Book excerpt: Atherosclerosis manifests as plaques within large arteries of the body and remains as a leading cause of mortality and morbidity in the world. Major cardiovascular events may occur in patients without known preexisting symptoms, thus it is important to monitor progression and regression of the plaque burden in the arteries for evaluating patient's response to therapy. In this dissertation, our main focus is quantification of plaque burden from the carotid and femoral arteries, which are major sites for plaque formation, and are straight forward to image noninvasively due to their superficial location. Recently, 3D measurements of plaque burden have shown to be more sensitive to the changes of plaque burden than one-/two-dimensional measurements. However, despite the advancements of 3D noninvasive imaging technology with rapid acquisition capabilities, and the high sensitivity of the 3D plaque measurements of plaque burden, they are still not widely used due to the inordinate amount of time and effort required to delineate artery walls plus plaque boundaries to obtain 3D measurements from the images. Therefore, the objective of this dissertation is developing novel semi-automated segmentation methods to alleviate measurement burden from the observer for segmentation of the outer wall and lumen boundaries from: (1) 3D carotid ultrasound (US) images, (2) 3D carotid black-blood magnetic resonance (MR) images, and (3) 3D femoral black-blood MR images. Segmentation of the carotid lumen and outer wall from 3DUS images is a challenging task due to low image contrast, for which no method has been previously reported. Initially, we developed a 2D slice-wise segmentation algorithm based on the level set method, which was then extended to 3D. The 3D algorithm required fewer user interactions than manual delineation and the 2D method. The algorithm reduced user time by H"9% (1.72 vs. 8.3 min) compared to manual segmentation for generating 3D-based measurements with high accuracy (Dice similarity coefficient (DSC)>90%). Secondly, we developed a novel 3D multi-region segmentation algorithm, which simultaneously delineates both the carotid lumen and outer wall surfaces from MR images by evolving two coupled surfaces using a convex max-flow-based technique. The algorithm required user interaction only on a single transverse slice of the 3D image for generating 3D surfaces of the lumen and outer wall. The algorithm was parallelized using graphics processing units (GPU) to increase computational speed, thus reducing user time by 93% (0.78 vs. 12 min) compared to manual segmentation. Book excerpt: As the single most important contributor to cardiovascular disease-related deaths, atherosclerosis serves as a significant topic for scientific research, particularly in terms of diagnosis and prevention. Of particular interest is the atheromatous plaque - an accumulation of harmful fatty materials embedded within the intimal layer of the arterial wall. However, due to the asymptomatic nature of atherosclerotic disease, detection and characterization of atherosclerosis at its earliest developmental stages remain difficult endeavors. Although several arterial imaging techniques have been developed to visualize and characterize various features of the disease, these methods either suffer from limitations in resolution or provide an inaccurate assessment of physiological conditions. These limitations therefore necessitate a high-resolution, label-free imaging technique for atherosclerotic disease detection. Nonlinear optical microscopy has emerged as a viable label-free means for imaging thick biological tissue, at submicron resolution and with highly specific chemical identification. Based on the nonlinear optical response of certain materials to light, nonlinear optical microscopy employs the use of long-wavelength photons in the near-infrared region of the electromagnetic spectrum to provide contrast and optical sectioning capabilities in highly scattering tissue. Recent studies have applied nonlinear microscopic techniques to the study of atherosclerosis, particularly with regards to characterizing structural features of the arterial wall and the protective plaque cap, as well as identifying cells and lipids within the plaque core. This dissertation investigates the use of three nonlinear optical processes - two-photon excited fluorescence, second harmonic generation, and coherent anti-Stokes Raman scattering - to identify and characterize structural and compositional features of the atherosclerotic plaque. In this work, a multimodal imaging platform capturing all three nonlinear signals is implemented to study cellular and structural changes in animal models of disease. Specifically, changes in the accumulation of lipids, macrophage cells, and structural collagen based on dietary conditions are analyzed in an atherosclerotic mouse model. Lipid morphology, composition, and crystal formation are also characterized. Finally, collagen structure and reorganization in an injury-based, rabbit model of disease is investigated. These findings reveal several important features of the atherosclerotic plaque and further establish nonlinear optical microscopy as a powerful tool for biological tissue imaging.

Magnetic resonance imaging (MRI) is a promising diagnostic tool for evaluating atherosclerosis because of its excellent tissue contrast non-invasiveness. However many vascular MRI techniques have not been utilized clinically due to practical issues related to imaging speed, workflow, contrast media, and image quality. The primary focus of the work in this dissertation is to address some of these limitations and improve the capability of vascular MRI to accurately detect and characterize atherosclerotic lesions. Specifically, there are four areas of improvements of vascular MRI: First, an online respiratory self-gating method (ADIOS) was developed for noncontrast magnetic resonance angiography of renal arteries. Some drawbacks of the conventional navigator gating method, including complicated planning and saturation artifacts were eliminated. Testing on healthy subjects confirmed that the proposed method was able to provide high-quality visualization of renal arteries with no navigator-induced artifacts, simplified setup, and shorter scan time. Second, a diffusion-prepared turbo-spin-echo technique (DP-TSE) was developed to enable 3D high resolution diffusion weighted imaging for carotid plaque characterization without the need of contrast agent. The proposed method showed significantly higher vessel wall visibility, less distortion, and less partial volume effect. Apparent diffusion coefficient demonstrated the potential to differentiate lipid core from fibrous plaque tissue. Third, a black-blood imaging technique (DANTE-SPACE) was developed to improve flow signal suppression in vessel wall imaging. A combined carotid and intracranial arterial wall imaging protocol was designed and tested. DANTE-SPACE demonstrated significantly improved arterial and venous blood suppression compared with conventional methods. Preliminary clinical study showed that the proposed method had the potential to detect vessel wall dissection and accurately differentiate plaque components. Fourth, an imaging method for coronary plaque characterization was developed to allow simultaneous dark-blood T1-weighted imaging and bright-blood imaging. An image-based affine motion correction algorithm was designed to allow 100% respiratory gating efficiency. Results from healthy subjects demonstrated the feasibility of whole-heart coronary plaque imaging with isotropic high resolution. Preliminary results from patients with coronary artery disease showed the potential of this technique to detect intra-plaque hemorrhage and inflammation in the coronary wall.
