Book excerpt: Water, sanitation and hygiene (WASH) investments are widely seen as essential for improving health in early childhood. However, the experimental literature on WASH interventions identifies inconsistent impacts on child health outcomes, with relatively robust impacts on diarrhea and other symptoms of infection, but weak and varying impacts on child nutrition. In contrast, observational research exploiting cross-sectional variation in water and sanitation access is much more sanguine, finding strong associations with diarrhea prevalence, mortality and stunting. In practice, both literatures suffer from significant methodological limitations. Experimental WASH evaluations are often subject to poor compliance, rural bias, and short duration of exposure, while cross-sectional observational evidence may be highly vulnerable to omitted variables bias. To overcome some of the limitations of both literatures, we construct a panel of 442 subnational regions in 59 countries with multiple Demographic Health Surveys. This large subnational panel is used to implement difference-in-difference regressions that allow us to examine whether longer term changes in water and sanitation at the subnational level predict improvements in child morbidity, mortality and nutrition. We find results that are partially consistent with both literatures. Improved water access is statistically insignificantly associated with most outcomes, although water piped into the dwelling predicts reductions in child stunting. Improvements in sanitation predict large reductions in diarrhea prevalence and child mortality, but are not associated with changes in stunting or wasting. We estimate that sanitation improvements can account for just under 10% of the decline in child mortality from 1990-2015.

Book excerpt: This paper combines 172 Demography and Health Survey data sets from 70 countries to estimate the effect of water and sanitation on child mortality and morbidity. The results show a robust association between access to water and sanitation technologies and both child morbidity and child mortality. The point estimates imply, depending on the technology level and the sub-region chosen, that water and sanitation infrastructure lowers the odds of children to suffering from diarrhea by 7-17 percent, and reduces the mortality risk for children under the age of five by about 5-20 percent. The effects seem largest for modern sanitation technologies and least significant for basic water supply. The authors also find evidence for the Mills-Reincke Multiplier for both water and sanitation access as well as positive health externalities for sanitation investments. The overall magnitude of the estimated effects appears smaller than coefficients reported in meta-studies based on randomized field trials, suggesting limits to the scalability and sustainability of the health benefits associated with water and sanitation interventions.

Book excerpt: In this paper we examine the impacts on child health, using diarrhoea as the health outcome, (amongst children living in households) with access to different types of water and sanitation facilities, and from socio-economic and child specific factors. Using cross-sectional health survey data for India, we employ the propensity score method to match children belonging to different treatment groups, defined by water types and sanitation facilities, with children in a control group. We also employ non-matching techniques to compare our results and to check for their robustness. Our results indicate that disease-specific awareness has strong marginal effects on reducing the predicted probabilities of diarrhoeal outcomes in young children, which are consistent across the models utilised. We also find disease-specific awareness to have the largest impact on reducing the burden of disease from diarrhoea across a select group of predictors. -- Diarrhoea ; Water ; Sanitation ; Propensity Score ; Matching Techniques

Ample evidence shows that poor water and sanitation causes diarrhea, but there is a growing body of knowledge discussing the magnitude of the impact of diarrhea on undernutrition. A recent hypothesis by Humphrey (2009), for example, states that the predominant impact of contaminated water and poor sanitation on undernutrition is via tropical/environmental enteropathy (triggered by exposure to fecal matter) rather than mediated by diarrhea. This new hypothesis has generated much debate, especially in the South Asia region, on the contribution of water and sanitation to the South Asian Nutrition Enigma. The region is characterized by unusually high rates of child undernutrition relative to its income level, as well as a slow reduction in undernutrition. Practitioners have struggled to decipher the reasons behind this 'anomaly.' This report provides a systematic review of the evidence to date, both published and grey literature, on the relationship between water and sanitation and nutrition. We also survey the potential impact of improved water, sanitation, and hygiene (WASH) on undernutrition. This is the first report that undertakes a thorough review and discussion of WASH and nutrition in Bangladesh. The report is meant to serve two purposes. First, it synthesizes the results/evidence evolving on the pathway of WASH and undernutrition for use by practitioners working in the nutrition and water and sanitation sectors to stimulate technical discussions and effective collaboration among stakeholders. Second, this report serves as an advocacy tool, primarily for policy makers, to assist them in formulating a multisectoral approach to tackling the undernutrition problem.

Each year, 1 in 10 child deathsroughly 800,000 in totalis the direct result of diarrhea. Can private-sector participation (PSP) in the urban piped water sector improve child health? The author uses child-level data from 39 African countries during 19862010 to show that introducing PSP decreases diarrhea among urban dwelling children under five years of age by 5.6 percentage points, or 35 percent of its mean prevalence. PSP also leads to greater reliance on piped water. To attribute causality, the author exploits time variation in the private water market share controlled by African countries former colonizers. A placebo analysis reveals that PSP does not affect symptoms of respiratory illness in the same children, nor does it affect a rural control group unaffected by PSP.

There has been a reduction by one-third over the past 20 years. In part related to increase in safe deliveries by skilled personnel, reduced fertility and antenatal care. Unfortunately the burden of mortality and morbidity still falls disproportionately on the poor and remains a great challenge in low resourced countries. This book is intended for policy makers and primary healthcare practitioners working in low resource environments as a guide for promoting hygiene in maternal child health services.
