Book excerpt: Morse connections favor mechanical stability of implant abutments due to the angulation of the wall studs. Several studies have examined the importance of mechanical stability for the maintenance of prosthetic restorations, in terms of both health of the peri-implant tissues and longevity of the prostheses. However, this mechanical union hinders the decoupling of the abutment-implant system and causes structural deformations during removal for the maintenance of the system, potentially harming the prostheses. This study aims to determine the traction force required to remove prosthetic components coupled to implants with Morse-type connections. Twenty osseointegrated implants with 3.5-mm diameters, 13-mm lengths, and Morse-type connections, and 20 indexed abutments were used in this study. The inner wall angulation for 10 implants was 24u00ba (Group 1) and that for the other 10 was 16u00ba (Group 2). Analysis of abutment roughness was performed before and at the end of the mechanical tests. The abutments of both types were connected to the corresponding implants using a torque of 20 N.cm. Load cycling tests employing a torque of 200 N.cm were carried out in 200,000 cycles to simulate chewing. A traction force was applied with a ratio (R) of 0.1 between the minimum and maximum loads to determine the force required to remove the abutment. During the analysis of the abutment in a profilometer, some imperfections due to the manufacturing process of the components were identified. These were more evident in parts with 24u00ba-angulation. Cycling tests did not establish the need for greater strength to decouple the implant abutments with wall angulations of 16u00ba and 24u00ba. Greater deformations were identified in the 16u00ba-angulation implants after final analysis in the profilometer. This study concludes that Morse-type connections with wall angulations of 24u00ba and 16u00ba favor the decoupling of the abutment-implant system with no distortions.

Bacterial leakage was assessed using 0.3 u00b5L of Escherichia coli suspension inoculated into the abutment screw chamber of the implants, which were then torqued and incubated at 37oC for 14 days.The same samples were than cut lengthwise, and the microgap measured at 3 points on each side using scanning electron microscopy (SEM). Microtomography was used to assess implant platform circularity to validate the SEM findings qualitatively.All implant Systems presented bacterial leakage, with no significant difference between groups. Neodent system presented the highest mean microgap values, followed by Nobel Biocare, Ankylos and Conexu00e3o, with no significant difference between groups.All systems showed conformational errors of circularity on microtomography images.u2028The tapered connection systems evaluated herein were not able to halt bacterial leakage, nor were free from conformational errors.

Book excerpt: The use of dental implants for treating edentulous patients with implant-supported fixed complete denture (ISFCD) has shown to improve the quality of life with great success over a long period of time. While the use titanium bar with acrylic denture teeth attached to the framework with denture acrylic is most common, zirconia is utilized due to its high strength, its color and translucency are close to those of teeth, and high biocompatibility that can reduce the risk of inflammation. However zirconia is 10 times harder than titanium based on the Knoop hardness scale and the long-term damage to the junction of titanium parts and zirconia abutment has not been widely investigated. The purpose of this study was to investigate the surface changes and the screw loosening after cyclic loading in vitro for Zirconia and Titanium and to compare the surface changes and the torque value before and after loading. Total of 24 multiple-unit titanium abutments were used. Twelve straight 3 mm height abutments were used for the two anterior implants. Twelve 30 degree angle correction abutments were used for the posterior two posterior implants. The abutments were placed on the testing model and torqued to 15 Ncm and scanned prior to placement under a digital light microscope and images were acquired for all the abutments to study the surface characteristic and to confirm the absence of any damage to the surface prior to placement. Masticatory forces were simulated using a testing machine (MTS- Machine Testing Systems). Each framework was subjected to a vertical 400 N of load for 500,000 cycles at a frequency of 2 HZ (2 Cycles/second). Abutments were scanned after the loading was complete with the SEM to detect any damages to the surface and none of the abutments showed any signs of damage or wear. The torque values to loosen the screws were analyzed by an analysis of variance (ANOVA) (MIXED procedure, SAS (r) Proprietary Software 9.3, SAS Institute, Inc., Cary, NC, USA) using the maximum likelihood estimation method to eliminate the need for normality and eq. The factors were the material type(Zr or Ti), the screw location (anterior or posterior) and the screw type (prosthetic or abutment screw), with all possible interactions included in the statistical model. Book excerpt: Objectives: The aim of this study was to predict the fatigue life of two different connection of dental implant, assess the probability of failure with different marginal bone levels and compare the mechanical test with a probabilistic approach.Materials and Methods: For the mechanical test 60 conical implants (u00d83.50 mm) and abutments were used, performed 2 groups (n=30 per group): Internal hexagon (group IH) and Morse taper (group MT). Three insertion levels in resin were tested (n=10 per level): 0 mm at the platform level (l1), 3 mm (l2) and 5 mm (l3) above the platform of the resin. All groups were subjected to quasi-static loading at 30 to the implant axis in a universal machine. Six three-dimensional models were created and assembled with the aim of reproducing the conditions used in the laboratory testing. The models were full constrained on the bottom and lateral surface of the bone and the loads obtained experimentally were applied. Fatigue material data and loads were assumed as random variables. Then, probability of failure was obtained by the probabilistic approach.Results: The mean fracture strengths for group IH were: 2092 N (l1), 1041 N (l2) and 898 N (l3); and for group MT: 1687 N (l1), 1644 N (l2) and 1159 N (l3). Finite element analysis results are in accordance with the in vitro mechanical test results. Group MT behavior is better than group IH with bone levels 2 and 3. Probability of failure in group IH has an important dependency of bone level, while, group MT, shows a similar behavior with bone levels 2 and 3.Conclusions: This stochastic model employs a cumulative damage model and probabilistic finite element method and has a good precision on the results.

To also investigate if torquing and retorquing the same implant screw has an influence on the RTV of the screw. Methods: Nine implants with an internal Tri-lobe connection (Nobel Replace Select, Nobel Biocare) were placed in cold-cure acrylic (ProBase Cold, Ivoclar) to simulate bone and a final abutment (Esthetic Abutment NobelReplace RP, Nobel BioCare) was inserted. Nine screw tightening protocols were tested: 1) torque to 35Ncm, 2) hold torque for 10 seconds, 3) hold torque for 20 seconds, 4) retorque at 2 minutes, 5) retorque at 10 minutes, 6) hold torque for 10 seconds and retorque at 2 minutes, 7) hold torque for 10 seconds and retorque at 10 minutes, 8) hold torque for 20 seconds and retorque at 2 minutes, 9) hold torque for 20 seconds and retorque at 10 minutes. Peak RTVs were measured after 30 minutes using a torque meter (MTT03-05, Mark-10). Each implant screw was changed after each insertion with 10 total screws used per implant. After determining the protocol with the highest average RTV, the specific protocol was used to evaluate if cycles of insertion on a single screw would have an impact on the reverse torque values. Five implants were used with the same five abutments. A single screw was torqued with the specified protocol and de-torqued after 30 minutes with the RTV measured. This was conducted another nine times over for a total of 10 cycles. Results: Protocol 2 (holding torque for 10s) had the highest average reverse torque value of 22.63Ncm and the Protocol 1 (control) had the lowest at 20.21Ncm with no statistically significant difference. A statistically significant trend was seen in the RTV over the number of insertion cycles with each insertion cycle showing an increase by 0.252Ncm. Conclusion: Implant screw tightening protocols do not influence the reverse torque values. Multiple insertions of an implant screw may increase the reverse torque value.

Primary stability is the mechanical engagement of an implant with the surrounding bone, whereas bone regeneration and remodeling phenomena create the secondary (biological) stability of the implant. It is of clinical relevance to know the development of implant osseointegration after installation, as well as to detect the incidence of screw loosening after the implantu2019s superstructure is mounted.Objectives: The aim of this study was to examine the progress of implant osseointegration and screw loosening by means of a resonance frequency analysis (RFA) in the rabbit tibia.Methods: One mature New Zealand White rabbit was used. Six titanium implants were installed in the rabbit tibiae bilaterally. A small 3D-accelerometer was mounted rigidly onto the implant abutment, and impulsive vibration was directly applied to the abutment to vibrate the implant in perpendicular to direction of the tibia and implant (x-axis). Two RFA values of x- and y-axis directions (RFx and RFy) were used for evaluation. The y-axis was defined in parallel to the tibia. Also implant stability was measured by implant stability quotient (ISQ) value with Osstellu00ae. These values were measured immediately after implant placement, twice a week for the first month and once a week for the second month. After 8 weeks, RF values and Peiotestu00ae (PT) values were recorded with 13 different screw conditions, i.e. abutment screw tightened with 10N torque (starting point) and the screw was loosened by 5 degrees from 0 to 90 degrees, and the animal was sacrificed.Results: In most implants, ISQ values tend to decrease until 7 to 10 days after installation and then gradually increased. ISQ values at day 7 and 10 were significantly lower than at other time points, except for two implants which located in the distal side of tibiae.
