One of the current health management areas where ICTs have been employed is in supporting communications between patients and physicians. This is particularly relevant for patients suffering from chronic diseases since there is evidence that better communication leads to better health outcomes. Researchers are investigating different chronic diseases to design and test technology interventions to promote better chronic disease care. However, few have investigated pediatric asthma as a case study for designing communication technologies. The World Health Organization (WHO) estimated in 2008 that 300 million people suffer from asthma, and that asthma is the most common chronic illness among children. Asthma interferes with breathing by preventing airflow into the lungs. It is difficult to determine the actual cause of asthma and to predict who will have asthma. These unique challenges provide opportunities to investigate pediatric asthma management. To address these challenges, I have conducted a series of studies with pediatric asthma patients, families, and healthcare providers to better understand their needs, challenges and strategies regarding the use of technologies. I have conducted interviews, a focus group, and a technology probe study to create and refine initial technology designs for children with asthma and their caregivers. Based on the Health Belief Model (HBM), patient-provider communications, and my findings in the prior studies, I designed a mobile and web service to increase asthma knowledge and symptom/management awareness in the child for better health outcomes, and to affect the perceived quality of interaction with healthcare providers. I show the results of my two field deployment studies with total 65 patients to learn how the system affected their practices and health outcomes. My contributions come from an increased understanding in three areas: physician-patient communication via a mobile and web services; ubiquitous communication technology designs to improve current pediatric asthma practices; and controlled evaluation of a ubiquitous communication technology, SMS, in the field.

Quality improvement methodology when implemented can narrow quality gaps. Objective: The objective of the project was to create and implement a plan of action to address identified gaps in key clinical activities of asthma care among pediatric population in a private pediatric setting in Northern California Methods: The project was centered on the use of Education in Quality Improvement for Pediatric Practice (EQIPP), a program of the American Academy of the Pediatrics. Both the pediatrician and the DNP student took this course and employed its methods to improve asthma management. EQIPP supports providers in improving their practice with didactic materials that help participants develop quality improvement project and tools to evaluate the outcomes of that project. Results: Based on the asthmatic patient data analysis the quality improvement team identified that the clinic lacks compliance in the following areas of national guidelines. a) Diagnosis of asthma, b) Asthma action plan and c) Asthma control and follow up. The team then developed and implemented an improvement plan based on EQIPP. Conclusion: The quality improvement project enriched the pediatric practice management of asthma patients and similar projects could be implemented in other settings too. Book excerpt: The National Institutes of Health Publication 08-5846 The Expert Panel Report 3 (EPR—3) Summary Report 2007: Guidelines for the Diagnosis and Management of Asthma was developed by an expert panel commissioned by the National Asthma Education and Prevention Program (NAEPP) Coordinating Committee (CC), coordinated by the National Heart, Lung, and Blood Institute (NHLBI) of the National Institutes of Health. Using the 1997 EPR—2 guidelines and the 2002 update on selected topics as the framework, the expert panel organized the literature review and updated recommendations for managing asthma long term and for managing exacerbations around four essential components of asthma care, namely: assessment and monitoring, patient education, control of factors contributing to asthma severity, and pharmacologic treatment. Subtopics were developed for each of these four broad categories. More than 22 million Americans have asthma, and it is one of the most common chronic diseases of childhood, affecting an estimated 6 million children. The burden of asthma affects the patients, their families, and society in terms of lost work and school, lessened quality of life, and avoidable emergency department (ED) visits, hospitalizations, and deaths. Improved scientific understanding of asthma has led to significant improvements in asthma care, and the National Asthma Education and Prevention Program (NAEPP) has been dedicated to translating these research findings into clinical practice through publication and dissemination of clinical practice guidelines. The first NAEPP guidelines were published in 1991, and updates were made in 1997, 2002, and now with the current report. Important gains have been made in reducing morbidity and mortality rates due to asthma; however, challenges remain. The NAEPP hopes that the “Expert Panel Report 3: Guidelines for the Diagnosis and Management of Asthma—Full Report 2007” (EPR—3: Full Report 2007) will support the efforts of those who already incorporate best practices and will help enlist even greater numbers of primary care clinicians, asthma specialists, health care systems and providers, and communities to join together in making quality asthma care available to all people who have asthma. The goal, simply stated, is to help people with asthma control their asthma so that they can be active all day and sleep well at night.~

An average, one of every ten school-aged children has asthma, and 13 million school days are missed each year due to asthma. Interventions to prevent or reduce the severity of asthma and to improve the quality of life among children and their families include the implementation of clinical practice guidelines, asthma care quality improvement programs, community preventive services, and other community-based approaches. The Healthy Hoops platform was used to evaluate the quality of life of children with asthma. This was first time a Healthy Hoops event was held in Kentucky and the first time quality of life was assessed at a Healthy Hoops event. Children and their parents/guardians attended the Healthy Hoops Kentucky event, in which each child received an asthma action plan. In addition, unique to Healthy Hoops Kentucky event, the quality of life of the child and their parent/guardian was assessed using Juniper's valid and reliable instruments. The thesis had three distinct study aims (1) to examine the quality of life of children with asthma as well as their parents' quality of life via the healthy hoops platform, (2) to investigate the feasibility of using the healthy hoops platform as a mechanism to assess the quality of life of children with asthma and their parents, and (3) to evaluate if the current advocated statistical techniques for analysis are most appropriate. In addition, the thesis develops a study design that would allow us to compare Passport participates who took part in the 2008 event (cases) with Passport members in 2008 who did not participate in the 2008 event (controls) over time; testing for differences in changes in quality of life and level of health care utilization over time. Permission from Passport to obtain this data from non-attendees is still under review. The results of the current study suggest that the Healthy Hoops platform is an efficient way in which to assess the quality of life of children with asthma and their parents and that the children's quality of life is significantly better when compared to their parents, which contradicts earlier studies. Lastly, it is suggested that the current advocated method for analysis is not appropriate.

Barriers to effective asthma management in children include the need for caregivers to identify and manage diverse environmental triggers and promote appropriate use of preventive asthma medications. Although health care providers may introduce asthma treatments and care plans, many providers lack the time and capacity to educate caregivers about asthma in an ongoing, sustained manner. To help address these complexities of asthma care, many providers and caregivers rely on patient navigators (defined as persons who provide patients with a particular set of services and who address barriers to care) (Dohan & Schrag, 2005). Despite growing interest in their value for chronic disease management, researchers and providers know little about how or what benefits patient navigators can provide to caregivers in managing asthma in children. To explore this issue, we conducted a mixed-method evaluation involving focus groups and a survey with caregivers of children with moderate-to-severe asthma who were enrolled in the Merck Childhood Asthma Network Initiative (MCAN). Findings suggest that patient navigators may support children’s asthma management by providing individualized treatment plans and hands-on practice, improving caregivers’ understanding of environmental triggers and their mitigation, and giving clear, accessible instructions for proper medication management. Study results may help to clarify and further develop the role of patient navigators for the effective management of asthma in children.

To the best of our knowledge, no study reviewed the literature on the effect of family asthma education on major asthma outcomes. This study aimed to explore the effect of family education programs on major asthma outcomes in children. Quasi-experimental studies and randomized controlled trials were conducted among children with asthma aged 6,Äì18 years and their parents were included. Pub Med, Science Direct, and Trip databases were used to extract data published in English from 2010 to 2021. Twenty-two studies were reported in this review. It was demonstrated that family empowerment interventions were effective in improving the quality of life of children and their parents, asthma symptom control, and pulmonary function. Family education that was specific to medication improved medication adherence, inhalation technique, and asthma control. Family asthma education enhanced asthma management and family functioning. This approach should be a cornerstone of pediatric asthma therapy. It helps health care professionals to build a strong connection and trustful relationship with children with asthma and their families.
