Book excerpt: "Diet, physical activity, and body weight are modifiable lifestyle factors for which modest improvements have been shown to reduce risk for development of Type 2 diabetes mellitus. However, increasing rates of disease suggest that large segments of the population remain unaware of the impact of lifestyle practices on overall health and quality of life. This pilot feasibility study was designed to implement and evaluate a peer-educator delivered 8-week nutrition and lifestyle education intervention for African American women ages 45 years and over and at risk for Type 2 diabetes their church setting. This 2 x 3 repeated measures study (usual care (UC), intervention (INT); baseline, endpoint, follow-up) used a convenience sample of 41 women (average age = 61 ± 8.92 years) at risk for diabetes from two predominantly African American churches in Greensboro, NC (N = 21, UC; N = 20, INT). Significant changes in fiber intake occurred over time, with no between group differences (INT = 10.9 to 12.5 g; UC = 12.7 to 9.6 g, p = 0.031). Group by time differences occurred for total fat (p = 0.013), monounsaturated fat (p = 0.045), and percent of energy from fat (p = 0.022), with the intervention group significantly decreasing fat intake. When age and total number of chronic conditions were added as covariates, total carbohydrate (grams) differed over time (p = 0.025, age and 0.007, chronic conditions), with INT reporting an increase and UC reporting a decrease. Limiting or restricting high sugar foods and limiting portion sizes significantly differed between groups at endpoint and follow up (p = 0.04). UC received the same education handouts, but not the classes and also reported some significant dietary changes over time, suggesting the benefits of targeted messages for this audience. The peer educator consistently and accurately delivered the curriculum as designed. Attendance rates indicated high exposure to the intervention, and participants were engaged and willing to participate in the program. Further development of more intensive intervention strategies focused on targeted behavior changes to reduce of Type 2 diabetes risk among African American women is needed."--Abstract from author supplied metadata.

World Health Organization (WHO) projects that the morbidity rate will double by 2005-2030 (WHO, 2011). Type 2-diabetes is a permanent condition in which there is high glucose level in the blood and it is the most familiar type of diabetes. It occurs in adults when the body does not produce adequate insulin or the body cells are not making use of the insulin (ADA, 2012). Community education is required to educated individuals and adult with type 2 diabetes on prevention, self- management, treatment, healthy eating, and physical exercises (Chen and Lin, 2010). It is important to encourage individuals with diabetes to learn more about their condition and all treatment required, including the warning signs, and symptoms of the disease (Polzer, 2008). Signs and symptoms of diabetes includes: frequent urination, unusual thirst, extreme hunger, unusual weight loss, extreme fatigue and irritation, infections, blurred vision, cut/bruises that are slow to heal, tingling and numbness in the hand or feet and recurrent skin, gum or bladder infections (ADA, 2012). Since education is found to be very important in effective treatment of diabetes, implementation plan is proposed by Doctors Community Hospital with the surrounding health centers to create awareness to the people on how they can live healthy, eat right, control their weight, participate in daily physical exercises, blood sugar check, and foot check and wound care. They should be taught about the complications of the disease, how to avoid it, and understand the risk involved if not well managed, in conjunction with the stakeholders, and the nursing management, the approval, planning, (Polzer, 2008). Implementation, evaluation and the dissemination of the project results were put in place. After the education, evaluation is conducted to note the outcome of the program. Questionnaires are given out at the end of the program to evaluate the awareness of the participant, the length of teaching, the presentation method, and the overall score of the program. Success of the program is noted by the outcome among the type 2 diabetes result, worth, and appropriateness of the techniques and enablement to achieve result. Long-term assessment of the community is necessary to determine the continuity of the program (Haby, 2008).

According to copious amounts of research studies, it has been found that for the patient to be successful in diabetic self-care management and to achieve within normal limit HgA1c levels frequent diabetic education and follow-up is necessary. In order for nurses to provide competent education to the diabetic patient, the nurse must first self-assess their knowledge and seek out education on diabetes management in areas lacking. The proposed implementation plan will include required education staff meetings and online continuing education credit courses for the specialized diabetic nurse. Establishing and implementing an education plan for nurses and the patient is key in achieving quality patient outcomes. The education implementation plan includes a preassessment of diabetic knowledge of the nurse and patient, education provided to staff nurses, an outlined diabetic teaching plan for patients with monthly follow-ups over a six-month period of time, monitoring of HgA1c levels throughout teachings, and post-assessment of the patient and nurse. A six-month pilot study prior to policy and procedure change will support the need for frequent teachings and follow-up. By providing the patient with current evidence-based education, the diabetic patient can ultimately be more successful in managing their type 2 diabetes.

It is estimated that about 90% of the currently diagnosed adults have type 2 diabetes, and based on the World Health Organisation (WHO) report, about 422 million adults were living with diabetes in 2014 compared with 108 million in 1980; this condition caused about 1.5 million deaths in 2012. In the United States of America, it is estimated that about 30.3 million adults are living with diabetes, with a further 1.5 million new diabetes cases diagnosed every year, representing an increasing prevalence of this condition. Diabetes represents a major public health challenge, despite advances in technology and the pharmaceutical industry. These problems may be in the form of acute or long-term complications. Therefore, in order to attenuate the problems of diabetes, management strategies usually include lifestyle changes such as increased physical activity and dietary interventions. Studies which evaluate the role of nutrition in the management of type 2 diabetes often involve human and animal models as these approaches enable us to have a broader and more in-depth understanding of the condition. In some cases, diabetes may co-exist with other conditions, such as stroke, and these may present unique challenges with regard to nutritional interventions. This Special Issue aims to evaluate the risk factors associated with type 2 diabetes and the role of the diet in the management of people with this condition. Book excerpt: Type 2 Diabetes Mellitus (T2DM) currently affects approximately 1 in 9 individuals in the United States. Access to education, transportation to appointments, and other social determinants of health can reflect poor self-management, adverse outcomes, and increased morbidity and mortality. The purpose of this quality improvement (QI) project was to evaluate the effectiveness of implementing a series of expert-led, educational classes with a toolkit for patients diagnosed with prediabetes or T2DM. The Health Belief Model was the framework used to develop and implement educational intervention through a 4-month period. This project was conducted at two sites in Northeast Kansas: one which provides primary care services to an underserved population, while the other is a satellite clinic located within a housing authority. The PDSA cycle was used to develop and evaluate program implementation throughout the project. A toolkit was developed and provided to individuals who attended the sessions. Educational sessions focused on T2DM overview, risk factors, comorbidities, medication, diet, exercise, stress, and self-management. Expert specialists were recruited for two of the educational sessions. A diabetes self-management questionnaire (DSMQ) score and glycated hemoglobin (A1c) value were obtained prior to the first educational session. A final DSMQ score and A1c value were obtained at the conclusion of the last educational session. Qualitative measurement revealed improved perceived self-management of T2DM as well as an overall improvement of A1c control after a 4-month period.

This DPI project adds to existing literature, identifying a DSME program effective in improving the health outcomes of individuals with Type II Diabetes. The Johns Hopkins Nursing Evidence-Based Practice Model guided identification of the practice question, evidence collection, and translation to practice. Quantitative data illustrates how understanding metabolic differences in simple and complex carbohydrates, effects of gluconeogenesis on blood sugar, and pairing foods in the right amounts, at the right times of the day allows for the remission of symptoms of Type II Diabetes.
