All rights not granted by the above license are retained by the author. Abstract: Coronary artery disease (CAD) is known as the second killer in Hong Kong. The Hong Kong Hospital Authority reported nearly 7000 patients suffered from acute myocardial infarction (AMI) in year 2010 to 2011. Percutaneous transluminal coronary angioplasty (PTCA) is a minimal access surgical treatment for coronary artery disease but studies have shown that patients experienced different levels of anxiety before and during PTCA which led negative impact to the patients. Music can be regarded as a safe, cost-effective therapy to reduce one's anxiety level. It can be carried out by nurses without any specific technique. However, there was no systemic review for using music therapy to patients undergoing PTCA. Therefore, this dissertation aims to evaluate the best available evidence on using music therapy for patients undergoing PTCA. Four electronic databases, Medline, CINAHL, Embase & PsycINFO, were searched for studies to investigate the efficacy of music therapy in patients undergoing PTCA. There were seven studies were eligible with data extracted and quality assessment performed by the critical appraisal skill programme (CASP) checklist. Four studies were graded as high quality, which consistently demonstrated a statistically significant more reduction in the anxiety level of patients who received music therapy either before and/or during PTCA than those who had no music therapy. Consequently, an evidence-based guideline of using music therapy for patients undergoing PTCA was developed according to the guideline development process of SIGN (2010). The use of music therapy is transferable and feasibly in the local setting with little manpower concern. The total cost estimated for running the innovation for a year was HKD 83,775 which was considered as cost-effective to reduce patients' anxiety level and avoid associated adverse events. A 14-months programme including communication with the stakeholders, pilot testing, staff training and clinical application of the proposed music therapy was designed. Qualitative and quantitative data on patient, healthcare provider and system outcomes would be accounted throughout the programme. The effectiveness of the guideline would be determined by the reduction of patient's anxiety level by using the Chinese version of State-Trait Anxiety Inventory (STAI) (Shek, 1993). Moreover, healthcare provider outcome would be evaluated by questionnaire and the expenditure of the proposed music therapy would be monitored closely. The study was conducted in one area hospital using the preoperative holding area in a general service surgical suite. Two groups of 20 patients each were given two questionnaires using a pre-test-post-test method. The instruments used were the Anxiety Status Inventory and the Self-Rating Anxiety Scale. The hypothesis that patients who are exposed to classical music in a preoperative holding area will experience less anxiety than patients who are not exposed to music in a preoperative holding area was not supported by this study were not statistically significant.

Forty-eight men and women admitted to cardiac progressive care unit of a local regional medical center served as subjects for the study. Subjects were assigned to one of three groups. Sixteen subjects received relaxation and imagery paired with live music, sixteen subjects received literature, and sixteen subjects received no intervention. Pre- and Post intervention data included pain medication intake and a self-report questionnaire concerning pain, anxiety, and present and future psychosocial perceptions. Patients in the music group were given a relaxation tape, which involved relaxation exercises and imagery, paired with music. Patients were called one week after discharge and given the questionnaire once more. Patients were also asked about their pain medication intake. Results from the study were collected and analyzed. The results revealed the music group scored significantly lower on the pre posttest questionnaire involving pain, anxiety and present and future perceptions. A difference was found in the groups' pre/post medication intake but results were not significant. The analysis found that the music group had significantly lower scores on the questionnaire one-week post discharge. The music group also had a significantly lower level of pain medication one-week post discharge. It was concluded that music therapy interventions are beneficial to subjects in a post-operative setting.

Surgical Fear Questionnaire (SFQ) and Patient Satisfaction Questionnaire (PSQ) were used to assess anxiety before and after music session and overall patient satisfaction postoperatively. The proportion of patients receiving midazolam during surgery was also recorded. Comparisons were made using Chi-Square, or ANOVA, where appropriate. Book excerpt: The purpose of this randomized controlled trial was to determine the effects of pre-transplant music therapy on distress, pain, anxiety, mood, quality of life (QOL), and pain medication use during the preparation period for hematopoietic stem cell transplant (HSCT). Distress, the primary outcome, was measured via the Distress Thermometer. QOL was assessed with the Functional Assessment of Cancer Therapy - General 7 (FACT-G7). Participants self-rated pain and anxiety on Numeric Rating Scales (NRS) and mood on the Rogers Happy/Sad Faces Scale. Participants randomized to the experimental group received three music therapy sessions prior to stem cell infusion day. Repeated measures analysis of variance (RMANOVA) was employed to detect differences between groups for distress and QOL, and t-tests were used to detect differences between groups for pain, anxiety, and mood. Data collection is ongoing and will conclude when 50 patients have been accrued. This report presents interim data analysis, with complete data available for 23 participants. The experimental group had higher distress that was statistically significant at baseline and on Day -1. Music therapy did not affect distress but showed other limited beneficial effects. There were no differences between groups at any individual time point for pain, anxiety, or mood. At all three time points, decreases in anxiety from pre-session to post-session assessment were statistically significant in the experimental group. In the first and third music therapy session, differences between pre- and post-session mood scores were statistically significant in a positive direction. Differences in pain medication use could not be calculated because too few participants required pain medication. Although music therapy did not affect the primary outcome of distress, the positive short-term effects on anxiety and limited positive effects on mood are important to acknowledge. The small sample size likely contributed to the lack of findings on distress, although the longitudinal assessment of this outcome may have also been a factor. This study provides an initial understanding of how music therapy may be effective on distress and other variables during the pre-transplant hospitalization period for patients undergoing allogeneic HSCT, as it is the first research study to examine this treatment phase. There are few music therapy research studies with patients undergoing HSCT, which leaves many options for future research. It would be worthwhile to explore any short-term effects of music therapy on distress and QOL, as well as longitudinal effects on anxiety and mood. Optimal music therapy session duration and frequency during the pre-transplant hospitalization period remains unknown. Research opportunities exist during other phases of HSCT treatment, such as the out-patient pre-transplant period, duration of hospitalization, or post-transplant follow up. Understanding the effects of music therapy for patients suffering from Graft-Versus-Host Disease could also be beneficial as this is often debilitating and can be life-threatening. Finally, there is a need for qualitative music therapy studies with this patient population, as none currently exist but could provide additional insight for future research studies as well as clinical practice.

Book excerpt: ABSTRACT: Twenty-three patients undergoing outpatient chemotherapy at Tallahassee Memorial HealthCare, a regional medical center, participated in this study. After informed consent was obtained, participants supplied demographic data and completed measures for depression, self-efficacy, and tension/anxiety. Heart rate was taken before and after the music and imagery session. Experimental participants chose one of three possible relaxation imagery scripts. The session began with a Progressive Muscle Relaxation and then transitioned into the chosen imagery script. Both sections were read softly by the music therapist and accompanied by live music played softly on a nylon-string guitar. After the session, heart rate was obtained and participants filled out a post-test tension/anxiety measure. A Mann-Whitney U test revealed that experimental participants experienced a significant decrease in tension/anxiety from pre-test to post-test (U= 32.5, c.u.= 33, p=.05). No significant differences were found for heart rate between groups (pre-test HR: U= 57.5, c.u.= 26, p= .05; post-test HR: U=40, c.u.= 26, p= .05). The results confirm that music therapy is effective for anxiety reduction with patients undergoing outpatient chemotherapy.
