Book excerpt: Demineralization of enamel has been one of the iatrogenic effects of fixed orthodontic therapy, for which various authors have suggested the use of fluoride to reduce the decalcification and development of white spot lesion. In the present study, the shear bond strength of light-cure composite adhesive, self-cure composite adhesive, light-cure glass ionomer cement and acidic primer were tested at the end of 24 hours after bonding. The results proved that the acidic primer system had highest shear bond strength while Light-cure glass ionomer system had the least shear bond strength. But the mean values met the minimal shear bond strength required for orthodontic bonding. After bond failure, least amount of glass ionomer cement was present on the enamel surface, followed by acidic primer system, light-cure composite and self-cure composite system. So, it is advantageous to use light-cure glass ionomer cement for bonding in terms of less amount of time spent and no enamel damage during cleaning procedure after debonding.

Rely-a-Bond showed significant increase in Shear bond strength from 1 hour to 24 hours. Smart Bond shows significant decrease in Shear bond strength over time. Rely-a-Bond has significantly increased Shear bond strength than Smart Bond at all times. Rely-a-Bond has high probability of failure at 1 hour; however show drastic improvement at 24 hours. Hence usage of Rely-a-Bond, force application should be deferred for 24 hours. Smart Bond adhesive is unsuitable for use as a bonding agent in routine orthodontic practice. However, the ability of this adhesive to adhere to wet surfaces could enable practitioners to bond attachments in wet conditions.

Book excerpt: The purpose of this study was to compare the bond strengths of three adhesive systems (two-paste, no-mix, and one light-activated) when used alone and combined with a porcelain priming agent to bond ceramic brackets to porcelain surfaces. Sixty porcelain specimens were randomly assigned to the six different treatment groups. One half of the groups were bonded with the porcelain priming agent and one of the adhesive systems and the other half with one of the adhesive systems without the priming agent. The shear bond strengths, expressed in megapascals (MPa), of all specimens were tested, using an Instron testing machine, 10 minutes after being bonded. The surface of the porcelain and the bracket base were examined, using the scanning electron microscope (SEM), to determine the bond failure patterns and check the prcelain surface for the presence of cracks and fractures. Across the three different adhesive systems, mean shear bond strength measurements ranged from 6.3-7.5 MPa when the porcelain primer was used and from 1.8-3.8 when the porcelain primer was not used. There was a statistically significant difference within each adhesive between those samples with the priming agent (mean shear bond strength of 6.5 ± 0.8, 7.5 ± 1.0, and 6.3 ± 1.2) and those without the priming agent (mean shear bond strength of 3.8 ± 0.4, 1.8 ± 1.8, nad 2.6 ± 1.3) (p ≤ 0.5). Differences between the three adhesives were not statistically significant (p ≤ .05). Evaluation of the bond failure sites, using the SEM, revealed failures at the porcelain-adhesive interface, within the adhesive and at the bracket-adhseive interface for the porcelain primer samples. The samples that were bonded with the different adhesives only displayed a failure exclusively at the porcelain-adhesive interface. The failure patterns were verified by qualitative energy dispersive x-ray analysis (EDS). It can be concluded from this study that using any of the three adhesive systems combined with the porcelain priming agent will provide sufficient bond strength to withstand normal orthodontic forces and not damage the surface of the porcelain when debonded.

Esthet-X Flow has slightly lower mean bond strength value (10.70+/-2.0 MPa) compared Filtek Z-350 and Transbond XT. All the three adhesives exhibited clinically acceptable bond strength. 60-70% of all the three adhesives exhibited cohesive type of bond failure. The efficacy of the flowable composites as a bonding agent needs invivo and clinical assessment through a survival analysis. Filtek Z-350 and Esthet-X flow exhibited problems with handling properties and flowability. Drifting of brackets was noted and pressure was needed for positioning the bracket with Filtek Z-350 and Esthet-X flow. The flow property and viscosity should be balanced to obtain optimal consistency to achieve optimal bond strength and improve handling property for Filtek Z-350 and Esthet-X flow.
