The three-dimensional Euler and full/thin-layer Reynolds-averaged Navier-Stokes equations for compressible flows are solved on structured hexahedral grids. The Baldwin-Lomax algebraic turbulence model is used for closure. The space discretization is based on a cell-centered finite-volume method augmented by a variety of numerical dissipation models with optional total variation diminishing limiters. The governing equations are integrated in time by an implicit method based on lower-upper factorization and symmetric Gauss-Seidel relaxation. The algorithm is vectorized on diagonal planes of sweep using two-dimensional indices in three dimensions. A new computer program named CENS3D has been developed for viscous turbulent flows with discontinuities. Details of the code are described in Appendix A and Appendix B. With the developments of the numerical algorithm and dissipation model, the simulation of three-dimensional viscous compressible flows has become more efficient and accurate. These factors include, but are not limited to the propulsion system, aerodynamic effects, rotation of the earth, oblateness, and gravity. A trajectory simulation that considers these different factors is generated by a code developed for this thesis titled Trajectories for Heavy-lift Evaluation and Optimization (THEO). THEO is a validated trajectory simulation code with the ability to model numerous launch configurations. THEO also has the capability to provide the means for an optimization objective. Optimization of a launch vehicle can be specified in terms of many different variables. For a heavy lift launch vehicle in this thesis, the goal of optimization is to minimize Gross Lift Off Weight (GLOW). THEO provides the capability to optimize by simulating hundreds of thousands of trajectories for a single configuration through the variation of preset independent variables. The sheer volume of these trajectories provides the means to locate configurations that minimize GLOW. Optimization can also be performed by determining the minimum amount of energy necessary to reach target burnout conditions. The energy requirements are then correlated to the propellant mass which can be used to estimate GLOW. This thesis first discusses the validation of THEO as a simulation program and the properties associated with accurately modeling a trajectory. It then relates how THEO and other developed tools can be utilized to determine a configuration that is optimized to minimize GLOW to orbit for adaptable payload sizes.

In order to survive in a competitive market, space launch providers must design systems with new technologies in shorter development times. This changing nature of space launch vehicle design requires a new way to perform safety analysis. Traditional hazard analysis techniques do not deliver adequate insight early in the design process, when most of the safety-related decisions are made. Early design decisions are often made using "lessons-learned" from previous launch systems, rather than interactive feedback from the new vehicle design actually being developed. Furthermore, traditional techniques use reliability theory as their foundation, resulting in the use of excessive design margin and redundancy as the "default" vehicle design choices. This equivocation of safety and reliability may have made sense for simpler launch vehicles of the past, but most modern space launch vehicle accidents have resulted from incorrect software specifications, component interaction accidents, and other design errors independent of the reliability of individual components. The space launch industry needs safety analysis methods and design processes that identify and correct these hazards early in the vehicle design process, when modifications to correct safety issues are more effective and less costly. This work shows how Systems-Theoretic Process Analysis (STPA) can been used as a powerful tool to identify, mitigate, and possibly eliminate hazards throughout the entire space launch vehicle lifecycle. This work begins by reviewing traditional hazard analysis techniques and the changing nature of launch vehicle accidents. Next, it describes how STPA can be integrated into the space launch vehicle lifecycle to design safer systems. It then demonstrates the safety-guided design of a small-lift launch vehicle using STPA. Finally, this work shows how STPA can be used to satisfy regulatory and range safety requirements. The thesis of this work is that integration of STPA into the design of space launch vehicles can make a significant contribution to reducing launch vehicle accidents.
