Book excerpt: For centuries men speculated about the process of gastric digestion, but Iate in the eighteenth and early in the nineteenth centuries physiologists, both physicians and laymen, began to accumulate experimental evidence about its nature. At the same time, others discovered that the stomach is capable of secreting a strong mineral acid, and the questions of how that secretion is produced and how it is controlled became enduring problems. A Iittle later, the discovery that an acid extract of dead gastric mucosa is capable of digesting meat put the study of gastric secretion and digestion on a firm mechanistic foundation. From that time to the present, physi ologists have assiduously investigated gastric secretion and digestion, with the result that knowledge ofthose topics is as comprehensive and penetrating as isthat about other physiological processes. In addition, that knowledge is the basis of discrimi nating and effective clinical practice. I have described the experimental study of gastric secretion and digestion for two reasons. The firstisthat the successes and some ofthe failures ofphysiologists over two centuries are important parts of intellectual history that deserve to be recorded. The second is that some of those who use the accumulated knowledge every day are curious about its genesis. I assume that my readers have the technical knowledge to understand what I have written. If my account does not fully satisfy their curiosity, I have provided references that will open the path to further study.

This phenomenal gradient as well as the demonstrated ability of gastric juice to digest tissues has motivated clinicians and investigators alike to emphasize acid secretion and acid ablation in studying the pathogenesis and therapy of peptic ulcer disease. Conse quently, over the past 150 years, we have made considerable progress in under standing the mechanisms and regulation of acid secretion by the stomach. Not surprisingly, therapy for both peptic disease and mucosal injury has also been predominantly directed at either neutralizing acid or suppressing its production. During the past 10 years, attention has been focused on factors other than acid in the genesis and therapy of ulcer disease. Work done worldwide demon strated that acid hypersecretion is not a common event in peptic ulcer disease. Therefore, we began realizing that factors other than acid secretion may be important in the genesis of ulcer disease or in gastroduodenal mucosal damage. In addition, new physiological information has established that the gas troduodenal mucosa is normally protected by a complex series of events includ ing mucus and bicarbonate secretion, cell renewal, surface mucosal restitution, and preservation of the microvasculature and mucosal proliferative zone.

In clinical medicine, Marshall and Warren's assertion that Helicobacter pylori is the main cause of gastritis, peptic ulcer disease, and gastric cancer made an immense impact. However, although peptic ulcers now can be treated with antibiotics, drugs that inhibit gastric acid secretion, including proton pump inhibitors (PPIs), are used in up to 10% of the adult population in Western countries in the treatment of gastro-oesophageal reflux disease and dyspepsia. For a long time, PPIs were the most frequently sold drugs, which made their economic impact huge. The reduction of gastric acidity induces the release of the gastric hormone gastrin, which stimulates its target, the ECL cell, to release histamine; this subsequently augments the acid secretion from the parietal cell. Gastrin also stimulates ECL cell proliferation, and long-term hyperstimulation leads to the development of tumors. The author of this book began his studies into the role of the ECL cell in human gastric carcinomas by demonstrating ECL cell markers in gastric cancer cells. This incriminated the ECL and its main regulator, gastrin, in gastric carcinogenesis. Every condition with reduced gastric acidity including treatment with drugs inhibiting acid secretion, leads to increased gastrin and thus to a predisposition to gastric cancer. This short book provides ample evidence of the pharmaceutical industry's influence on medical science. It also highlights the lack of respect and attention that the medical world gives to a biological function like gastric acidity, which prevents microbes, including viruses and prions, from entering the body through the gut.
