 201 Chinese adult patients with type 2 diabetes mellitus who attended the outpatient department of the Endocrinology unit at the hospital from September 2004 to November 2004 was obtained for this study. Data were collected using a self-reported questionnaire. Structural equation modeling (SEM) analysis was conducted to test the hypothesized model, and examine and explore the relationships between and among the individual and environmental factors and DMSM. Findings: The initial hypothesized model did not fit the observed data. Based on statistical findings and theoretical soundness, the model was modified. The final model had a good fit to the data. The key findings were that belief in treatment effectiveness and diabetes self-efficacy were key proximal factors affecting DMSM. Knowledge and the environmental factors indirectly affected DMSM through belief in treatment effectiveness and self-efficacy. The findings based on the multi-group SEM analysis indicated that patient-provider communication was a more important factor affecting DMSM for the insulin-treated subgroup, while family support was more important for the non-insulin treated subgroup. Conclusions: The good fit of the final model indicated that significant determinants of DMSM described in American studies were also important predictors of DMSM in Chinese with diabetes. The model illustrated that individual and environmental factors affected DMSM both directly and indirectly. Understanding the complex relationships between and among the individual and environmental factors and DMSM helps the practitioner to design intervention programs promoting DMSM using a more comprehensive approach. The practitioner should also be aware of the differences between the subgroups when developing intervention programs.

Book excerpt: The purpose of this study was to assess the impact of using an online personal health record (PHR) on clinical outcomes of patients with type 2 diabetes. This study used an intervention mixed methods approach with a quantitative randomized, controlled trial combined with a qualitative exploration of how patients used the PHR. A total of 140 participants with type 2 diabetes were randomized to either receive a PHR and training to use the PHR, or to a control group who received usual care. Both clinical and social cognitive outcomes were measured and compared at baseline and follow-up, three to six months after baseline. A linear regression was also used to model the relationship between PHR use, social cognitive factors, and hemoglobin A1c (HbA1c). The quantitative analysis revealed no differences in diabetes clinical outcomes (HbA1c or blood glucose) from baseline to follow-up between either the control and intervention groups, or within the intervention group, between individuals who continued to use the PHR after baseline (PHR users) and individuals who ceased use after baseline (PHR non-users). Additionally, the linear regression revealed no significant relationships between PHR use, social cognitive factors, and HbA1c. In-depth interviews were conducted with all members of the intervention group to explore how patients used the PHR in their diabetes self-management. Themes that emerged from the qualitative analysis were used to explain quantitative findings and identify continued research gaps that must be addressed in future research. Nine themes emerged from the qualitative analysis, seven of which expressed barriers that helped to explain the lack of quantitative difference between the groups. The additional two themes identified positive outcomes from PHR use that support hypothesized benefits from PHR use. Future research should focus on the integration of the PHR into patients’ regular diabetes self-care routine and identifying which features of the PHR have the potential to lead to clinical improvement.
