Book excerpt: Beautifully illustrated and comprehensive, this must-have atlas guides readers through the delicate, high-risk surgical approaches they need to successfully manage pathology of the temporal bone, the most complex anatomic area in the human body. Each consistently organized chapter addresses a different technique, beginning with a clear definition of the approach followed by a discussion of indications and key information on anatomic orientation, surgical steps, and, in many cases, surgical anatomy. Clearly labeled, full-color illustrations accompany step-by-step descriptions of common and unique operative techniques. The author, a world-renowned otorhinolaryngologist, provides detailed explanations of important anatomic landmarks and advice on how to choose the appropriate instruments at each surgical stage.Features: 328 full-color photographs and diagrams orient the reader to the clinical setting Helpful pointers appear throughout the text -- such as the precise usage of the burr -- to help readers hone their surgical skills Definitions and tips are incorporated as footnotes in each chapter to aid quick and easy reference Thought-provoking section presents surgical techniques, instruments, and prostheses that were developed and designed by the author Extensive glossary contains the most frequently used terms in otology and neurotology An ideal companion for use in both the dissection laboratory and the operating room, Atlas of Temporal Bone Surgery is an indispensable one-stop reference for otologists, neurotologists, and any specialist involved in skull base surgery or otolaryngology.

New microdissection approaches have been developed, most importantly anterior microdissection. In addition to the tympanic cavity, this approach allows a direct view into the anterior membrane of Prussak's space, the anterior surface of the tensor fold, and the supratubal recess, areas not explored earlier. The regular and hitherto unknown auxiliary aeration and drainage pathways are shown in detail. Aeration of Prussak's space is documented using both microdissection and serial sections; these pathways from the mesotympanum or from the lower lateral attic are independent of the tympanic isthmus. The pathology caused by chronic inflammation on the aeration and drainage routes is also documented, and new surgical microdissection methods for removal of the tensor fold are described. The atlas is invaluable in the temporal bone laboratory for all residents learning anatomy and pathology of the middle ear compartments, and for the experienced otologist the photographic documentation gives reliable evidence of the variable structures in the epitympanic compartments.

Available in PDF, EPUB and Kindle. Book excerpt: Imaging of the temporal bone has recently been advanced with multidetector CT and high-field MR imaging to the point where radiologists and clinicians must familiarize themselves with anatomy that was previously not resolvable on older generation scanners. Most anatomic reference texts rely on photomicrographs of gross temporal bone dissections and low-power microtomed histological sections to identify clinically relevant anatomy. By contrast, this unique temporal bone atlas uses state of the art imaging technology to display middle and inner ear anatomy in multiplanar two- and three-dimensional formats. In addition to in vivo imaging with standard multidetector CT and 3-T MR, the authors have employed CT and MR microscopy techniques to image temporal bone specimens ex vivo, providing anatomic detail not yet attainable in a clinical imaging practice. Also included is a CD that allows the user to scroll through the CT and MR microscopy datasets in three orthogonal planes of section.

Book excerpt: A very good knowledge of anatomical sections is necessary to be able to interpret fine imaging of the temporal bone. Both serial histological sections and CT material are presented in this volume. Three planes of space - horizontal, frontal and sagittal - are presented. The very thin histological slices make it possible to identify and study the vascular and nervous structures, section by section in and around the temporal bone. This very fine reading of the anatomical slices is an ideal aid for the clinician who must interpret normal and pathological CT sections.

Book excerpt: Temporal Bone Histology and Radiology Atlas provides a user-friendly approach to understanding both microscopic and radiographic anatomy of the temporal bone. It examines horizontal and vertical histologic sections and correlates them to the more commonly seen radiographic images, primarily on CT and also on MR. This enables the reader to "see" (by visualizing) much more when they look at radiographs than they otherwise would. This text is easy to use and can be referred to in detail as well as briefly and frequently in the course of otolaryngology or radiology practice, and can be digested comfortably for maintenance of certification (MOC) and Boards preparation. Key Topics: * Anatomical relationships * Fetal and postnatal development * Concerns doctors should have regarding radiographic images * Special preparation techniques for electron microscopy and DNA extraction Special histology techniques Temporal Bone Histology and Radiology Atlas is designed for otolaryngologists and radiologists in all phases of their careers, from medical school to residency and fellowship training to Boards to MOC and in ongoing practice. Neuro-otologists and neuroradiologists will benefit from this centralized compilation of information as well.

This volume is a very important contribu tion to the student who is learning these surgical approaches. Surgical otoneurology has now passed the infancy stage, but is still an adolescent. As more otologists and neurosurgeons become skilled in this type of surgery, new and better approaches will evolve. Certainly there needs to be much better management of the carotid artery as it passes through the temporal bone. Better techniques to preserve the IX, X, and XI nerves in the jugular bulb area should be developed, and more delicate procedures for management of lesions inside the cochlea and vestibular labyrinth should be developed. As our diagnostic techniques have improved, particularly through imaging, surgical techniques to match the improved diagnostic techniques will emerge. For future otoneurologists who are pre pared, many problems involving the temporal bone that are now considered untreatable will be successfully managed for very grateful patients. The purpose of this text is to familiarize the otoneurosur geon with the anatomy of the temporal bone, skull base, infratem poral fossa, and cerebellopontine angle. This anatomy will be taught by demonstrating surgical procedures. This atlas which is an example of cooperation between the schools of Los Angeles and Verona will permit the reader to rehearse otoneurosurgical procedures in the laboratory, and, when the techniques have been mastered, apply the various approaches in the treatment of inner ear and skull base lesions. William F. House MD.
