A questionnaire was used to determine the physicians' perceptions regarding NP practice. The sample population was 33 primary care physicians in Kentucky. Sixty-seven percent of the primary care physicians sampled indicated moderate support for the NP role.

Physician support of the utilization of NPs is of vital importance to the future of the NP profession. The purpose of this study was to evaluate primary care physicians' support of NP utilization. In order to attain the goal of excellent patient care it must be provided in a multifarious fashion, which can be established by MD and NP collaboration; hence, Imogene Kings' theory of goal attainment was the theoretical framework for this study. In this quantitative descriptive study 120 Northern Kentucky primary care MDs were randomly selected and surveyed in order to evaluate their support of NP utilization. The results indicated that Northern Kentucky primary care physicians have moderate to strong support for nurse practitioner utilization, and that physicians who practice with nurse practitioners have higher NP utilization support than those physicians who do not. The results of this study provide some insight into primary care physicians' perceptions of NP utilization in the health care realm.

Nurse practitioners have a strong record of accomplishment of providing both high quality primary care and access to care for underserved populations including older adults. A review of the literature reveals that there are many studies looking at comparisons between NPs and physicians related to clinical outcomes, and patient satisfaction. However, not many studies evaluate what people understand the role of NP to be. This non-experimental descriptive study looked at the perceptions of the role of nurse practitioners among older adults ages 62 and beyond. Whether elders had received health care from a NP in the past, either personally or a family member, positively influenced the subsequent health care choice of the elder for a NP.

Available in PDF, EPUB and Kindle. Book excerpt: Consumer acceptance of the nurse practitioner (NP) role has been well documented in studies throughout its thirty year history. However, acceptance has been primarily contingent on the consumer having prior knowledge of or experience with NPs with little attention directed toward the attitudes the general public. The purpose of this study was to assess public awareness of and support for the NP role. This study was carried out by means of a cross-sectional descriptive design. Using a convenience sample, a demographic data sheet and the Nurse Practitioner Perception Scale (NPPS) were administered to all employees of Northern Kentucky University (N=1010) with a response rate of 31% (N=315). The demographic distribution was predominantly college educated caucasian females with an average age of 40-50. NPPS scores revealed solid overall support for NPs in all categories: role function, role support, and legislative issues. The only demographic variable significantly associated with support for NPs was education (college graduates). In general, traditional nursing functions received the greatest support whereas traditional medical functions generated more hesitant support. Independent NP practice was poorly supported, while collaborative practice was well supported.

Available in PDF, EPUB and Kindle. Book excerpt: The nurse practitioner is a relatively new member of the health care team in Manitoba. A collection of agencies have worked both independently and mutually since the mid 1990s to formally implement the nurse practitioner role. Manitoba Health, Winnipeg Regional Health Authority, Manitoba Association of Registered Nurses, College of Physicians and Surgeons and University of Manitoba Faculty of Nursing have all been involved. While it is envisioned that the nurse practitioner role will mesh with the physician role in a collaborative relationship, a paucity of literature exists that examines physicians' perceptions of nurse practitioners and collaboration with nurse practitioners. This qualitative project explored physicians' knowledge of nurse practitioners and perceptions of collaboration with nurse practitioners. Focus group discussions with eleven physicians (8 resident physicians and 3 faculty physicians) were conducted to develop an understanding of their knowledge and perceptions. Both resident and faculty physicians had specific, yet largely contrasting concerns about the nurse practitioner role, which subsequently influenced their perceptions of collaboration with nurse practitioners. Resident physicians tended to accept the nurse practitioner role but were restrictive in terms of responsibilities and capabilities. Faculty physicians tended to view the nurse practitioner role and collaboration without limitations but very much dependent on the individual nurse practitioner and his/her background, personality and ability. It is hoped that the findings of this project will contribute to the body of knowledge about physician and nurse practitioner collaboration. Additionally, themes emanating from this project may guide future discussions between the disciplines with respect to nurse practitioners and collaboration with nurse practitioners.

Book excerpt: Dissatisfaction amongst US physicians has been steadily increasing over the past few decades as health care reform changes the practice of medicine (Hoff, Young, Xiang, & Raver, 2015; Rosenstein, & Mudge-Riley, 2010). In 2008, physician dissatisfaction rates had increased to 19% (Mazaurenko & Menachemi, 2012). By 2012, 42% of physicians voiced dissatisfaction, with family medicine reporting most likely to be dissatisfied (Sorrell & Jennings, 2014). One factor that could affect physician career dissatisfaction/satisfaction that has not been studied is collaborative relationships with other healthcare providers, such as nurse practitioners. This study used secondary quantitative data from the National Survey of Primary Care Physicians and Nurse Practitioners (2012). Logistic regression was used to analyze the relationship of nurse practitioner collaboration and the other independent variables with the dependent variable of physician career satisfaction. In order to address the research goals the independent variables of primary interest were as follows 1) whether PCPs work with NPs in their office; 2) what the quality of the PCP relationship is with the NPs; 3) what is the share of work performed by NPs. Descriptive statistics and binary logistic regressions were run to test the significance of the three hypotheses. Analysis yielded many observational results on the PCPs descriptively but did not show any significant results on the proposed hypotheses regarding PCP career satisfaction. However, one of the greatest strengths of this study was the attempt to bridge the gaps of knowledge regarding PCP and NP collaboration and physician career satisfaction. The fact that results were not significant does not negate the need for further studies on this issue, especially since no other studies exist. In conducting future research, obtaining a larger sample of PCPs, including PCPs who work with NPs, would be an important step in correcting some of the limitations of this study. The lack of significant results also raises further questions regarding the difference between career and job satisfaction with the latter referring to the current work environment rather than the cumulative career experience.

Interprofessional collaboration in the provision of primary health care has been widely espoused in the health care literature as a means of achieving the goals of primary health care reform. Primary health care nurse practitioners in collaboration with physicians and other allied health professionals have a fundamental role in enhancing primary health care in Ontario. The purpose of this study was to explore and describe nurse practitioners' experiences and perceptions of interprofessional collaboration with physicians in the provision of primary health care in Ontario. A qualitative descriptive study design was used and six nurse practitioners working in a variety of primary health care settings in Ontario were purposively sampled and interviewed regarding their experiences and perceptions of interprofessional collaboration with partnering physicians. Interviews were analyzed using qualitative content analysis techniques and themes were identified. Seven themes were identified as key factors influencing collaboration within the nurse practitioner - physician dyad from the nurse practitioner's perspective. These themes included: quality of communication, complementary vision, physician remuneration methods, establishing and maintaining relationships, investing time and energy, nurse practitioner competency and expertise and mutual trust and respect. A model of nurse practitioner - physician interprofessional collaboration is used to organize the themes identified. The findings of this study support current recommendations for joint education initiatives for health care professionals and practice initiatives aimed at improving collaboration between partnering nurse practitioners and physicians. Areas for future research include incorporating the client's experiences and perceptions within collaborative practice as well as developing and evaluating interventions that strengthen collaboration within the health care team.

Book excerpt: Ministry of Health in Bahrain is facing a number of challenges including the escalating cost of health services, increasing the incidence of chronic diseases, and shortage of primary care physicians (PCP). Introducing Nurse Practitioner (NP) specialty in the primary care setting is one way to meet these challenges. The purpose of this quasi-experimental study was to examine the knowledge and attitudes of primary care physicians (PCP) about the NP role prior to and following an educational intervention. The modified Northern Emergency Nurse Practitioner Staff Survey was used to measure the PCP' attitudes and knowledge. A total of 90 PCP completed the pre-test and post-test. The study findings showed that PCP had poor understanding of the NP role. Their knowledge and attitudes changed significantly after the educational intervention. Post-test data indicated a positive correlation between knowledge and attitudes toward the NP role. More educational preparation and orientation to NP role for PCP is needed before the implementation of the role in primary care setting. There is a need for the role to be clearly defined prior to implementation to help gain understanding and acceptance.
